What the care home does well

There was a relaxed friendly atmosphere in the home and residents and staff were welcoming to visitors. The home is bright, airy and decorated in a homely manner, providing residents with a pleasant place in which to live. Clear and comprehensive instruction is available for staff to ensure that individual needs may be met, and these "care plans" are regularly reviewed to ensure they are up to date. Residents are encouraged to be involved in the local community and activities suitable for their age group. For example sailing, visits to the shops and cinema and attendance at the local college. All residents attend day care provided by the company or other organisations, where they are involved in various activities. One resident spoken to was looking forward to attending a "Westlife" concert at the National Exhibition Centre.

What has improved since the last inspection?

Residents have all been offered the use of a lockable box in which to keep personal items, although some have declined this offer. One bathroom and two resident rooms have been redecorated.

(2 Windsor Avenue) unless that person also falls within the category of Learning Disability (LD) i.e. dual disability 8th December 2005 Date of last inspection Brief Description of the Service: 2 Windsor Avenue is one of two care homes situated in the small town of Desborough, which are owned and operated by Richard and Heather Mullin under the umbrella name of HR Care. The home accommodates five young people with learning disability as their primary diagnosis and all the young people are additionally wheelchair users. The home offers personal care but does not provide nursing care and will not accommodate young people who are known to be physically aggressive to other service users or staff. The home is a converted dormer bungalow on the outskirts of the town, which has been extensively and tastefully adapted to fully meet the needs of the client group. A wide range of moving and handling aids, including hoist tracking systems in bedrooms and bathrooms, and specialist bathing and showering, have been installed. The home also has an attractive, enclosed garden, which is fully accessible to all the young people accommodated at the home and has a lawn and large pond. The staff at the home facilitate access to a good range of education, leisure and entertainment opportunities for young people and the home has its own adapted mini bus to provide appropriate transport. All bedrooms at the home are large and have en suite facilities (one facility is shared by two young people) and shared areas consist of a large lounge, dining area and conservatory. The home is exceptionally clean and well appointed and provides a comfortable, homely environment for the young people. Two hours were spent prior to the inspection reviewing previous requirements and recommendations and collating information provided by the service. The Commission sent comment cards out to a random selection of residents and to General Practices providing a service to the home but no responses had been received to these. The inspection took place over a period of two and a half hours as part of the statutory inspection programme. Two residents were chosen in order that their experience in the home could be assessed. The method used was “Case Tracking”. This involved looking at their records, talking to them and observing the care received. In addition to this staff rotas and medication records were seen. Information was available from a questionnaire completed by the providers of the service. Fees range from £1.700 to £2000 per week, according to resident’s assessed needs. EVIDENCE: Comprehensive assessments of care needs were seen in the resident files sampled, and plans of care formulated to enable staff to meet assessed needs. This judgement has been made using available evidence, including a visit to the service. Good systems are in place to ensure that all aspects of resident need are identified and documented for staff guidance, to ensure that these needs may be met in the home. Individual choices are respected and residents are assisted to lead as independent life as possible within the confines of their disabilities. EVIDENCE: Two residents were chosen in order that their experience in the home could be assessed. This involved looking at their records, talking to them and observing staff interaction as well as looking at the environment. Residents had detailed assessments in place and care plans had been formulated to guide staff on how to meet the assessed needs. These care plans were detailed and specific, regularly reviewed and made reference to individual choices concerning lifestyle, hobbies and interests. Monthly meetings are held with the resident and their key worker where plans may be discussed and problems highlighted. Residents spoken to expressed their satisfaction at the care and attention received, confirmed that they had choice concerning daily life and that they liked living in the home. Residents were well groomed and appeared well cared for and were dressed in clothing appropriate to their age group. This judgement has been made using available evidence, including a visit to the service. Social Activities and meals are both well managed, creative and provide daily interest and variation for people living in the home. EVIDENCE: All residents attend day care facilities, either provided by the company or other organisations, where they may be involved in various activities. Both residents spoken to confirmed that they enjoyed this and were looking forward to attending on the day of the inspection. Two of the residents are currently attending a local college once a week, where they are involved in an Arts and Crafts course. Records demonstrate that activities such as sailing, bowling, shopping and visits to the cinema are organised. One resident spoken to stated that she was going to a “Westlife” concert at the National Exhibition Centre. All of the residents had been away on holiday this year. Some to Hunstanton with staff from the home, to a new disabled facility there and others to holidays arranged by other organisations. Visitors are welcome in the home and records of visits by family and friends were seen in the case notes chosen. One of these residents has regular home visits where he stays for a few days. They also confirmed that they were given a choice of food and that the standard of food was good. One resident stated that he particularly likes cheese. Menus provided as part of the pre- inspection questionnaire completed by the providers, demonstrated that a varied, nutritious diet is offered. Suitable aids and adaptations are available to assist residents at meal times. One resident who was having a late breakfast was observed to be assisted in a sensitive manner. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. This judgement has been made using available evidence, including a visit to the service. Resident’s physical and emotional needs are met in the way they prefer. There is a safe system in place for administration of medication. EVIDENCE: Care plans demonstrated clear individualised guidance for staff in meeting resident’s needs. None of the residents currently attend a place of worship, but this is from choice. One of the residents used to attend church and a minister from the local church has made contact to offer her services where required. Evidence of visits from health care professional and appointments with hospital specialists was seen in the files examined. None of the residents is able to administer their own medication owing to their disabilities, but systems for the ordering, storage, administration and disposal of medication in the home were seen and found to be satisfactory. This judgement has been made using available evidence, including a visit to the service. The company takes all complaints seriously and acts upon them. Residents are protected from abuse. EVIDENCE: There have been no complaints received by the Commission for Social Care Inspection since the last inspection and none recorded in the home. A complaints procedure is available in the home and residents are offered the chance to raise any concerns at their monthly review meetings. Staff training records demonstrate that they have received training in the protection of vulnerable adults from abuse. JUDGEMENT – we looked at outcomes for the following standard(s): 24, 25, 26, 27, 28, 29 and 30 Quality in this outcome group is excellent. This judgement has been made using available evidence, including a visit to the service. A safe, clean, well-maintained environment is provided for the residents. Specialist equipment is provided to meet their needs. EVIDENCE: A limited tour of the environment was undertaken. This demonstrated that the home is decorated and furnished to a high standard, creating a homely environment in which to live. Accommodation provided makes provision for the physical disability of the residents, with specialist beds, hoists and bathing facilities being provided. Resident rooms showed evidence of personalisation, with personal pictures, posters and electrical equipment on display. A communal lounge and dining area is provided as well as a conservatory and pleasant garden. This judgement has been made using available evidence, including a visit to the service. Procedures for the recruitment of staff provide safeguards necessary to offer protection to the people living in the home. Staff are provided with training and in sufficient numbers to meet the needs of the residents. EVIDENCE: A selection of staff files and training records were seen. These demonstrated that recruitment procedures are in place to protect residents from potential harm. This includes Criminal Records Bureau checks as well as taking up two references for each staff member. The staff training records demonstrated that 10 out of the 17 care staff employed hold a National Vocational Qualification in care, which gives them a basic understanding of care needs, and a further three are working towards this qualification. This exceeds the required minimum of 50 of staff with this qualification. This judgement has been made using available evidence, including a visit to the service. The Registered Manager ensures that the home is run efficiently, in a way that serves the best interests of the residents. EVIDENCE: The Registered Manager has several years experience of caring for this client group and holds the Registered Manager’s Award, which gives an understanding of management issues in the care setting. In order to monitor standards within the home, she carries out monthly audits of all systems within the home and works on the floor on alternate weekends in order to maintain contact with the residents and staff. Monthly meetings are held with individual residents and their key worker to ensure their views are taken into consideration. Records of the testing of fire alarms, emergency lighting and fire drills were seen and found to be satisfactory.
