The Care Centre is part of a new build complex, which includes Gravesend Community Hospital and a day care centre. Kent County Council work closely with the Primary Care Trust, who run the hospital and day centre, and who supply nurses for one of the Care Centre units which provides nursing care.
Annual Service Review Page 2 of 7 1 5 0 1 2 0 0 9 It is situated in the centre of Gravesend, near to shopping and other town facilities, and within walking distance of the river and public parks. Accommodation is provided in single rooms with en suite toilet and shower facilities. The Care Centre provides health and social care for up to eighty older people in four units (Emerald, Topaz, Opal 1 and Opal 2), on three floors. All floors can be accessed via a large passenger lift. Care is provided for older people with residential care needs; older people with residential and dementia care needs; and older people with nursing needs. Most beds are for permanent care, but the Centre also provides short term respite beds for older people with dementia. The building is a Public Finance Initiative (PFI), which is run and maintained by Grosvenor Facilities. It is very advantageous for the residents to have many health facilities on site, such as dentist, physiotherapy, X ray and out patient clinics. Contracts are arranged via Kent County Council. The maximum fee is currently 379.02 pounds per week. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review:
What did we do for this annual service review? Gravesham Place Integrated Care Centre had a key inspection carried out on 15th January 2009, and was rated as an excellent service at that inspection. The Inspector has reviewed all the information that the Commission has received, or asked for, since the last key inspection. This includes: Formal notifications about things which have happened in the service, which the care home is legally required to send to CQC. The Annual Quality Assurance Assessment (AQAA), which was sent in by the service. AQAA is a self assessment that focuses on how well the centre is meeting outcomes for people who live there. It also provides detailed information about how the centre is running. Information about any concerns or complaints; or referrals to the Social Services Safeguarding Adults department. Details from the last key inspection and report. Relevant information obtained from other organisations. Information obtained from phone calls and letters received since the last inspection. What has this told us about the service? The last key inspection found that the home was providing an excellent service. There is a strong emphasis on ensuring that residents are listened to, and that their views and ideas are taken into account. An AQAA self-assessment was completed by the manager within a month of this review; and it was returned to us within the required timescale. The AQAA makes it clear that residents views are still of the utmost importance, and form the basis for changes within the centre. Residents meetings are held at least quarterly, and residents are encouraged to express their views in regards to all aspects of the service and care given. A complaints/compliments/suggestions book has been placed in the main reception area so that people can easily share their views. The manager monitors this on a weekly basis. Questionnaires are regularly undertaken by the centre and by Grosvenor Facilities, who are the managers for the building. The centre provides good information for prospective residents, including the statement of purpose and the service users guide. This documentation is going to be updated in the next year. The manager states that current residents will help to provide insight and assistance in making sure that this documentation is helpful and effective. All residents have a pre-admission assessment prior to coming in to the centre; and all residents have reviews with their care managers within four weeks of admission. Annual Service Review Page 4 of 7 The provision of health care is enhanced by having the Gravesend Community Hospital within the same building. This means that there is easy access to all health care facilities and professionals, such as physiotherapist, speech therapist, dentist, dietician and optician. Residents are invited to discuss their care plans; and the plans provide details of all aspects of the care to be given. There has been additional training for staff during the last year in managing end of life care. The centre is divided into different units according to residents assessed needs; and activities are planned which are suitable for people with nursing needs and people with dementia. There is a sensory room in the dementia unit, which provides a quiet atmosphere for residents who are feeling under stress. Residents are enabled to go out to different local places in good weather; and entertainment is provided on a regular basis. There are good links with the local community, including visits from ministers of religion, and church services held within the centre. There is also a multi-faith room in the garden for everyones use. Residents are provided with a good variety of meals, which include a choice of three main dishes at lunch times. The menus run on a three weekly cycle, and are reviewed at regular intervals. The manager states in the AQAA that a new Kent County Council complaints procedure has been implemented. All complaints are recorded, and are responded to in an appropriate manner. The manager is familiar with the correct protocols for contacting the Social Services safeguarding team, and is co-operative in working with them to ensure that residents are protected from abuse. The environment is well maintained by Grosvenor Facilities, and this ensures that there is a homely environment and a high standard of cleanliness. The manager has weekly joint meetings with Grosvenor Facilities to discuss any issues. There are good systems in place to ensure that health and safety standards and infection control monitoring are reliably checked and carried out. Staffing levels are maintained in accordance with the dependency levels of the residents. There are good recruitment procedures in place, which promotes protection for the residents. All staff have a detailed induction programme. Care staff are expected to complete NVQ training, and the AQAA shows that there are currently 95 per cent of staff who have completed this training, which demonstrates the centres commitment to good staff training, and good outcomes for residents. Mandatory staff training is kept up to date, and there are opportunities for additional training and ongoing staff development. The centre is part of the Kent Vocational Training Programme, which encourages young people into the care profession. Residents are able to take part in recruitment procedures for some staff, and the manager stated that their insight and comments are extremely helpful. The manager provides strong leadersip within the home, and promotes good team work and communication. Staff meetings are held every month, and all staff have regular one to one supervision. The manager has a senior staff team, who work with her to provide support to all other staff. Annual Service Review Page 5 of 7 What are we going to do as a result of this annual service review? The Commission is confident that this service is continuing to operate to an excellent standard, and we will continue to monitor it through our ongoing processes. A key inspection will be carried out by 15th January 2012.
