Date of last inspection 8th December 2004 Brief Description of the Service: April Cottage care home cares for twelve older persons in two converted detached properties converted into one buiilding for its present purpose. The home is situated in a residential area and within walking distance to the market town centre of Coalvile. Residents have access to a variety of shops and other amenities in the town centre. There is easy access for private and public transport. The premise consists of two floors accessible by use of the stairs and passenger lift. There are a variety of facilities in the home such as an adequate number of toilet and washing facilities based on the number of residents residing in the home. There is also lounge and dining space for residents use. The home comprises twelve single bedrooms, six with ensuite facilities. A garden is situated to the rear of the premises. This inspection took three hours to complete. The outcome of the inspection was very positive in that two residents spoken with indicated that they are very satisfied with the care provisions in the home. The registered manager and one staff member were also spoken with as part of the inspection process. No Requirements or Recommendations were made during this inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 3 and 4. The admission procedure is good. Detailed information is provided for residents and their relatives about the care provided in the home. EVIDENCE: An examination of the Statement of Purpose indicated that the document provides accurate information about services provided in the home. Conversation held with two residents indicated that they are receiving services as outlined in the Statement of Purpose. One resident’s Terms and Conditions of their residency and contract seen indicated that residents are made aware of their rights whilst residing in the home. Two residents’ assessments were inspected and conversation held with the residents indicated that the content of the documents accurately identifies their care needs. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10 and 11. Residents’ care plans are written in detail giving staff members’ good guidance on how to meet residents’ care needs. The medication process works well in ensuring that residents receive their medication as prescribed. EVIDENCE: Two residents care plans were inspected. The documents contained adequate information to give staff members’ guidance on how to meet residents’ care needs. Conversation held with one staff member indicated that she regularly reads residents’ care plans. The staff member gave good verbal responses regarding providing care in accordance with the care plans. Two residents spoken with indicated that they are very satisfied that care staff members are providing good care. One resident commented ‘They look after me well here’. Another resident stated ‘They’re marvellous, the ladies are nice’, referring to care staff members. Two residents’ spoken with indicated that they are able to see a doctor or nurse when needed. One resident commented ‘They’ll contact the doctor for you’. Professional visitors’ records seen indicate that they visit the home to attend to residents’ medical and nursing care needs. An examination of the medication process found that the home is managing the process well. For example two residents’ medication records seen were up to date. The medication policy seen contain adequate information for staff members’ guidance concerning safe use of medication. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 and 15. Giving residents’ choices over their lifestyles and providing good quality meals are managed well for residents residing in the home. EVIDENCE: Two residents were spoken with and they indicated that they are satisfied with the choice and standard of meals in the home. One resident commented ‘The food is alright’. The menu seen indicated that choice and variety of food is offered on a daily basis. Two residents spoken with indicated that they are satisfied with having regular visits from relatives and friends. The visitors’ records seen indicated that regular visits are made to the home. One resident stated ‘Most days I have visitors’. Discussion held with the registered manager and two residents indicated that a programme of activities is offered to residents on a regular basis. For example during the inspection residents were engaged in doing physical exercises according to their abilities. One resident spoken with indicated that she enjoyed the exercise. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18. There is an adult protection procedure to respond to suspicion or allegations of abuse. EVIDENCE: A detailed inspection of the written adult protection procedure indicated that processes are in place to respond to suspicion or allegations of abuse. One staff member spoken with was able to verbally demonstrate her knowledge of the home’s adult protection process. Two residents spoken with indicated that they feel safe residing in the home. The adult protection policy seen contain sufficient information for staff guidance on how to respond to issues concerning adult protection. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 21, 23, 24 and 26. The premise is kept clean and maintained to good standards with a strong emphasis on residents’ comfort. EVIDENCE: An examination of the flooring, walls and fixtures in the home indicated that the home is being maintained to very high standards and that a programme of maintenance is ongoing to maintain the premises to good standards. The number of toilet and washing facilities are sufficient in numbers based on twelve people residing in the home. The number and size of bedrooms are adequate based on architectural documents seen and conversation held with two residents. Observations made indicated that residents are able to take into the home their own furniture. One resident stated ‘I’ve got a bit of my own furniture’. Observations made at the time of the inspection found the entire home to be very clean and hygienic. The infection control policy seen contain sufficient information for staff members’ guidance concerning safe hygiene practice. One staff member spoken with gave good verbal responses concerning effective hygiene control. The home is commended for maintaining a clean and well-maintained home and for creating a homely environment. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27. The care hours and staffing numbers in the home are sufficient for the provision of residents’ care and safety. EVIDENCE: The staffing rota was examined and a physical count indicated that there is adequate numbers of care staff on duty. The staffing rota seen and observations made indicated that there is appropriate skill mix of staff employed to work in the home. Two residents spoken with indicated that they are very satisfied with response times from care staff members, indicating that a staff member is always available. One resident commented ‘They’re good carers night and day’. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 37 and 38. Residents’ care records and fire safety is managed well for residents’ protection. EVIDENCE: A recent fire safety check was carried out and the outcome was very positive. The home was commended for having an excellent fire evacuation procedure. This is based on reading the fire safety report and discussion held with the registered manager. An inspection of fire safety equipment such as fire extinguishers found that they are checked at regular intervals. Written fire safety procedures have sufficient information for staff guidance concerning fire safety. Two residents’ care records seen for example care plans and medication records indicate that residents’ rights and safety appear to be safeguarded through effective management of their records.
