A visit was made to this home on may 13th 2010. We found that the service had very good pre assessment records in place for two new residents: these records contained information that related to the needs of the prospective residents and any risks associated with their behaviour. Both residents had informative care plans that related to their assessed needs, these care plans also related to the the risk assessments that had been carried out. The care plans were person centered, up to date and there was evidence of evaluation of the care and changes being made when needs altered. The care plans contained risk assessments that were associated with known behaviour; there was evidence in the care planning to show that the two residents had been involved with their care planning. There were good daily notes that contained relevant information about the residents and if any changes in their mental health on a day to day basis. The risk assessments also contained information about what support was needed for trips out and how many escorts required to provide the support and to counteract any unnecessary risk taking by the residents. The medication records for both residents were in good order and a spot audit carried out revealed that all medicines that had been signed for as having been given tallied with those medicines left. We spoke with one of the residents who told us that they liked living in the home and had made the right choice. They told us about all the activities that they had been doing which were also clearly recorded in their notes. This resident told us dont worry about me I am fine. We noted that the home had suitable policies and procedures in place for all trips and what was required for escort duties.
