What the care home does well

Arlington House offers a homely and well maintained environment. The manager and staff encourage residents to be as independent as possible and to be part of the day to day running of the home. Residents are treated with a high level of dignity and respect and staff have an excellent understanding of the individual needs, preferences and personalities of each resident. There is a warm rapport between staff and residents. Residents presented as well groomed, relaxed and happy in the home. Care plans are comprehensive and form a good basis for an assessment of need and to provide the day to day support to enable residents to maintain their independence and be provided with appropriate care and support. Health care needs are well met and the staff at the home liaise well with health care and medical agencies. The management team and staff form a cohesive team, with good communication between themselves, with the residents and with other professionals. Record keeping was accurate, up to date and organised.

What has improved since the last inspection?

The owner has ensured that appropriate checks are carried out on all staff working at the home and has completed risk assessments on radiators and associated pipe work.

What the care home could do better:

The home has maintained the quality of care and effective management of the home. An update for staff on adult protection procedures is needed.

The house has been extended and converted to provide comfortable living accommodation for up to 18 older people. The home is set in landscaped and well maintained gardens. The property is well decorated and comfortably furnished with a choice of lounge areas. The bedrooms are provided over two floors; there is no lift access between floors. The home is well staffed and training is available. This unannounced inspection was carried out on 12th December 2005 over a period of 4 hours. The owner and manager were present throughout the visit. Five staff, including an awake night staff were interviewed. Care practices and interactions between staff and residents were observed. Residents presented as very happy, relaxed and well cared for. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Arrangements for assessing the needs of residents are well managed. EVIDENCE: There is an assessment of need process, records were available and had been included each care plan. Discussion with the manager and the care plans demonstrated that the resident, family members and representatives are included in the assessment process. Assessment of residents needs were thorough and well detailed. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 10 and 11 The health care and medical needs were identified, promoted and well met. There is a warm and interactive rapport in the home and residents rights to privacy and choice are treated with respect and dignity. EVIDENCE: Two care plans were viewed on this inspection. Medical histories, health assessments and how and who were involved in meeting these needs were recorded in each plan. The manager and staff on duty at the time of this inspection provided a clear and detailed insight into the medical and health needs of the residents. The regular visits and details of consultations made by doctors, district nurses, the optician, chiropodist and other medical professionals are recorded. Records provided evidence that the home liaises with relevant medical agencies to seek advice and further consultation when necessary. The manager and owner have escorted residents to hospital appointments. Discussion with staff and observations throughout the visit evidenced a very sensitive and respectful approach to ensuring the privacy and dignity of residents is respected. The manager and staff provided a clear commitment to caring for the residents at the home and provided a dedicated insight into caring for residents who become increasingly frail and supporting both residents and their families through the processes of death and bereavement. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13 and 14 This home provides a warm and enabling environment, promoting the wishes, views, independence and choices of residents. EVIDENCE: Relatives and friends are able to visit the home as often as they wish and are encouraged to be involved in the care of the residents. Some residents go out into the local community, enjoy visiting family and church visits. Staff presented a clear commitment to enabling residents to have choice and control over their lives and staff were knowledgeable about the individual likes, dislikes and daily routines of the residents. Some service users were observed to be enjoying the company in the lounge, whilst others preferred the privacy of their own rooms. When interviewing night staff at 10:00 pm, one resident was still up watching TV whilst others had gone to bed at different times. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 17 and 18 The staff have a clear awareness of the need to provide a safe environment for residents and to promote and protect the legal rights of the residents. EVIDENCE: The manager and staff presented a good understanding of the vulnerability of older people living at the home, the need for consistent observation and care and providing a safe environment. However, one staff member was not sure about the adult protection reporting procedures and agencies outside of the home. Risk assessments were detailed and included in the care plan. All residents are on the electoral register. Staff confirmed that the owner of the home transports residents who wish to visit the local polling station, whilst other residents have postal votes arranged. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 29 and 30 The staffing arrangements in the home are well managed and staff working there are able to meet the needs of the residents. EVIDENCE: On the day of this inspection, the home was adequately staffed. The manager and many of the staff have worked at the home for an extensive period of time and provide a good level of experience. Interviews with staff confirmed that training is available and identified that staff have an in depth knowledge of how to meet the residents’ needs. The owner confirmed her knowledge of the Criminal Records Bureau and the need to provide appropriate checks before any member of staff commences employment at the home on the day of this inspection. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 35, 37 and 38 The management of the home was well organised, with clear lines of accountability, roles and responsibilities. Records were, well maintained, accurate and up to date and health and safety checks had been carried out. EVIDENCE: The manager, who is a qualified nurse, was present throughout this inspection and provided a thorough knowledge of the, residents’ needs, management systems and the running of the home. Staff were very positive about the management of the home and felt very supported. The manager stores and manages the spending money of some residents and individual records and receipts are maintained. Fire safety checks had been regularly completed. Record keeping in the home was detailed, accurate and up to date.
