What the care home does well

People living in this home are well cared for by a well managed, educated and competent care team. Those residents who were spoken with said they liked living in the home and that the staff were courteous, friendly and responded to their needs in a kind manner. There is an extensive education programme provided for staff to enable them to provide the care and support to people living in this home. The assessment and review of care is thorough with an activity and recreation programme, which provides stimulation to the residents People living in the home live in comfortable accommodation.

What has improved since the last inspection?

One bedroom has been redecorated. Locks have been installed to bedroom doors to enable privacy and to allow staff to enter in case of emergency.

What the care home could do better:

The owners must remember to send a copy of the reports of their monthly monitoring visits to the CSCI.

Condition of Registration The second place in the category DE, registered in November 2003, is for one named person only (named in the letter of proposal to register dated 6th November 2003). A review of the home`s ability to meet this person`s needs must take place within one month of his admission, and review documentation including any available medical and social work assessments of his needs supplied to the NCSC. Condition of Registration A condition of registration is that the maximum number of service users in the home with nursing needs does not exceed 22 and the maximum number of service users with personal care only does not exceed 21. 9th August 2005 2. Date of last inspection Brief Description of the Service: Adderley Care Home provides nursing and personal care for up to 43 people. There are 17 places for older people over 65 years of age, 24 for people over the age of 65 years who have dementia and 2 places for people under the age of 65 years who have a dementia. On the day of the inspection there were 18 people receiving personal care and 23 nursing. There is a dedicated self contained unit in the home specifically for people who have a dementia. The home is situated in the small market town of Long Sutton and is part of a complex, which includes a separately registered home providing personal care with some self contained flats in the same grounds. The home is close to shops, banks, post office and other facilities. Accommodation is purpose built with single and double bedrooms on the ground and first floors. The first floor is served by stairs and a shaft lift. There are accessible gardens and car parking is available at the front of the home. The philosophy of the home includes encouraging people to exercise their rights to the full, to treat each person as an individual and to respect privacy at all times with the aim to create a warm, caring and friendly atmosphere. This inspection was unannounced and started at 8.30 am. It took place over 5 and a half. The inspector spoke to 10 residents, 8 staff and the deputy manager. The main method was called “case tracking”. This involved selecting 2 residents and tracking the care they received. This was done through the checking of records, discussion with them, the care staff and observation of how care was delivered. A pre-inspection questionnaire was completed by the home. Four comment cards completed by residents and their relatives were also received. There were no concerns. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3 and 6 Adderley Care Home meets the needs of people entering the home. People receive an assessment, which results in their needs being met. Residents and their relatives receive information to allow them to choose whether or not to stay in the home. EVIDENCE: All residents are assessed before entering the home and written confirmation is sent to them that the home is able to meet their needs. There is a detailed statement of purpose and service user’s guide and a copy of the service user’s guide is given to each person when being admitted to the home. Copies of both documents and the most recent inspection report are in folders at the entrance to the home. Staff receive a comprehensive induction programme, which enables them to understand the needs of the residents in the home. One recently appointed new member of staff commented, “I received a well planned induction and was supported and guided during my induction”. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 9 and 11 There is a new detailed care planning system in this home. This ensures that the health and welfare needs of people living in the home are fully met. Medication is safely administered by nurses. Residents and their relatives are cared for and supported at the time of death. EVIDENCE: A new system for recording care has been introduced. This includes assessment details, psychological and physical assessment, care plan, risk and moving and handling assessments, nutritional assessment and daily record. Training has been provided to enable staff to know how to complete the new records. There was evidence of review but again this needs further development to ensure that wherever possible residents are involved in identifying their needs. Each resident has their own Key Worker and staff work in teams. Registered nurses administer medication. Records were well maintained. Residents commented that staff respected their privacy and dignity and the inspector observed staff talking to residents in a calm and polite manner and handling many of the frail residents in a gentle unhurried manner. The home also has a detailed policy concerning death and bereavement, which gives guidance to staff. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 and 15 There is a social activities programme, which provides stimulation and interest for people living inn the home. Visitors are made to feel welcome and supported. EVIDENCE: The home employs a member of staff who is responsible for providing activities. He is to receive training in the future. Activities are varied and involve all residents. For residents who cannot take part in group activities the person spends individual time with them. Visitors are welcome to visit whenever they wish to do so and local churches have links with the home. The inspection took place during breakfast and lunch. Comments from residents were “I don’t like the food”, “I like the food here” and “the food is very enjoyable”. Staff were seen to assist residents who were unable to feed themselves. This was done by staff sitting with them and helping them in an unhurried manner. The home was awarded by South Holland District Council on the 30/8/2005 a Food Safety Award in recognition of the catering standards in the home. This has been displayed on the wall in the entrance to the home. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 Any complaints received are handled properly and residents know that any complaints they have will be listened to and taken seriously. EVIDENCE: No complaints have been received by the CSCI or the home since the last inspection. Residents felt they could approach staff if they had any concerns. Lincolnshire Social Services and Lincolnshire Police investigated an allegation of abuse in August 2005. This was thoroughly investigated but there was insufficient evidence to support the case. There was full co-operation by the home throughout the investigation. There was no concern about the care of residents in the home. All staff receive abuse prevention training. Staff who spoke to the inspector were aware of what abuse was and what they should do if abuse was suspected. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 24 and 26 Residents live in clean, well decorated and safe accommodation. Any maintenance is promptly addressed. EVIDENCE: Residents who spoke to the inspector said how satisfied they were with the decoration and cleanliness of the home. They also said they liked their bedrooms. There were also colourful, accessible, safe garden areas. Since the last inspection improvements to the decoration and furniture continue. Comments from residents were, “I like my bedroom” and “it is very nice here”. On the ground floor there are 6 toilets with one assisted bathroom and one shower room. On the first floor there are 8 toilets, 2 showers, one special Parker bath and one assisted bath. There are also 2 bedrooms, which have en-suite facilities. Adderley Care Home DS0000002653.V264778.R01.S.doc Version 5.0 Page 14 All bedrooms, toilets and bathrooms have locks for privacy but also to enable staff to enter in case of an emergency. The home employs separate laundry and domestic staff and residents commented that the home was clean and tidy. Domestic staff and laundry staff were seen to be going about their work with pride. The home was odour free throughout. Gloves and aprons were also available. The home also has an infection control policy. The laundry has 2 commercial washing machines and 2 commercial tumble dryers. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28 and 29 The home was adequately staffed with employees who are experienced and competent to care for older service users. A wide range of in-house training and National Vocational Qualification training is provided. Staff are correctly recruited. EVIDENCE: Each member of staff is recruited correctly with checks by the Criminal Records Bureau. They then receive a comprehensive training programme following their induction and individual training issues are identified and a training plan is produced. Each employee is given a mentor to assist them through their induction and ongoing training programme. Staff also receive formal supervision. This training is in addition to training in care (NVQ). Staff have also received specific training concerning the needs of people who have dementia. Comments from staff were “we receive a lot of training”, “I feel safe working here” and “I received a comprehensive induction which helped me understand the needs of residents in this home”. The home is also a centre for nurses from other European countries undertaking an adaptation programme to enable them to be registered by the Nursing and Midwifery Council. The home also provides training for nursing students. The manager monitors the dependency of people living in the home. None of the staff or residents expressed any concerns about staffing levels. Comments from staff were “I have time to look after the residents”. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 35, 36, 37 and 38 The home is well lead by a competent, experienced and committed manager. This has lead to a confident and trained staff team. People living in this home are confident in the staff and management in the home. EVIDENCE: The manager has extensive care and management experience. In addition to her nursing qualifications she has qualifications in management, care of older persons, care of the dying and those people who have a dementing illness. All care staff and nurses receive formal supervision 6 times a year and an annual appraisal. The home had detailed policies and procedures, which included clinical procedures. The policies were reviewed in August 2005. In addition the home also had comprehensive health and safety policies, which also included risk assessments.
