The age range of service users admitted to this home shall be between 18 and 35 years The number of service users must not exceed five in number The home may only admit residents who fall within the Autism spectrum 22nd December 2005 Date of last inspection Brief Description of the Service: The home is located on the edge of the village of Irchester, Northamptonshire, with good access to local bus services and major road networks. The home is a large five bedroom detached property with ample parking space to the front and a large well-established garden to the rear. There are five good-sized individual bedrooms complete with ensuite facilities and an additional shower room. A double garage to the front of the property provides an office and educational/activity resources. Weekly fees range from £3,889 to £5,673 dependent upon the individual level of support required. This is an overview of what the inspector found during the inspection. The focus of inspections undertaken by the Commission for Social Care Inspection (CSCI) is upon outcomes for Service Users and their views of the service provided. This inspection was a ‘Key Inspection’ that focused on the key standards under the National Minimum Standards and the Care Standards Act 2000 for homes providing care for adults aged 18-65 years. Prior to the inspection taking place the Commission for Social Care Inspection sent out to the home a pre-inspection questionnaire and comment cards for completion by service users, visitors/relatives and health and social care professionals who have contact with the home. Some service users and visitors feedback cards were returned back to the Commission for Social Care Inspection prior to the inspection taking place and gave an indication of the general satisfaction of the service provided for people living at the home and for those who are involved with the health and social care needs. The pre inspection questionnaire provided information on the administration and management systems within the home. The primary method of inspection used was ‘case tracking’ that involved viewing the service users care plans (a care plan sets out how the home aims to meet the personal, healthcare, social and spiritual needs of the service user), and tracking the care that they receive through discussion with residents, staff and observation of care practices. Policies, procedures and records in relation to staffing, complaints, medication, general maintenance and upkeep of the home were viewed. Discussion took place with staff, and management and general observations of care practices were made. The inspector spent two and a half hours planning the areas to focus on at this inspection, based upon information gained from reviewing the homes service history, the last two inspection reports and information from the pre inspection data collection systems. The inspection took place over a period of approximately four and a half hours. What the service does well:

The home specialises in meeting the needs of the Autistic service users, there was excellent systems in place to demonstrate that the home was run with the best interests of the service users in mind, the care plans and risk assessments were very individualised and person centred. Educational, social and therapeutic activities were promoted to enable service users to live as independent a lifestyle as possible. There was a high staff to service user ration in operation and the staff were observed to be very skilled in responding to non verbal communication and in recognising the emotional needs of the service users. There were effective strategies in place for staff to identify and respond appropriately to anti social behaviour. There was a commitment to ongoing staff training and development, and robust recruitment and selection procedures were followed. What has improved since the last inspection? Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 2 Quality in this outcome area is excellent. Prospective service users, and their representatives, can be assured that the homes admission processes will enable them to make a decision as to whether the home could meet their needs and aspirations. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The admission process involved service users going through a carefully planned transition programme, which was individually tailored to each service user. There were records of pre admission assessments of needs having taken place and ongoing assessments of service users needs following admission into the home; there was evidence of involvement and support from the multi disciplinary team such as health, social and educational professionals. The placement plans had been signed by the service users or their representatives, and placing social worker. Through discussion with the homes service manager, care staff and documentation viewed it was established that the home works in full accordance to its Statement of Purpose. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 6,7& 9 Quality in this outcome area is good. Service Users are provided with opportunities to make everyday choices and are supported in living as independent a lifestyle as possible. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The care plans were individualised and contained detailed information for staff to follow, to support service users in meeting their personal aims and objectives. Through discussion with the service manager, staff and observation of practice, it was demonstrated that meaningful activities were encouraged. In all areas within the house there were cue cards in written and pictorial formats to remind and reinforce the service users of their individual personal goals, and the sequence on how to achieve them, following the TEACH
Alderwood LLA Limited (Irchester) DS0000065111.V318564.R01.S.doc Version 5.2 Page 12 approach (Treatment and Education of Autistic and related Communication Handicapped Children) There were comprehensive risk assessments in place that demonstrated that identifiable risks surrounding the safety of individual service users, staff and visitors, had been addressed and how the risks were to be managed. In addition there was staff signatory sheets to indicate that staff had read and understood how the service users care was to be delivered, and had been made aware of the individual risks for service users, and the risk management plans. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12,13,15,16 & 17 Quality in this outcome area is excellent. There are opportunities for service users to lead fulfilling lifestyles both in and outside of the home This judgement has been made using available evidence including a visit to this service. EVIDENCE: All of the service users had access to a wide range of community resources were they could take part in peer and cultural activities, such as local Country Parks, library, cafes, shops, leisure centre, pubs and cinema. The philosophy and culture within the home was of supporting lifelong learning and developing individual coping skills, for example shopping, laundry food preparation and self care activities. The parent company also operates two children’s homes and is registered as a school. Service users continue to benefit from the organisation of a school environment and the specific input of teachers from the school when developing individual schedules.

There was records available of weekly contact with parents and relatives and service users, had opportunities for private time as appropriate to their individual needs. Each service user had an individual activity plan, that included a range of pursuits specific to their individual abilities and tolerances, such as walks around country parks, visits to the library, local shop, educational and leisure outings Service users were observed carrying out their chosen activities interests such as listening to music and cookery, on the evening of the inspection one service users was supported by staff in making a Lemon Meringue Pie, other service users were observed during the inspection undertaking educational activities. The company has three static caravans one caravan is based on a site at a local leisure attraction and this provides the opportunity for service users to enjoy weekend breaks and meet with relatives away from the home. To enable service users who would not be able to tolerate public swimming sessions the home had made arrangements with a local leisure centre, where the swimming pool was hired for the sole use of the service users. The service users right to experience appropriate, personal relationships is recognised and treated with sensitivity and respect. Records were available of service users food likes and dislikes, nutritional needs, and the staff support required at meal times, the service users weights were monitored and action taken should there be a significant weight gain or loss. Staff were observed providing support to service users, in a respectful manner that recognised their individual skills to be as independent as possible. There was a homely atmosphere within the home; the service users were relaxed and friendly with each other and the staff. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 18,19 & 20 Quality in this outcome area is excellent. The home encourages and supports service users to lead a healthy lifestyle. This judgement has been made using available evidence including a visit to this service. The service users care plans were very individualised, identifying what health and personal care support service users required. Within the service users personal bedrooms and within bathrooms there were cue cards to facilitate service users in following the sequence of their individual personal care routines, enabling some independence to be achieved. The staff were very skilled in responding appropriately to non-verbal communication and recognising the physical and emotional needs of the service users. There was effective monitoring systems in place and staff said that analysing behavioural events to promote well being of he is able The manager as a trainer is able to impart skills and knowledge to new staff and encourage them to develop their own critical analysis skills when working with and responding to service users and other staff. There was information available within the care plans on the input from other healthcare professionals such as a neuropsychiatrist, psychologist, general practitioners, optical, dental and chiropody services.

JUDGEMENT – we looked at outcomes for the following standard(s): Standards 22 & 23 Quality in this outcome area is good. Service users and their representatives can be assured that any concerns or complaints they may have will be listened to and acted upon and that service users are protected from abuse. This judgement has been made using available evidence including a visit to this service. EVIDENCE: There were effective systems in place to communicate with the service users families such as weekly telephone calls with parents, and regular care reviews taking place involving service users where possible and their families. The management and staff act as advocates for the service users and when speaking with staff they were fully aware of how to raising any concerns they may have in relation to the care and welfare of the service users. On viewing the homes policies and procedures, there were procedures in place to respond to concerns/complaints and protection of vulnerable adults issues, no complaints or protection issues had been raised with the Commission for Social Care Inspection during the period from the last inspection visit taking place and this inspection visit. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 24 & 30 Quality in this outcome area is good. Service users live in a home that is homely and adapted to meet their individual needs. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The layout of the home was spacious and had been adapted to meet the needs of people living with Autism, there were educational and recreational facilities available for service users to access and space for privacy. The bedrooms viewed were clean and tidy and had been decorated to meet the needs of the individual service users levels of tolerance to objects and colour schemes. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 32,34 & 35 Quality in this outcome area is excellent .The service users are protected and supported by a staff team that are appropriately trained, competent and qualified to meet their needs. This judgement has been made using available evidence including a visit to this service. EVIDENCE: There was high staffing ratios and good staff support systems in place, with formal 1-1 staff supervision records available, staff said that informal supervision and debriefing sessions also took place. There was a supportive culture within the home, based upon valuing the individuality and uniqueness of service users and staff. Through discussion with staff and observation of the care practice it was demonstrated that the staff had a sound understanding of the needs of people living with Autism, and could transfer them to the individual needs of the service users living at the home.
Alderwood LLA Limited (Irchester) DS0000065111.V318564.R01.S.doc Version 5.2 Page 20 All staff that work at the home undergo a full induction-training programme that includes three days classroom instruction to equip the staff with the knowledge and skills required to support people living with Autism, following the induction training the new member of staffs care practice is monitored by the manager, senior staff and colleagues. There were records available of all training that had taken place, within the staff files and there was an annual training planned. Training was extended to include agency staff working at the home to ensure that they had the knowledge and skills to care for the service users living at the home. Staff said that all agency staff were made fully aware of the complex individual needs of the service users prior to any face to face contact with the service users. Staff files viewed contained all of the necessary documentation to evidence that the home operates a robust staff recruitment and selection process. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The management style was open and supportive this was reflected in the feedback received from the service user and visitors questionnaires and from observations made during the inspection visit of staff and service user involvement. The service users were observed to interact well with staff the staff expressed their satisfaction with working at the home and the level of support available from the management and peers.

Risk assessments were in place for general home safety and individual risk assessments specific to service users and staff. The management recording systems provided a clear audit trail in line with the policies and procedures in place. All care records were kept secure and confidential records were stored in a locked file. Staff meetings take place regularly and the service users care and support needs were regularly reviewed. Service users finances were handled appropriately, and money held on behalf of the service users was stored securely. There was an effective quality assurance system in place, and the input from the parents, placement officers and other professionals that visit the home provides opportunities for external scrutiny of care practice.
