Conditions of registration: 1. To include 3 people with an assoicated Physical Disability (PD). Date of last inspection 17/11/04 Brief Description of the Service: 27/29 Kingsley Road is a Residential Care Home providing long-term care for 10 Adults with a learning disability. The home is run by the Royal Mencap Society and is situated on a busy main road within easy reach of local shops and amenities and has good access to public transport. Northampton town centre is only a short distance away. Residents rooms are located on all three floors of the home with a stair lift which can be used to assist with access to the first floor. There are eight single bedrooms and one double. Three of the bedrooms have en-suites and there are two shared bathrooms in the home. This is an overview of what the inspector found during the inspection. This unannounced inspection was carried out over the morning of a weekday. The inspection involved talking to Residents about their life in the home and reviewing a sample of records to see how their care is planned and supported. The Inspector met with Staff and also the new Manager to discuss the care provided and the training provided for Staff. Observations of Staff interactions with Residents and daily routines were made during the inspection. Pre-inspection information was provided by the home and some of this has been included within the report. Recommendations made at the last inspection were also reviewed. Comment cards sent out to the home had been distributed to Residents and relatives/visitors. A total of ten comment cards were received from Residents and eight from relatives/visitors. What the service does well: What has improved since the last inspection?
Staff advised that the temporary Acting Manager has worked hard in addressing previous shortfalls. While there are a number of areas where improvements are still required discussion with Staff confirmed that some progress has been made in areas such as care planning and Staff training. A new permanent Manager has been appointed and interviews were due to take place for a Deputy Manager. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7 and 9 Care planning processes do not fully support individual choices and aspirations. EVIDENCE: Some improvements have been made in the records, which are in place to plan Residents care, and it was confirmed through discussion with a Resident that they are aware of and involved with the care planning process. Observations, discussion with Residents, Staff and review of the records identified that further development of the care planning and risk assessment processes is required. Currently plans do not include all aspects of health and care needs or identification and planning to achieve personal goals. A Resident told the Inspector that they wished to move out of the home and have more independence and had wanted this for some time however there was no evidence of any goals to increase levels of independence in the home prior to review of needs. The new Acting Manager confirmed that a review would be arranged and an advocate sought to enable the Resident to have some independent advice and support with decision making about the future.

Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11, 12, 13, 14 and 15 Residents are supported in maintaining family links and leading a lifestyle, which includes various leisure and community activities however would benefit from more opportunities for personal development. EVIDENCE: On arrival at the home one Resident had left for day placements and others were about to leave. Residents spoken to said they were happy with how they spend their days. Some Residents are at home during the day and their routines for getting up in the morning were seen to be flexible with two Residents choosing to have a lie in until the others had left for the day. Staff talked about activities such as visits to the pub, swimming, shopping and one Resident who is doing a computer course. Residents have stated in the returned comment cards that the home provides appropriate activities and this was supported by conversation with two Residents. It was however difficult to ascertain the level of choice and opportunity that had been provided to Residents and as discussed in the previous section more work is required in developing individual goals and aspirations.

The new Acting Manager is aware of the need to review the provision of activities on an individual basis to ensure that they are linked to the personal goals and interests of the individual. Residents were very keen to talk about their forthcoming holiday with some choosing to go to Blackpool and others to Hemsby. Residents who were at home were discussing arrangements for shopping and packing for the holiday. A large party had been held the previous weekend for a Resident celebrating a ‘milestone’ birthday. Staff confirmed that the guest list had been drawn up with the Resident and included family, friends and others who have been significant in his life. Relatives have confirmed that they are welcomed into the home and that they are consulted and kept informed of important matters. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18 and 19 The overall standard of care and personal support is good however more information is required relating to health to confirm that health care needs are fully met. EVIDENCE: Comments from Residents confirmed that they feel well cared for and safe in the home. This was supported by comments from relatives who were all happy with the overall care provided with two particularly commenting on the happiness of the Resident. Residents are supported in establishing personal care routines, which are appropriate to them and are encouraged to shop for their own clothes with staff support. The majority of Residents stated that their privacy is respected with one stating it is sometimes. Staff were observed to respect Residents privacy throughout the inspection. Discussion with Staff confirmed that they are mindful of the need to monitor and act on any changes to Residents health. However review of one Residents records identified that records relating to health care needs were not up to

JUDGEMENT – we looked at outcomes for standard(s) 22 The home has a procedure for dealing with complaints which everyone is aware of. EVIDENCE: The complaints procedure was not reviewed however comment cards received from relatives/visitors confirmed that they were aware of the homes complaints procedure and Residents all knew who to talk to if they were unhappy with the care that they received. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24, 25, 26 and 28 The home was warm and comfortable but would benefit from additional shared seating space. EVIDENCE: A full tour of the premises was not carried out during this inspection. The Manager confirmed that all Residents are accommodated in single bedrooms with the exception of a married couple who share a double room. Observations were made during conversations with Residents and Staff of the communal areas of the home. These areas were seen to be clean and tidy. There is only one lounge area in the home at the present time, which was comfortably furnished and contained a television. It was not possible to assess the impact of the limited lounge space on Residents due to the timing of the visit, however the Acting Manager has

There is a shared dining room with space for all Residents to eat together however at least one Resident prefers to have meals at a dining table in their room. The large garden at the rear of the home contained a marquee at the time of the inspection, which had been used for a birthday party at the weekend. One bedroom was seen with permission from the Resident. The room was of a good size and very personalised with items of importance to the Resident. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 35 Additional support and planning is required to ensure that Staff are appropriately qualified and trained to meet Residents needs. EVIDENCE: Records relating to Staff training were not up to date making it difficult to confirm Staff training needs. Discussion with Staff confirmed that some recent training had taken place, which did not seem to have been added to the training records. The Acting Manager has advised the Inspector that since the inspection she has re-organised all the records. Information submitted prior to the inspection identifies that only three out of the twelve Staff employed have completed a National Vocational Qualification in care. The National Minimum Standards recommend that at least 50 of Staff complete this qualification at level 2 or an equivalent course by 2005. Discussion with Staff indicated that some had requested training and were keen to work towards a National Vocational Qualification however to date a course has not been booked for them. Service users benefit from competent and accountable management of the service. The Commission considers Standards 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 42 Failure to ensure that Staff are appropriately trained in movement and handling puts Staff and Residents at risk. EVIDENCE: At the time of the inspection a new Manager had just been appointed and was going through her induction period. An Acting Manager has been managing the home through a difficult period while a new Manager was recruited and is currently inducting the new Manager. A relative said that the Acting Manager has done a good job and turned problems around in hours. Relevant safety checks are carried out which include checks on fire safety equipment, electrical equipment and visits by the Environmental Health Officer. Recent training for a member of Staff included fire awareness and first aid however she hadn’t received up to date movement and handling training. Given that some Residents need assistance with movement and handling the
