What the care home does well

The service has comprehensive and robust policies and procedures in place that lead to increased protection for residents. Assessment records and care plans are also comprehensive and provide a holistic presentation of resident`s needs and preferences. The management approach at the home was open and inclusive, staff felt supported by the management team. Activities of daily living are made flexible in order to meet residents individual needs and expectations.

What has improved since the last inspection?

Since the last inspection the registered managers hours are now supernumerary and therefore sufficient time is provided for required management processes and activities.

What the care home could do better:

Channels of communication could be improved and so reduce or resolve any incidence of dissatisfaction experienced by residents and or their relatives.

The home is located close to the centre of the town within a fifteen-minute walk. and is situated within a ten-minute walk of the train station. East Midlands Airport is a thirty-minute drive from the home.The home is purpose built and accommodation is on two floors accessed by a passenger lift. all bedrooms are single with en-suite facilities, all areas of the home are accessible to service users in wheelchairs. Situated on the banks of the famous Grand Union Canal the home has pleasant views of Narrow boats and Canal events are a regular feature. The home has a patio area with a pond and greenhouse. This inspection was unannounced and took place over three hours and was facilitated by the home’s registered manager. The inspector selected four service users and tracked the care they receive through review of their records, discussion with them (where possible), the care staff and observation of care practices. The inspector spoke with staff members regarding training and support and with one visitor to the home. This was a positive inspection and was facilitated by the manager and staff in a helpful, friendly and professional manner. Since the last inspection in June 2005 the CSCI has received one complaint regarding care practice and the provision of equipment. The complaint was referred to the provider to investigate. The investigation was undertaken in a professional manner and appropriate action taken. The CSCI sends out service provider pre inspection questionnaires and resident questionnaires throughout the inspection year. These are questionnaires to ascertain the satisfaction of service users, relatives / visitors and professionals as to the standard of care and service provided. At the time of this inspection the inspector had received eleven-service user and nine relative/visitor comment cards. Most comment cards returned gave positive feedback of the quality of care and service. Seven comment cards indicated that staffing levels could be improved. Only specific standards were inspected during this inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3. Assessment procedures are robust so ensuring that all needs are identified and can be met. EVIDENCE: Assessment records for four residents were inspected; these contained all necessary assessment information and risk assessments. Assessments are carried out prior to admission by the homes registered manager or by a senior manager. Residents and visitors spoken with confirmed that assessments of their needs had been carried out prior to admission. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8 and 10 Comprehensive care policies and procedures support staff to meet resident’s needs in a respectful and appropriate manner EVIDENCE: Care plans for four case tracked residents were inspected and appeared to be meeting all assessed needs. Records were seen of consultation with other healthcare professionals and outside agencies where appropriate. Risk assessments, care plans and equipment required to prevent or treat pressure areas was recorded in residents care records. Residents and visitors spoken with felt that staff treated them with respect and maintained their privacy and dignity. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 14 and 15 Residents are enabled to exercise choice and autonomy and are provided with a wholesome and nutritious diet. EVIDENCE: Residents spoken with confirmed they were able to exercise choice and autonomy and were able to control their own personal and financial affairs. The lunchtime meal was served during this inspection. The meals being served appeared appealing and nutritious. The home has two dining rooms, one upstairs and one downstairs, both dining rooms were congenial and staff were seen assisting residents in an appropriate and sensitive manner. Residents spoken with were satisfied with the quantity and quality of meals, snacks and drinks provided and confirmed they could take their meals in their rooms if they wished. The inspector observed a member of the catering staff offering the choice of meals for the following day, a hot choice was available for lunch and tea. One resident praised the catering staff and said they were very helpful and accommodating. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 Complaints and adult protection policies were robust and afforded protection and minimised risk to all service users accommodated. EVIDENCE: The home has comprehensive adult protection and complaints procedures in place. Staff members spoken with had received training in adult protection procedures and were aware of the correct procedures to follow. Residents spoken with said they would feel comfortable making a complaint to staff and were aware of the home’s complaint procedures. JUDGEMENT – we looked at outcomes for standard(s) None of the above standards were inspected during this inspection but were inspected at the last inspection which took place in June 2005. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for standard(s) 27 and 29 The numbers and skill mix of staff deployed were sufficient to meet the needs of residents accommodated. The companies recruitment procedures were robust and afforded protection for residents. EVIDENCE: At the time of this inspection there were fifty-one residents living at the home. The staff duty roster for the week in which this inspection took place was inspected. Seven care assistants and one trained nurse were on duty each morning/afternoon and evening. Three care assistants and one trained nurse were on duty at nighttime. The registered manager hours (full time) were supernumerary. While some comment cards received did indicate that residents may benefit from increased staffing numbers, evidence gathered from case tracking on the day of this inspection indicated that staff were meeting the needs of residents accommodated. The registered manager monitors the dependency levels of residents accommodated and staffing levels are calculated according to these. Senior managers at the company’s head office carry out all recruitment processes and procedures. The company’s recruitment policies adhere to all relevant regulations. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32 and 33 The home is managed by a competent and able manager and is run in the best interests of residents. EVIDENCE: The registered manager has many years management experience and is a qualified nurse. Residents and staff spoken with felt supported by the management team and felt the leadership style was open and inclusive. Residents and staff meetings are regularly held. A comprehensive programme of quality audit was in place and this included resident satisfaction surveys.
