The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Service users said they were happy with the level of care that the staff give, with their rooms and the food. "The best ever" and "Can`t fault the food" were some expressions about food from service users. "Always choices" another. They also said they enjoyed the trips out to Newstead Abbey and looking forward to going to Chatsworth. They said the staff treat them with dignity and their visitors are always made to feel welcome. All rooms have en-suite facilities and are furnished to include personal items and these give a homely feeling to the personal spaces. The communal areas are well maintained and carry through the homely atmosphere.

What has improved since the last inspection?

Previous requirements from the last inspection have been met and the activities organiser is developing an activities programme.

What the care home could do better:

Identifying interests and hobbies for each service user on the activity records would benefit the organiser to develop programmes to interest more service users in their activities as some service users have expressed they are "bored with bingo and games. " Staffing levels on each shift to be sufficient to meet the needs of the service users.

Service Users shall be within category OP (51) The Manager completes the Registered Managers Award. Date of last inspection 26th April 2005 Brief Description of the Service: Located in a residential area and within a mile of Hucknall town centre, Annesley Lodge provides personal care for up to 51 older persons. All rooms offer en-suite accommodation, 45 single rooms and three double rooms. There are a number of comfortable, homely communal areas and facilities include the provision of all personal care, meals, laundry and hairdressing. The home is purpose built and owned by Anchor Trust. Annesley Lodge Care Home DS0000050038.V252202.R01.S.doc Version 5.0 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. The unannounced inspection started at 10.20am and was over 5 hours, during which time 12 service users were spoken with, some in private and some in a group in the lounge, 5 members of staff were also interviewed and the records of 6 service users were case tracked. Case tracking uses the service users views on their experiences and expectations of living in the home with analysis of the records and interviews with members of staff to ensure that those living at the home have their needs met and their health and welfare maintained appropriately. Assisting with the inspection was Wendy Bell Senior Care Assistant. Overall, the home has developed a homely, personalised environment . At the start of the inspection a number of the service users were in the lounge area reading their daily newspapers. One service user was outside smoking, which was her personal choice. Lunch was served in the large dining room, the food looked well presented and nutritious. There was choice of food and drink and some service users, who preferred to have their meals in their rooms, had their meals served, covered and on trays at the same time as those in the dining room. There was recently a summer fete which was enjoyed by those spoken with and a sum of money was raised for the home. The recent steak night was also spoken about with enthusiasm by the service users. The hairdresser was present during the inspection and the service users said they were happy with the way he does their hair. Staff and service users interactions observed were positive and staff were polite and helpful. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,4,5 Service users are given the contract, terms and conditions and information they and their families need to make an informed choice of home. The home also assesses each prospective service user to ensure needs will be met. EVIDENCE: Staff said there had been six new admissions recently and two new service users case tracked said they had been given an opportunity to visit. They also said they had information which they could read and questions were asked to find out what they needed. Records also showed evidence of signed, negotiated contracts and assessment of needs was seen. The personal history in each case tracked gave an insight into the past lifestyle of the service user Annesley Lodge Care Home DS0000050038.V252202.R01.S.doc Version 5.0 Page 9 Health and Personal Care The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10,11 Service users care plans set out their needs which are met. They are treated with respect and their privacy is respected. One service user administers his own medication. Service users’ wishes following death are identified. EVIDENCE: Service users spoken with all said that their care needs are met and in the ways they like. They said they are treated with dignity and the staff always knock on the doors of their rooms, respecting their privacy. Records seen at inspection showed thorough care plans setting out the needs of the service users. Risks identified with risk assessment are highlighted with “Risk stickers” which prompt care staff to be aware of the risk. The care plans tracked also record the service user’s wishes on death. During the inspection staff interactions with the service users were observed to be respectful and when entering rooms staff were observed to knock and introduce themselves. Annesley Lodge Care Home DS0000050038.V252202.R01.S.doc Version 5.0 Page 10 Medication for the majority of service users is administered by staff and the lunchtime administration was observed. The case tracked service users’ Medicines Administration Records were correctly completed, no gaps and clear identification of medication that was given “when required”. All medicines were stored in the correct manner. The room where medicines were stored had an air conditioning unit on to maintain the proper temperature. One service user prefers to give his own medicine and all the documentation and safekeeping of these was appropriate. The service user also prefers to collect his own medicines. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15 Service users interests and previous lifestyle are taken into consideration when developing care packages and contact with family and friends is encouraged. The food is well presented and appeared appetising and nutritious, served in the dining room, but some residents preferred to eat in their rooms and this was supported. EVIDENCE: Service users spoken with were all very positive about the meals and the quantity and quality of the food, “you can’t fault it” and “it’s the best you can get.” Choices were observed with some service users eating sausage, mash and cabbage, others with ham salads. The food was served by staff wearing the appropriate tabards and those who preferred to eat in their rooms were provided with food which was covered and served from a tray from a portable tray holder. Staff were observed to maintain food hygiene standards. Service users stated that whilst they enjoy bingo and puzzles, they are bored with this. One service user felt the activities made her “feel like a child.” She did say though that she enjoyed the trip to Newstead Abbey and this was confirmed by other service users who also said they enjoyed it. The activity records for service users seen at inspection contain names and room numbers Annesley Lodge Care Home DS0000050038.V252202.R01.S.doc Version 5.0 Page 12 but no other information. The running records of a number of service users state “had usual chat”, “did not want to take part in activity” “enjoyed activity”. There was evidence that one service user who wished to vote was assisted to do this. Service users confirmed that their family and friends are always made welcome and although there wasn’t anywhere really private downstairs, their rooms were appropriate for private visits. Service users spoken with had telephones and on inspection of one of the double rooms a landline telephone was observed. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,18 Complaints are dealt with appropriately and the Home protect the service users’ from abuse. EVIDENCE: Service users spoken with and tracked said they were aware of who to complain to but have no complaints. Complaints written in the complaints book were dealt with following the Home’s policies and outcomes also recorded. Staff have the relevant Protection of Vulnerable Adults training and those interviewed during the inspection were able to confirm the procedure should abuse be suspected. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,22,23,24,25,26 The Home is well-maintained with access to all areas. Some maintenance work within the en-suites was identified. The rooms suit individual needs, are comfortable, clean and hygienic. The specialist equipment required by some is available and there are sufficient toilet and washing facilities for all the service users. EVIDENCE: At the time of inspection, the lounge carpet was being spot cleaned and all other areas of the home seen were clean, hygienic and well maintained. There were however, damaged metal shower drainage covers in the floors of a number of the en-suite facilities seen. The service users spoken with said their rooms were comfortable, with their own “bits and pieces” around them. They said they felt they were suitable for their needs. Hoists were observed in two of the bedrooms of the service users case tracked and conversations with the service users confirm their use. There is a lift available between floors which is also regularly maintained. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30 Staff are trained and competent and service users are in safe hands with suitable staff recruited. The staffing levels on the day of inspection were appropriate but there is evidence that these levels are not always maintained which for a large home could lead to service user needs not being met. EVIDENCE: Service users said that the staff work with them in a very professional way and that they feel comfortable with the care they give. The staff records show the training records. One member of staff interviewed is working on NVQ2. Others confirmed they have received training which included Protection of Vulnerable Adults, Dementia Care, Back Care and First Aid. The staff records seen show Criminal Records Bureau and Protection of Vulnerable Adults checks, together with two references. The District Nurse visiting stated that she felt the care given by the staff was appropriate and that once she has instructed the staff they carry out any instructions well. Staff on duty at the time of inspection stated that there was enough staff today but that is not always the case. The duty rota for September and October show afternoon staff levels reduced to four on fifteen shifts to meet the needs of 50 service users. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32,33,36,37,38 The Home’s management and conduct are focused on the best interests of the service users and the home’s records, policies, procedures and staff supervision reflect this. EVIDENCE: The Manager and deputy were not present during this inspection, but the Senior Care Worker and staff explained that they put the service users’ interests first and the service users spoken with felt that this does happen. The home’s records are well maintained and all the required servicing records seen were up to date, including fire alarm testing, electrical and lift servicing records. The Accident book and complaints books were also inspected and found to be up to date and corresponded with events in the records case tracked.
