Conditions of registration: 1. That included in the numbers are provision for two people with physical disabilities aged 50 years and over. 5th July 2006 Date of last inspection Brief Description of the Service: The home is located in the Derbyshire countryside close to the town of Belper. The home has 40 places - all of which are in single rooms; half of these have en-suite facilities. The home consists of an older building with a larger extension, which has been built into the embankment. The lounge and dining rooms are situated in the older part of the home, and include a conservatory that is used every day by residents and is a popular meeting place. The size and shape of the bedrooms are varied and dependent on the location within the home. The weekly fee range provided at the time of this inspection visit ranged from £ 400.00 to £ 650.00 Items included in this fee were: A bathrobe, bath and hand towels and face towels and fresh flowers each week if required. Items not covered by this fee included: Toiletries (but home will purchase for individual if required), Hairdressing services, Reflexology, Private Manicure, Private physiotherapy and Newspapers / magazines. This is an overview of what the inspector found during the inspection. The quality rating for this service is 3 star. This means the people who use this service experience excellent quality outcomes.
This key inspection was unannounced and took place over approximately seven hours. Key inspections take into account a wide range of information and commence before the site visit by examining previous reports and information such as any reported incidents. The site visit is used to see how the service is performing in practice and to meet with the people using the service. The focus of inspections undertaken by the Commission for Social Care Inspection is upon outcomes for service users and their views on the service provided. This process considers the provider’s and registered manager’s capacity to meet regulatory requirements, minimum standards of practice; and focuses on aspects of service provision that need further development. An Annual Quality Assurance Assessment (AQAA) had been completed by the service. This is a self-assessment for providers, which is a legal requirement. This assessment gives the provider and registered manager an opportunity to let us know about their service and how well they think they are performing. The information provided in the AQAA is reflected within this report. At this inspection visit two people were case tracked. Case tracking is a method used to track the care of individuals from the assessments undertaken before they are admitted to a service through to the care and support they receive on a daily basis. This includes looking at care plans and other documents relating to that persons care, talking to staff regarding the care they provide and if possible talking to the individual. Both people case tracked were able to express their views of the service and the support it provided. Other people living at the home that were not case tracked were also spoken with. Their views of the service and the care and support provided are included within this report. Two members of staff were spoken with at some length and their views and opinions of the care provided, the support and training given to them is included within this report. The comments provided with seven surveys received from the people living at the home, seven relatives/ representatives’ surveys and three staff surveys are also reflected throughout this report. The comments within these surveys have also been incorporated within this report. The registered manager and homes administration manager were on duty on the day of this inspection and provided the relevant information requested. From the positive comments made by both people living at the home, their relatives and the staff team, it is apparent that the home is run in the best interests of the people living there. It should continue its good practice of analysing the care and service provision and striving to continually improve to yet higher standards of achievement.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. People can be confident that the home can support then, as accurate assessments are undertaken before admission is agreed. EVIDENCE: The people case tracked had been assessed before admission was agreed. This ensured the home were able to meet their needs. The registered manager or deputy manager undertook assessments and the information seen within these assessments addressed all areas of health, personal and social needs and preferences. This ensured that each person’s specific needs were identified to enable plans of care to be put in place to ensure they could be met. The assessments also identified any religious, spiritual or cultural beliefs. The registered manager was then able to ensure these beliefs and preferences could also be met. Relatives comments regarding the admission process were positive and indicated that the admission process was handled in a sensitive and friendly way by the staff. One person said “our family situation was very difficult when my grandparents came to the home. The staff were amazing, very understanding, helpful and sympathetic”. Information provided by the people living at the home confirmed that they or their relatives were given sufficient information before they moved into the home to enable them to make an informed decision about living there. The records seen also demonstrated that on admission further information was obtained about each person. This was called the Service User Profile and provided staff with important information that enabled them to get to know each person better and the support they needed. It looked in more detail at individual’s family and life history, including their working years, their interests, hobbies and beliefs and their preferences regarding meals and beverages and their daily routines. Records were in place to show that care reviews were undertaken every six months or sooner if necessary. This enabled the individual, their family and others involved in their care to discuss the support given and address any changing needs. Many of the people living at the home confirmed that they were invited to six monthly reviews and found them very useful. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The health, personal and social care needs of individuals was met and people were supported to take their medicines in a safe way. EVIDENCE: The records of care and support for the two people case tracked were informative and addressed all areas of assessed need including any health conditions and prescribed medications. The information within these plans of care had been reviewed each month or sooner if needs had changed. The communication between the people living at the home and the staff that worked there was good. People were involved and included in the way they were supported. Comments from people living at the home confirmed that they were involved in the planning of their care and many were able to confirm who their key worker was and the role of their key worker. These assessments addressed any areas of risk that had been identified through the support plans. For example areas such as mobility, including risk of falls, pressure area care, safe use of bed rails, social inclusion and community access and activities. Other assessments were also in place to ensure people’s safety was maintained such as nutritional assessments, prevention of falls from heights and dependency assessments. Health care needs were clearly addressed and this was demonstrated in the records seen of the people case tracked. Everyone had access to healthcare such as G.P services, chiropodist, dentist and optician. People living at the home confirmed this. One person said, “ The doctor is always available”. Alternative therapies were also available for those people who wanted them and were provided by an alternative therapist. One person spoken with said, “ I have an Indian head massage, shoulder and foot massage every week. Its good for me and I really enjoy, plus it’s very relaxing”. Relative’s comments indicated that they were happy with the care and support provided,” After nearly two years of visiting my mother I am full of admiration for the professionalism and quality of care provided by the staff at Bankwood” and “ the care he has is the next best thing to being at home”. People were supported to take their medication safely. The medication records of the two people case tracked were looked at and no errors were noted. Records had been completed accurately. Medications were stored correctly and all records seen were clear and accurate. Staff were observed with some of the people living at the home and demonstrated a caring and friendly approach. People were observed to be supported in a respectful and dignified way. One relative commented, “They are kind, considerate and caring and treat residents with great dignity”. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. People are treated as individuals and the home is responsive to their preferences and beliefs. People are able to keep in touch with their family, friends and representatives and have nutritious and attractive meals that are enjoyed. EVIDENCE: An activities planner was in place and demonstrated that creative thinking went into providing interesting activities that stimulated the mind, were fun and relaxing. The activities planner included notable dates for a variety of religious faiths as well as seasonal events associated with each month. This demonstrated that the home was responsive to different cultures and religions and promoted individuals awareness and memory regarding the time of the year and associated events. The activities coordinator attended the residents meetings that were held every three months. She confirmed that this gave her further opportunity to discuss any new ideas that she or the people living at the home had.
Bankwood Care Home ( Ashbourne Senior Living) DS0000058023.V368303.R01.S.doc Version 5.2 Page 14 Activities were provided in a group format such as quizzes, word association and memory games and discussion time, where visual and musical props were used. There was also several one to activities such as letter writing and reading, hand and nail manicure, reflexology and hand massages. ‘Ladies’ and ‘gents’ time was also an activity and these focused on specific interests of both sexes. The activities coordinator discussed the makeover session that had been provided by Boots. As stated in standards 7-11, alternative therapies were also available for those people who wanted them. The alternative therapist was providing these treatments on the day of the inspection visit. The activities coordinator and registered manager discussed further developments with regards to the in house activities that were available. This included transforming the activities room into a cinema. Equipment was in the process of being purchased for this and enamel memorabilia prints had been purchased for ‘the cinema’. People living at the home said the activities provided were good. They confirmed that they were varied and were given the option to participate, or not depending on their choice. Outside entertainers also visited the home on a monthly basis. People spoken with confirmed that this entertainment was very good and enjoyed. Community access included trips to garden centres, places of interest and holidays in Blackpool. One person said “ I enjoy going out on trips to the pub and shopping.” One person spoken with confirmed that they had been on a summer holiday in Blackpool for the last three years and thoroughly enjoyed each one. Saying, “ it’s a fantastic holiday the entertainment is excellent, I really enjoyed it. We have our own carers which I prefer as I know them”. People’s cultural needs were met as Church services were held at the home on the last Wednesday of each month. On the day of the inspection visit two members of a local faith group were providing musical entertainment accompanied by the guitar. Several people joined in with the hymn singing. The manager also confirmed that another faith group were providing a service at the weekend for anyone who wished to join in. One of the people spoken with said they would be attending this service, as this was their religious denomination. People were able to keep in touch with family and friends. This was through the open visiting at the home and through the letter writing support provided, as mentioned earlier.

Many positive comments regarding the staff team were made, such as “All the staff make everyone feel very welcome and put everyone at ease. The atmosphere is very happy and it is made to feel like your own home. I know my mother is well cared for and now can feel totally relaxed about her. I really enjoy coming to visit her.” Comments from the majority of people living at the home indicated that they were able to make choices in their daily lives such as the time they got up and the time they went to bed. One person did indicate within a survey that they usually were able to decide when to go to bed but stated this was ‘not always’. Unfortunately the identity of this person was not disclosed; therefore this could not be discussed further. The people living at the home enjoyed the meals provided. All of the people spoken with and comments within surveys confirmed this. Comments included, “the meals are fantastic” and the meals are lovely, we have an excellent cook”. The lunchtime meal was sampled and was found to be of a good quality. It was observed that people were offered a glass of wine with the lunchtime meal. It was confirmed by a person living at the home this was common practice. The home operated an eight-week rolling menu. People spoken with confirmed that they were able to order their meals in advance. Daily meal order sheets were given out for people to place their orders. Menu cards were also available on the dining room tables and were given out to people who preferred to eat in their rooms or at a table by their armchairs. Both the order sheets and menus indicated a choice of meal. A new menu system has been implemented since the last inspection. This system is known as the NUTMEG menu. Discussions with the head chef confirmed that this menu was aimed at people over the age of 70, and provided a high nutrition diet, which included snacks and fruit in between meals. This demonstrates that the home is proactive in providing nutritionally balanced diets to promote and maintain individuals health. It was confirmed that there were hundreds of recipes available within the NUTMEG menu to ensure a varied choice was available. The NUTMEG menu however was adapted in certain circumstances where people required a specific diet for health care reasons. This ensured that everyone’s individual needs were met. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Concerns are looked into and actions are taken to put things right and the practices in place safeguard people from abuse and neglect. EVIDENCE: The complaints procedure was displayed and available to the people living at the home and their relatives. It was noted that the complaints procedure directed people to the Commissions previous local office contact details. This was discussed with the registered manager and following this inspection it has been confirmed that this has been amended to show the present Regional contact details. On discussing complaints with individuals, they commented that that they were aware of the complaints procedure and were happy with the care they received and had no complaints. Relative’s comments reflected this also, additional comments made included, “ any problems and I’m told promptly, it gives me peace of mind knowing my husband is well cared for”. The service had received one complaint since the last inspection visit. This related to the facilities available within someone’s private accommodation. Records demonstrated that this had been addressed satisfactorily. We have received no complaints regarding the service since the last inspection.

Out of the six staff that had not undertaken this training in the last year three had attended this training within the last two years. This indicates that the majority of the staff team are aware of the correct procedures to be followed in the event of any concerns or allegations of abuse. Staff spoken with demonstrated a good understanding of the procedures that were to be followed to ensure people at the home were kept safe. Comments from people living at the home indicated that they were safeguarded appropriately and included, “I feel safe and relaxed here”. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19,26. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home provides sufficient space and facilities to meet needs and preferences and is kept to a good standard to ensure peoples safety and wellbeing is maintained. EVIDENCE: The home provided a safe, well-maintained environment for the people living there. All of the environmental repairs identified at the last inspection had been undertaken. Additional refurbishment had been undertaken such as the hair salon, which had been tastefully redecorated. Bankwood Care Home ( Ashbourne Senior Living) DS0000058023.V368303.R01.S.doc Version 5.2 Page 19 Hall carpets were due to be replaced from the office to the conservatory and, as stated earlier in this report, the activities room was undergoing refurbishment to provide a cinema area, for the people living at the home. The dining room was tastefully decorated. It was noted that this area would not accommodate everyone living at the home, if they chose to eat in the dining area. However it was confirmed by the manager and by people spoken with that many people chose to take their meals either in their room or seated within other communal areas of the home. The laundry area was seen and housed sufficient equipment to meet disinfection standards and maintain individuals laundry requirements. Comments regarding the laundry were generally positive and most people were happy with the service provided. However one comment within a survey was made regarding items of missing clothing, however it has been confirmed that these items of clothing were discovered to be missing at this person’s previous home, prior to moving to Bankwood care home. At the time of this inspection, plans were in place to install a new lift. One person spoken with stated that this had affected them, as they had been required to temporarily vacate their private accommodation and move to another room. On discussion it was confirmed that this person was satisfied with their temporary accommodation but said they were looking forward to returning to their own room, as it was more spacious. People spoken with were happy with the overall maintenance and hygiene standards maintained at the home. Comments included, “it’s beautiful here and always clean and fresh” and “ its kept clean and tidy and the views are magnificent”. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. People have safe and appropriate support as there are enough competent staff on duty at all times. Appropriate staff checks ensure individual’s safety is maintained EVIDENCE: The rotas demonstrated that eight staff were rostered on shift throughout the morning, two of these were nurses and seven during the afternoon and evening, again two were nurses. At night four staff were on duty, one being a nurse. Staff spoken with generally indicated that staffing levels were satisfactory in ensuring the needs of individuals were met. It was confirmed that several of the staff team lived within the grounds of the home and an agreement had been made for them to provide on call cover if required. The people living at the home indicated that there was enough staff available when they needed them. From observation there appeared to be sufficient staff within the communal areas to assist individuals as required/requested.

All of the information required by law was in place within these documents and this ensured the safety of the people living at the home was maintained. The training records were seen and demonstrated that staff training was kept up to date and refresher courses were provided as required. Staff spoken with confirmed that the training provided was regular and useful in developing their skills and knowledge. This enabled them to provide a good standard of care to the people living at the home. The records demonstrated that out of twenty-one care staff, six senior carers had a National Vocational Qualification (NVQ) in care at level 2. This included two team leaders who were in the process of completing an NVQ at level 3. In addition to this an additional three care staff were in the process of completing their NVQ in care at level 2. This demonstrates that the service is on track to meet the national targets, which are set at a minimum of 50 of the care staff team having achieved an NVQ at level 2 or equivalent. The activities coordinator was also doing an NVQ 2 in activity management, which in turn would enhance her skills and knowledge, enabling her to provide an even greater variety of activities for the people using the service. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,35,38. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. People have confidence in the service provided because the home is led and managed appropriately, with peoples health and safety maintained and finances safeguarded. EVIDENCE: The registered manager had been in post for sometime and was well established. Since the last inspection the registered manager has achieved an NVQ 4 in care management. Staff, relatives and people living at the home were all extremely complimentary regarding the registered managers skills and ability in running the home.

They also confirmed that they received regular supervision and this was seen in the staff training files examined. People living at the home spoke warmly regarding the registered manager saying, “ She’s a lovely woman, so caring”. Relative’s comments indicated that the management of the home was good at communicating any concerns and at ensuring their views and opinions were sought. This demonstrates that an open and inclusive approach was in place in the running of the home. One relative commented on the relatives meetings that were held every three months. Saying, “the relatives meetings held every three months is a wonderful idea and most informative. It enables you to get to know other relatives”. Meetings for the people living at the home were held each month and those people spoken with said that they attended and enjoyed these meetings as it gave them an opportunity to ‘have their say’. As stated earlier in this report the activities coordinator also attended these meetings so that any ideas or suggestions regarding activities and social events could be discussed. Staff meetings were also held each month and this ensured that communication between the staff team was promoted. It also gave the staff an opportunity to discuss any general issues or ideas they had. The staff spoken with indicated that staff meetings were useful in keeping them up to date with any changes. Prior to this inspection visit questionnaires had been sent out by the home to all of the people living at Bankwood Care. These questionnaires addressed many different areas of care and services provided, such as, the information provided in the service user guide, the role of the key workers, the food and catering service, the housekeeping and laundry service, the staff team and a visiting survey aimed at visitors and their experiences of visiting the home. This information will assist the manager and operation manager to measure the home against expected outcomes. Records of the monies held by the home for the people case tracked were seen, and demonstrated that the accounting procedures for individual’s personal finances was managed in a safe way. This demonstrated that individual’s monies were managed in their best interests. The environment is safe for people because the appropriate health and safety checks are carried out. All but one service certificate was up to date. The 5 yearly electrical premises certificate in place was dated November 2002. It was confirmed that this had been undertaken in April 2008 but to date the home had not received a copy of the certificate. However a copy was forwarded to the commission upon receipt.
