We give a quality rating following a full review of the service. We call this review a ‘key’ inspection We do an annual service review when there has been no key inspection of the service in the last 12 months. The home is registered for a maximum of 17 service users to include:- up to 17 service users in the category of DE(E) Dementia over 65 years of age up to 2 service users in the category of DE Dementia aged between 55 and 65 years of age up to 2 service users in the category of MD(E) Mental Disorder over 65 years of age The Registered Person must ensure that all staff working in the home have dementia and mental disorder awareness and training, which equips them to meet the assessed needs of the service users accommodated, as defined in the individual plan of care. The service should at all times employ suitably qualified and experienced members of staff, in sufficient numbers, to meet the assessed needs of the service users with dementia and mental disorder. The service should employ a suitably qualified and experienced manager who is registered with the CSCI. Work on the external grounds of the home, to make the area safe, attractive and accessible to service users must be completed by 1.11.05. Annual Service Review Page 2 of 6 If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service Hillcrest Care Home is situated on the outskirts of Tyldesley town centre and is close to local amenities and is on a main bus route. St Georges church is adjacent to the home. The home is a converted vicarage with extensive grounds and provides private accommodation on the ground and first floor. The home has had an extension to the property providing three additional bedrooms with en-suite facilities, an additional lounge and a lift has been installed. Three bedrooms are shared rooms, the rest are single bedrooms. All bedrooms have a hand washbasin and five rooms in total have en-suite facilities. Toilets are close to residents bedrooms and the lounge and dining areas. The home offers accommodation and care for up to seventeen residents over the age of sixty-five years, who have dementia, including up to two residents under sixty-five but over the age of fifty-five, who have dementia and two designated residents who are over sixty five years of age who have a mental disorder. Fees at the last key inspection were 415 pounds per week. Additional charges are made for hairdressing, outings and entertainers. 1 5 0 1 2 0 0 9 Annual Service Review Page 3 of 6 Service update since the last key inspection or annual service review: What did we do for this annual service review? The annual service review for Hillcrest Care Home included the following: The annual quality assurance assessment document (AQAA) was sent to us by the service. The AQAA is a self assessment and a dataset that is filled in once a year by all providers and is one of the main ways we get information from providers about how they are meeting outcomes for people using their service. The AQAA also provides us with statistical information, service trends and patterns in social care. Surveys of people living in the home and staff of the home. A review of what the service has told us since the last key inspection. A review of the findings of the last key unannounced inspection. What has this told us about the service? The AQAA was returned promptly and completed to a good standard. It showed that the home regularly consults with people living there and family members and could give examples of how those discussions have led to changes in practice. The AQAA particularly stressed the use of good staffing at appropriate times and that the low staff turnover has been beneficial in allowing more investment in the existing staff group. The AQAA stated that the home undertakes a comprehensive pre-service assessment as part of a full admission procedure which is well documented and is related to individual need. It stated that full care plans are produced within the first six weeks of admission and it emphasised the importance of choice and self determination in underpinning the approach to the work of the home. The AQAA documented that activities are based on a person centred approach which, again, emphasises the individualised work in the home. This is also extended to the formats for meals which are based on individual choice. The AQAA stated that the principles of homeliness and non-institutionalisation are the basis for the physical environment for people living there. It reported that the home is well equipped and has been improved in a number of areas to maintain standards. It stated that there are full health and safety checks undertaken plus full certification of utilities to ensure a safe environment. The AQAA said that staffing levels meet and exceed what is required for the safe care of people living there. The document stated that full policies and training are being provided to staff in safeguarding and that of the eighteen care workers, thirteen have a vocational qualification which is above the required standard. It reported that staff are given full supervision and appraisal so that they are appropriately supported in their work. The AQAA noted that the registered manager exceeds the required qualifications for Annual Service Review Page 4 of 6 her post. It also stated that there are full management procedures to support her in her work, including a business plan and business objectives. The document confirmed that a full quality assurance procedure is in place as a means of seeking continual improvement. Completed surveys were received from a number of staff and people accommodated in the home. They all were complimentary about the care provided. One stated that the staff look after her as best they can even though she can get agitated at times and another in response to the question what does the home do well? replied everything as far as we are concerned. Staff comments included this is a lovely home and the home is lovely and clean and they are all well and happy. A review of the information received since the last key inspection has shown that there has been very little that has been notified to us. Only one death of a service user and an injury to another has been reported. There have been no complaints, concerns or safeguarding matters that have been sent to us. There has been no need to visit this home since the last key inspection. A review of the findings of the last key inspection held in January 2009 confirmed that this home was rated a 2 star, good service. Of the seven outcome groups, six were assessed as good and one as adequate. The report resulted in four requirements. What are we going to do as a result of this annual service review? The annual service review has shown that the home continues to provide a good service to people living there. The Care Quality Commission will continue to monitor information about this service and will carry out an inspection when required. We can inspect the home at any time if we have any concerns about the quality of the service or the safety of the people living there.
