Conditions of registration: 1. No additional conditions of registration apply. Date of last inspection 19th July 2007 Brief Description of the Service: 97 Albert Promenade (also known as Cliffe House) is registered to provide care for three people who have learning disabilities. The owners, Mr and Mrs Cliffe, also live at the property. The home is close to local shops, pubs and bus stops. Whilst three bedrooms are available, two people currently share a room by choice. There are two lounges and a dining room, which are shared by people who use the service and the owners. There is an attractive patio garden to the rear of the property. At the time of the inspection, fees ranged from £367 to £1,003 per week. Copies of the home’s Statement of Purpose and Guide are available from the owners on request as is a copy of our last inspection report. This is an overview of what the inspector found during the inspection. The quality rating for this service is 2 stars. This means the people who use this service experience good quality outcomes.
A visit to the home took place on 30th June 2008, lasting approximately five and a half hours. The main method of inspection used on that day was ‘case tracking’ which involved selecting two people who use the service and tracking the care they receive through review of their records, discussion with them & the owners and observation of care practices. All three of the people who use the service were spoken to during the course of the visit. The inspection also took account of all information received since the date of the last visit. Comment cards were received from one person who lives at Albert Promenade and one member of staff. The owners’ annual quality assurance assessment was not available at the time of writing this report. What the service does well: What has improved since the last inspection?

Bank accounts have been opened for two people who were identified at the time of the last inspection. The owners have produced written guidance on what people should fund from their personal allowances and what is to be funded by the home. Improvements have been made to the way in which medication is stored & administered. Medication was stored in its original packaging at the time of the visit. Records of administered medicines met relevant requirements. Portable electrical appliances have been tested since the date of the last inspection. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 1&2 Quality in this outcome area is adequate. Information about the home is only available in a standard written format and is therefore not accessible to all of the people who live at Albert Promenade. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Information about Albert Promenade was available at the time of the visit. The person who completed a comment card indicated that they had received enough information about the home before they moved in. Mrs Cliffe (one of the registered providers) stated that she had made arrangements for the documents to be produced in a more accessible format (a recommendation from the last inspection) but that she had yet to receive the results. She agreed to amend several documents identified at the time of the visit to reflect our change of address. The outcome for Standard 2 (Needs Assessment) could not be assessed on this occasion. The people who were chosen for the purposes of case tracking have lived at Albert Promenade for fourteen & twenty-two years respectively. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7 & 9 Quality in this outcome area is good. Staff members have the information they require to meet individual needs. Improvements have been made to the way in which people’s finances are managed to ensure that their best interests in this area are protected. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Individual plans were available for the people who were chosen for the purposes of case tracking. The plans contained detailed biographical information and set out individual likes & dislikes. They emphasised people’s strengths as well as documenting their needs. Person centred plans were also available for each person, setting out the relationships which are important to them. Risk assessments have been completed and the reasons documented for any restrictions placed on behaviour. Improvements have been made to the way in which people’s finances are managed since the date of the last inspection. Bank accounts have been opened for two people who were identified at the time of that visit. The registered providers have produced written guidance on what people should fund from their personal allowances and what is to be funded by the home.

JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 15, 16 & 17 Quality in this outcome area is good. The wellbeing of the people who live at Albert Promenade is promoted through their involvement in meaningful daytime activities. They enjoy good food in pleasant surroundings. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The people who live at Albert Promenade attend college, work at a recycling project and undertake one-to-one activities with staff members on different days of the week. They stated that they could decide how to spend their free time, reporting that they like to go to the shops, cafes and the park. Two of them stated that they attend religious meetings with Mr & Mrs Cliffe (the registered providers), which they enjoy. The people who live at Albert Promenade indicated that they benefit from positive relationships with each other and staff members. None of them have any contact with family members. Mr Cliffe is a trained chef and cooks the meals. Records indicate that people at the home enjoy a varied diet. Mr & Mrs Cliffe agreed to keep a record of all the meals that are provided, including any alternatives to the main menu. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19 & 20 Quality in this outcome area is good. People’s personal support & healthcare needs are met in a manner which promotes their dignity & privacy. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The people who live at Albert Promenade stated that they are happy with the care they receive from staff members. They reported that either Mr or Mrs Cliffe provides support with their personal care depending on their gender. The records that were inspected detailed the personal care each person requires. They also set out details of any healthcare needs that have been identified and of any action that is felt to be necessary as a result. Records of the healthcare appointments attended by each person indicate that they have access to community nurses, doctors, dentists, opticians & podiatrists as required. Improvements have been made to the way in which medication is stored & administered since the date of the last inspection. Medication was stored in its original packaging at the time of the visit. Records of administered medicines met relevant requirements. The medication cabinet is situated in a toilet / shower room, thus potentially exposing tablets to high temperatures & humidity. A recommendation has been made that the advice of a pharmacist should be sought regarding the location of the cabinet.

The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 22 & 23 Quality in this outcome area is good. The people who live at Albert Promenade are able to express their views and are protected by current practice in the home. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The people who live at Albert Promenade stated that they enjoy a good relationship with all staff members, particularly Mr & Mrs Cliffe. This was evident on the day of the visit. A comment card indicated that staff members listen and act on what people say and that the person who completed it knows how to make a complaint. A complaints procedure was available at the time of the visit. Mrs Cliffe stated that she hoped to produce this in a more accessible format (as recommended at the time of the last inspection). She agreed to amend the procedure to reflect our change of address and to include contact details for Social Services. She reported that no complaints had been received since the date of the last visit. The home has policies and procedures on the protection of vulnerable adults and whistle blowing. Mrs Cliffe agreed to amend the policy on abuse to reflect the role of Social Services. She stated that she still hoped to access training provided by the local authority on safeguarding adults (as recommended at the time of the last inspection). Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 24 & 30 Quality in this outcome area is good. The wellbeing of people who use the service is enhanced by living in a clean and comfortable environment. This judgement has been made using available evidence including a visit to this service. EVIDENCE: People stated that they are happy with the environment in which they live. The person who completed a comment card indicated that the home is always fresh & clean. The property is decorated and furnished to a very good standard. Two people currently share a bedroom. A third bedroom was being decorated and furnished at the time of the visit to enable both to have single rooms if they wish. There is an attractive garden to the rear of the property, which is accessible to all. People stated that they enjoy having their tea outside in the summer. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34 & 35 Quality in this outcome area is good. The people who live at Albert Promenade are protected by the home’s arrangements for recruiting and training its staff. This judgement has been made using available evidence including a visit to this service. EVIDENCE: People who live at Albert Promenade receive support from a small and stable team of staff. Mrs Cliffe stated that no new members of staff have been employed since the date of the last inspection. The records that were inspected indicated that appropriate pre-employment checks had been carried out for two existing members of staff. Mrs Cliffe has completed National Vocational Qualifications at levels 2 and 4. She stated that three of the four members of care staff have completed National Vocational Qualification level 2. Records indicate that staff members have received training on issues such as first aid, food hygiene, medication & record keeping. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 39 & 42 Quality in this outcome area is good. Improvements have been made to the overall running of the home ensuring that the needs of the people who live there are met and their best interests protected. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Improvements have been made to the overall running of the home, particularly in relation to the management of people’s finances and the storage & administration of medication. Mrs Cliffe has completed the Registered Managers Award. Mr & Mrs Cliffe and their staff continue to attend relevant training courses to update their skills. Records indicate that the people who live at Albert Promenade completed a satisfaction survey during July 2007. Mrs Cliffe stated that she intends to produce a more accessible survey form (as recommended at the time of the last inspection). She reported that she had sent surveys to other stakeholders but had not received a response.

At the time of this visit, Mr Cliffe stated that he had still not completed some of the tasks, including reviewing the fire risk assessment and replacing all fire doors. A requirement was made at the time of the last inspection that the registered person must ensure that requirements made by the fire authority are met. This issue has been referred back to Leicestershire Fire Service. Records indicate that the fire alarm system is tested regularly and that portable electrical appliances have been inspected since the date of the last visit.
