If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service 36 Forest Road is a small home providing a service for two young adults with a learning disability and additional complex needs and behaviours. It is a semi-detached house in a road with similar houses and is situated near to the town of Hinckley and to a range of facilities. The house has two bedrooms, bathroom, an open plan lounge, dining area and kitchen. There is a pleasant garden to the rear of the house. High staffing ratios are provided, to meet the needs of the young people and to promote their independent living skills and community integration.

What did we do for this annual service review? We looked at the previous Key Inspection Report and information that we have received or asked for from the service since the last Key Inspection. This included: The Annual Quality Assurance Assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gives us some numerical information about the service. Surveys sent out by the Commission to relatives of the people who live in the home, the staff who support them and outside professionals who work with them. We received three surveys from staff, one survey from a relative and one from an outside professional. Information about significant events in relation to people living in the home, which the registered manager (following their responsibilities under the Care Homes Regulations) inform the Commission of. These are called Notifications. What has this told us about the service? The relative who responded to our survey was very positive about the way that the service was meeting their relatives needs. They said that the service always respects their relatives dignity and supports them to live the life they choose. They also said the the service had responded appropriately to any concerns that they had raised. They made the following additional comments; There is a very good management team who work to a high standard. All staff are well trained and highly motivated. There is always someone available to speak to whenever necessary. Staff are all very thoughtful and very caring and very responsive to any changes in needs. The professional who responded to our survey was also very positive about the service provided saying that the service; Works in a person centered way. Has high level health action plans. Is pro-active in social inclusion and has developed many local links with the voluntary, statutory and private sector. The professional also praised the written information and documentation in the home. The AQAA was completed to a very high standard and gave comprehensive information about the service. It conveyed the commitment of the company to ensure that staff are trained to a high standard so that they can effectively meet the needs of the people living at Forest Road. All three staff who responded to our survey said that they were given training relevant to their role, which helped them to meet the individual needs of people and kept them up to date with new ways of working. Staff also said that the manager met with them regularly to discuss how they are working and gave them good support, Skills for Living is a good company to work for. Annual Service Review Page 4 of 6 The AQAA also outlined the systems in place to make sure that people who live in the home are satisfied with the service and are able to make choices and decisions about their lives. A key part of this process is ensuring that staff have the necessary skills to communicate with people. Each person living at Forest Road has a communication passport which outlines the way that they prefer to communicate. If people living at Forest Road need to make a decision about their lives but cannot due to their needs, relatives, keyworkers and professionals meet to do this at a review or Best Interest Meeting. The professional and staff members who responded to our service both commented that the service provides really good health care support for the people living there. The AQQA informed us that each person has a comprehensive Health Action Plan that includes encouraging physical activity to promote a healthy heart and mental well being. What are we going to do as a result of this annual service review? The next inspection of this service will be based on the Fees and Frequency Regulations 2007 and the risk level of the service.
