Dementia - Code DE. Date of last inspection Brief description of the care home Avondale is a detached property that is situated within a residential area. The home is owned by Aswan Care Ltd and offers accommodation for up to 31 Older people including people who suffer from Dementia. There are 27 single ensuite bedrooms and 4 single only. The dementia unit is separated via a locked door and is fully equipped with lounge, conservatory, enclosed garden, and an assisted bath. Some single rooms are not occupied whilst major upgrade is taking place. The Home is Care Homes for Older People Page 2 of 8 Brief description of the care home undergoing refurbishment and some areas are not in use at the present time. There is a separate smoking lounge. A passenger lift offers access to the first floor and a range of specialist lifting equipment is available for people with dependent needs. The current charges are £294.00 to £390.00 per week. There are separate charges for hairdressing and newspapers. The service user guide, and statement of purpose are available in the entrance hall and the current Certificate of Registration is also on display. Care Homes for Older People Page 3 of 8 What we found: This service previously had a key inspection which was carried out on 07/08/08. At that time it received a quality rating of a one star or adequate service. This inspection, which was not announced prior to visiting is what we call a random inspection and was done in response to information we had received which raised concerns that previous requirements may not have been met, which may affect the health, safety and welfare of people using the service. We visited the home and looked at the standards that we felt related to the concerns raised and the progress with meeting the outstanding requirements that we set at the last inspection. We spoke with the manager and staff, observed care practice and examined the records of 6 people using the service to determine whether standards were being met. The main findings of our inspection are set out below under the standard headings we inspected. Health and Personal care. We received information about 3 people who had fallen and sustained fractures as a result. These 3 people are presently subject to investigation through Adult Social and Health Care as part of the safeguarding of adults procedures. We examined the archived records of 3 people that had fallen and sustained injuries and found shortfalls in the frequency of assessments for falls and pressure sore risk. We also found that changes in their condition when they had received hospital admissions had not been properly planned for when they returned to the home which did not ensure that staff were aware of how to care for them. The manager told us that since these 3 people had sustained injuries she had been working hard with the support of the local authority monitoring and evaluation officers to improve on care planning and risk assessments and ensure that staff had suitable training in their work. We examined the records of 3 other people and found that they were now being properly assessed for any risk of them falling and a care plan was in place that informed staff how to manage the risk. Staff training records showed that on 10th March 2009 staff had received manual handling training updates including the use of hoisting equipment to ensure that they were fully up to date with safe practices when moving and handling people. We identified improvements in the regular review of care plans indicating that the manager now uses information from risk assessments to make changes to the plans each month or as conditions change. We saw improved recording of any nutritional or pressure sore risk that informed a suitable plan of care to ensure peoples health was maintained. Care Homes for Older People Page 4 of 8 Staffing. We saw how staff had previously been left in situations such as medical emergencies when they had not received any first aid training, however the manager showed us evidence that staff had now been booked on a first aid course. We identified that staff had received training in how to safeguard people from abuse. The numbers of staff during the afternoon shifts were found to be insufficient to manage the meal time and the care and support that people needed, the manager made immediate arrangements to increase the staffing numbers accordingly.
