This inspection was carried out on 1st November 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Adequate. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The home provides care in a pleasant environment, where residents can choose how to spend their day. Staff are provided with training and information to ensure that they provide care in an appropriate manner ensuring residents privacy dignity and safety at all times. All residents spoken with were very positive about the care they received including the meals and activities provided.

What has improved since the last inspection?

The manager has now been registered with the Commission for Social Care Inspection following her `fit person interview`. Risk assessments have been carried out amongst residents regarding their wish or ability to administer their own medication. The quality assurance questionnaire has now been distributed to residents and outcomes recorded. The manager has started to raise the issue of adult abuse awareness at staff meetings ensuring staff are reminded of the need to be vigilant and what to do if they suspect something wrong.

What the care home could do better:

The manager must ensure that staff sign to say they have administer medication to residents and the where there are gaps that these can be explained. This is an outstanding immediate requirement and potentially places residents at risk. This is considered to be a serious shortcoming as only trained and qualified nursing staff are signing medication records and this below the standard expected for qualified nurses.

Old age, not falling within any other category (OP) - 62 Physical disability (PD) - 1 Terminally ill (TI) - 1 Date of last inspection 10th June 2005 Brief Description of the Service: Aslockton Hall provides personal and /or nursing care for up to sixtytwo older people. The registration includes 1 physical disability and 1 terminally ill place. The premises are located in a rural setting comprising a period building adapted and extended to offer spacious and attractive accommodation. There are various seating areas and lounges around the home, which offer different views of the gardens. There are extensive grounds and gardens maintained to a very high standard, which are made use of during the warmer months when parties and garden fetes are held. The home has been adapted to meet specialist’s needs and there is a range of equipment available to support service users personal requirements. This unannounced inspection took place as part of the annual inspection programme it was carried out by one inspector over four hours and is the second inspection for this year. The communal areas of the home were inspected and a sample of bedrooms were also seen. The manager was on annual leave and the deputy manager, Margaret Smith, was available throughout the inspection. Staff and care records were examined. Two staff, four residents and two relatives were spoken with. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 5 The needs of the residents are met within this setting. EVIDENCE: Five plans of care were looked at during this inspection and all met the standard. Plans included information regarding the residents needs prior admission and then shortly after admission. It covers medical history, social history as well as the residents’ needs for personal care. Visitors spoken with said that they had been able to visit the home prior to their relative moving in. Residents spoken with said that they had not had trial visits but that was usually due to circumstances i.e. they were in hospital and it was not possible. The home does have a policy regarding offering trial visits for all perspective residents and placements are not confirmed until all parties and happy. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 11 Individual plans address residents needs but residents are placed at risk where medication sheets are not correctly completed. EVIDENCE: Residents where possible are involved in creating their care plan and evidence was seen for this. Where residents are unable plans state the reason why. Plans provide detail as to how residents care it to be met and there was evidence noted that plans are reviewed on a regular basis. Plans include risk assessments including prevention of falls. Residents’ medical needs are clearly identified with separate plans of care for each need identified. Residents spoken with could not always remember whether they had seen their care plan but said that staff are always available and always provide care as they want. Evidence was seen that GP’s were called when needed, and residents spoken with said they saw a doctor whenever they needed to. Visitors spoken with felt that health care needs were looked after very well and staff ensured that they were kept informed of any situation that may have arisen. At the last inspection a requirement was set for the manager to risk assess all resident as to whether they were able to self medicate. Evidence that this requirement has been met was seen on all plans of care viewed. On viewing a selection of medication administration records it was clear that trained nurses are failing to maintain the records appropriately. The manager must ensure that this immediate requirement is met. Although care plans were of a good standard and contained forms that covered all aspects of the National Minimum Standards, on all plans viewed it was noted that no information had been documented regarding the residents’ wishes as to their care and comfort required should their health deteriorate and die. The manager must ensure that residents and relatives are involved in creating a plan that details their wishes regarding this area. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. EVIDENCE: Aslockton Hall Care Home DS0000026412.V263097.R01.S.doc Version 5.0 Page 12 Complaints and Protection The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 Residents and family feel able to make a complaint and are confident it would be dealt with. Strategies are in place to raise staff’ awareness regarding adult abuse. EVIDENCE: Neither the home nor the Commission has received any complaints since the last inspection. All residents spoken with said they would feel confident in taking their concerns to the manager, and felt that all staff would listen and be sympathetic. Staff spoken with had a good understanding of what constituted abuse and said that this issue was raised at their interview and also had been discussed at team meetings. A requirement was set at the last inspection regarding making arrangements to raise staff awareness regarding adult abuse. This is now considered met. Residents spoken with said that they felt safe and staff always spoke to them with respect. Visitors spoken with said that they felt confident to leave their relative in the home in the knowledge that nothing bad would happen to them. The home has a copy of the Nottinghamshire Adult Protection Procedures. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 21, 22 and 24 A homely environment is provided, which is comfortable and safe. Bedrooms suit the needs of individual service users and sufficient shared toilets and bathrooms are available. EVIDENCE: Residents spoken with said that they were able to access bathing facilities and toilets. Toilets were seen close to sitting rooms and dining areas as well as residents who had ensuite facilities. Grab rails were seen around the home and staff spoken with said that they had access to various aids and adaptations. Although they did comment that although they were hoists on each floor sometimes they may be in use at the other end of the home, sometimes creating a delay if a member of staff needed to go and fetch it. The manager should assess whether there are sufficient hoists to meet the needs of the residents. Those bedrooms viewed were pleasantly decorated and clean. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 and 29 Staff are employed in sufficient numbers and employment procedures are robust to maintain the safety of residents. EVIDENCE: Staff rotas were looked and it was noted that sufficient staff were provided. Staff spoken with said that usually there were enough staff on duty to enable them to meet residents need, where there were shortcomings this was caused by staff calling in sick for a shift. The deputy manager provided evidence to show that where staff did phone in sick every effort was made to ensure residents were not placed at risk. A requirement was made at the last inspection to ensure that all staff received up to date Criminal Records Bureau checks prior to taking up post. Evidence was seen that this is taking place, this requirement is now met. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 and 33 Residents live in a well managed home where their views are listened to and acted upon. EVIDENCE: The manager has now undergone her fit person interview with the Commission and has been registered as a manager. A requirement was set at the last inspection to carry out effective quality assurance and quality monitoring. Evidence was seen that this process has now begun and the requirement is met.
