The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The services that are provided by the home meet the needs of the people who live there. Staff members treat residents with respect and are mindful of their right to privacy. One resident stated that it is like staying in a hotel with the added advantage that help is available if needed. Residents stated that they enjoy the meals that are provided. There are two sittings, enabling residents to choose a mealtime that suits them. The standard of accommodation is excellent providing residents with comfortable and homely surroundings in which to live. Residents` needs are met by trained staff members.

What has improved since the last inspection?

Action has been taken to address patches of damp in bedroom 11 (a requirement from the last inspection). The registered manager stated that further action would be taken as soon as work on the roof has been completed. The registered manager has undertaken a survey of residents` views but only received one response. She has produced an amended questionnaire and intends to complete a further survey.

The Home will limit the service to the following Service User categories: Older People 22 (OP) To include a maximum of 4 within the registration of 22 with category of D(E) 10th May 2005 Date of last inspection Brief Description of the Service: Belton House is a 22 bedded residential care home for older people located in the small village of Belton in Rutland. Most bedrooms are well in excess of the Minimum Standard sizes. The home has been the subject of major renovation, which has significantly improved its facilities. This work is nearing completion. A new patio area has been built with level access so that residents can enjoy the well kept gardens. A range of facilities have been provided in the grounds a new putting green, croquet area and petange area with a pavillion (with inside and outside seating area, a disabled toilet and small kitchen for residents / relatives to make drinks). The home orders taxis for residents. There is also a frequent bus service, which able residents can use, and the home has now purchased their own minibus. There are also self-contained bungalows as part of the site where residents can develop wider social contacts. The focus of inspections undertaken by the Commission for Social Care Inspection (CSCI) is upon outcomes for service users and their views of the service provided. This process considers the establishment’s capacity to meet regulatory requirements & minimum standards of practice and focuses on aspects of service provision that need further development. This inspection took place over the course of approximately four and a quarter hours. The primary method of inspection used was ‘case tracking’ which involved selecting three residents and tracking the care they receive through review of their records, discussion with them (where appropriate), staff and observation of care practices. Three residents were interviewed during the course of this visit. The registered manager facilitated the inspection. What the service does well: What has improved since the last inspection? What they could do better: The home met twelve of the thirteen standards inspected on this occasion. It had already taken action to address the one shortfall that was identified – i.e. the ratio of qualified staff (standard 28). No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8 & 10 Residents’ health care needs are fully met. Staff members treat residents with respect and are mindful of their right to privacy. EVIDENCE: Residents stated that staff at the home would take action to ensure that any health care needs were met. Individual plans contain details of any needs that have been identified and of any action that is felt to be necessary as a result. The home maintains records of all appointments with health care professionals. Residents stated that they are treated with respect. They reported that staff members use their preferred form of address and that they knock & wait for a response before entering their rooms. This was observed during the course of the inspection. The requirements of standards 7 & 9 (Service user plans & Medication) were not fully inspected on this occasion. It was noted that issues relating to the use of a resident’s memory box were featured in her care plan. This is seen as good practice. A sample of medication administration records were found to meet relevant requirements. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13 & 14 Arrangements relating to the daily life of residents are well managed. EVIDENCE: Residents stated that they are able to maintain contact with their families & friends. A number of residents received visitors during the course of the inspection. Residents stated that they are able to determine their own daily routine, deciding, for example, when to get up & to go to bed and how to spend their day. One resident stated that it is like staying in a hotel with the added advantage that help is available if needed. Whilst the requirements of standard 15 (Meals & Mealtimes) were not inspected on this occasion, residents stated that they enjoy the meals that are provided. Meals are served at two sittings, enabling residents to choose the time that suits them. JUDGEMENT – we looked at outcomes for the following standard(s): 18 Arrangements for responding to allegations of abuse protect residents’ rights. EVIDENCE: The home has a copy of the local multi-agency policies and procedures on the protection of vulnerable adults. The registered manager indicated that she is aware of the action to be taken in the event of an allegation of abuse. She agreed to amend the home’s abuse procedure to clarify the position with regard to contacting the local Social Services Department. Staff members have received training on the protection of vulnerable adults. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 26 The standard of accommodation is excellent providing residents with comfortable and homely surroundings in which to live. EVIDENCE: The home has been the subject of major renovation, which has significantly improved its facilities. This work is nearing completion. She reported that further action would be taken as soon as work on the roof has been completed. The areas of the home that were inspected were decorated and furnished to a high standard. They were clean and free from offensive odours. A number of residents have furnished their rooms with their own possessions. Two residents stated that they felt cold although a reading taken on the day of the inspection indicated a suitable temperature. Staff took appropriate action to address the residents’ concerns at the time. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 & 30 Arrangements for training staff are generally well managed. EVIDENCE: The registered manager stated that two members of care staff have completed National Vocational Qualification level 3 and one NVQ level 2. Three of the thirteen members of care staff were in the process of completing NVQ level 2 and a further five were about to start the award. The registered manager stated that difficulties accessing funding for the training had prevented staff from obtaining the qualification. New members of staff complete induction training to the standards set by Skills for Care (the Training Organisation for Personal Social Services). Records indicate that staff members have received training on a range of issues relevant to their work. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33 & 35 The home is well managed. EVIDENCE: The registered manager has completed a NVQ level 4 in management & care. Records indicate that she has attended relevant training. The registered manager stated that she had undertaken a survey of residents’ views but had only received one response. She has produced an amended questionnaire and intends to complete a further survey. Progress regarding this issue will be checked at the next inspection. The home maintains records of any money it handles on behalf of residents. Two members of staff had signed the records that were inspected. A random check indicated that the recorded balance was correct.
