The city centre of Nottingham is about 1 mile away and there is a direct bus route to and from the city centre with a bus stop about 300m metres from the care home. A park and ride service is available, about 500 metres from the care home. The home is split into three units, two of which provide up to fifty-one (51) places for people who may have a Dementia related illness (residential, non-nursing). The other unit provides personal care (residential care, non-nursing) for up to twenty-five (25) older people. There is a choice of lounges and combined dining room areas. The building is wheelchair accessible with adaptations and equipment appropriate to the needs of the service users. The garden area to the front of the premises has an enclosed safe garden to enable residents to access it safely. Parking is limited at the front of the building but there is more space available at the rear of the building itself as well on the road parking. We found some evidence of improvement in the handling of medicines in that the receipt and disposal of medicines is recorded. However, the quantity of a medicine remaining when a new supply is received is not clear from the records and medicines could not be accounted for. During the inspection we conducted audits of some medicines and found six discrepancies. Some medicines were stored in unlocked cupboards or medicine refrigerators; these cupboards and fridges were in locked rooms that only staff allowed to administer medicines could enter. Thermometers used to check the fridge temperatures were not the maximum/minimum type. The temperature of one refrigerator was recorded each morning and afternoon. There were no temperature records for the second refrigerator, and the temperature was above the recommended maximum at the time of the inspection. Cupboards for storing controlled drugs complied with the relevant regulations. However, the key for one of these cupboards was missing and some controlled drugs were being illegally stored. A locksmith arrived at the home during the inspection to put a new lock on this cupboard. Controlled drugs records were being kept in a file, and not in a bound book with numbered pages, as required. Controlled drugs no longer needed had not been disposed of promptly. One person looked after her own medicines, with support from her family. There was no up-to-date assessment of this persons ability to self-medicate in her care plan.
