What the care home does well

All of the residents who were spoken with said they were very happy, they liked living at Berry Hill, and that the staff were very good. There was clear evidence of activities and occupation for the residents, with many examples of artwork and craft on display. The staff say they are receiving lots of support and training, and enjoy working at Berry Hill. There is a strong relatives group who support the residents and represent their best interests. The building is homely and comfortable and well maintained.

Service Users shall be within categories OP with 1 bed registered for Terminal Illness One service user may be aged 64 years as named in application dated 24/5/05 PD 24th November 2005 Date of last inspection Brief Description of the Service: Berry Hill Park Care Centre is a sixty-three bedded care home providing nursing and residential care, owned by Southern Cross Healthcare. It is a twostorey building within the upper floor accessible by stairs and two passenger lifts. Berry Hill also has extensive grounds. The building is situated on a main road with access to public transport and a direct bus route to Mansfield, which is three miles away. The accommodation was originally divided into two wings but is now regarded as one whole unit. Accommodation consists of a total of 48 single rooms, all ensuite except one and eight double rooms, all ensuite. Staffing provision is primarily by qualified nurses and care assistants for nursing service users and senior carers and care assistants for personal care only service users. The care manager is responsible for the whole complex. There is a car parking facility at the front of the home and an enclosed, fenced garden at the rear, which overlooks a large field, used by the general public. The Inspection took place through the middle of the day with one Inspector present, and lasted approximately four hours. The methodology used was to visually inspect parts of the premises, talk with six residents, three members of staff, and case track three residents. Case tracking involves looking at a range of care records, and making a judgement about the quality of care that residents are receiving. Unfortunately it was not possible to speak to any relatives as none were visiting Berry Hill during the time of the Inspection. On the day of the Inspection there were fifty-five residents in residence. The fees range from: £436 (private residential) to £575 (nursing) per week. JUDGEMENT – we looked at outcomes for the following standard(s): 123&5 The quality outcome in this area is: Good Prospective residents at Berry Hill have the information they need to make an informed choice about where to live. Each resident has a written contract/statement of the terms and conditions of residence for Berry Hill. No resident moves into Berry Hill without having had his or her needs assessed and been assured that these will be met. Prospective residents and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of Berry Hill. Berry Hill does not offer Intermediate Care. EVIDENCE: The Statement of Purpose and Service User Guide were both seen and both contained comprehensive information about Berry Hill and the service offered. Both documents meet the National Minimum Standards identified in Care Homes Regulations. Copies of these documents are on display in the entrance hall, and there is a copy in every resident’s bedroom. Two residents were asked about the service user guide, and both said they had a copy. At the last Inspection issuing every resident with a copy of the Service User Guide was set as a recommendation, which has now been achieved. Three resident’s files Berry Hill Care Home DS0000040343.V299594.R01.S.doc Version 5.2 Page 8 were seen and each one contained a Social Services Assessment, together with a range of in-house assessments. There was clear evidence of residents having been assessed prior to moving into Berry Hill. Each of the files also contained a copy of the terms and conditions of residence. The Service User Guide makes reference to trial visits for residents and their families, and three residents when asked said that they had visited Berry Hill with their families before they came to live there. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 8 9 10 & 11 The quality outcome in this area is: Excellent Resident’s health, personal and social care needs are set out in an individual plan of care at Berry Hill. Residents have their health care needs fully met. Residents are protected by Berry Hill’s policies and procedures for dealing with medication. Residents feel they are treated with respect and their right to privacy is upheld at Berry Hill. Residents are assured that at the time of their death, staff at Berry Hill will treat them and their family with care, sensitivity and respect. EVIDENCE: Three resident’s files showed that each one had an individual plan of care. Reviews of the care plans were found to be timely – Care Homes Regulations suggest care plans for older people should be reviewed monthly. Both requirements and recommendations were set around care plans at the last Inspection, and both were found to have been met. One related to residents or their representatives being consulted during the care planning process, and one related to reviewing care plans regularly. Evidence was seen to show that both these had been met. All three files contained detailed information relating to resident’s health. Berry Hill Care Home DS0000040343.V299594.R01.S.doc Version 5.2 Page 10 Two residents were asked about their experiences of health care at Berry Hill, and both said that if they needed to see a doctor the staff would get one for them. Both residents said they were quite happy with the health care arrangements at Berry Hill. A requirement was set around health care at the last Inspection. A review of the care plans that this requirement related to, evidenced that the requirement had been met. Berry Hill operates a monitored dosage system for their medication. By chance the system was being changed over to Boots the chemists’ system, on the day of the Inspection. The records relating to medication were seen and found to be complete and well organised, with photographs of residents in the records to aid identification, and a doctor’s (G.P.) review form to record when and how medication is being monitored by the doctor. Two residents were asked about medication, neither self medicated, but both said they were quite happy with the arrangements, and that they preferred the staff to sort out their medication for them. With one resident saying: “It saves me having to worry about it.” Observations of staff interacting with residents during the Inspection showed staff to be polite, courteous and to treat residents with respect. Bedroom doors were seen to be knocked on before entering, and staff were observed to be kneeling down to talk to residents on their level, rather than bending over them in demeaning manner. Five residents were asked about how the staff treated them, and every one spoke very positively, and said that they thought the staff were “very good” and “do a fantastic job.” One resident said that: “The staff treat us very well, and we are very happy here.” The comments made by the residents complimented what the Inspector observed during the Inspection. Of the three resident’s files that were seen, two contained information relating to the resident’s wishes at the time of their death, this included identifying burial or cremation, and details of which undertaker to use. Discussions with the manager around this issue identified that some residents and their families are extremely reluctant to discuss issues around death, and therefore it has not been possible to record everyone’s wishes within their file. This issue was identified fully at the last Inspection. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 13 14 & 15 The quality outcome in this area is: Excellent Residents find that the lifestyle experienced at Berry Hill matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Residents at Berry Hill maintain contact with family/ friends and representatives and the local community. Residents are helped to exercise choice and control over their lives. Residents receive a wholesome appealing balanced diet in pleasing surroundings at times that are convenient to them. EVIDENCE: Berry Hill employs an activities co-ordinator, who was present during the Inspection. The Activity co-ordinator takes a lead in making sure that there is entertainment and activities for the residents. During the Inspection a number of residents were playing bingo, and there were many examples of artwork – produced by the residents on display in Berry Hill’s corridors. An area at the top of the stairs held examples of crafts made by the residents. Several residents were spoken with about activities and there was a very positive response, with residents talking about trips out to local areas, and a trip to the seaside being planned. The bingo session was observed for a short while, and there was a happy relaxed atmosphere, although everyone seemed quite keen to win! Berry Hill Care Home DS0000040343.V299594.R01.S.doc Version 5.2 Page 12 A regular religious service is held at Berry Hill for residents, and there is regular monthly entertainment provided by a variety of professional entertainers who come in to perform. Berry Hill has a relatives committee, who organise their own meetings and attendance is by invitation. The committee organise fundraising and social activities, as well as acting as a voice for the residents. Information about forthcoming meetings was prominently displayed throughout the building and each resident receives personal notification of the meeting and an invitation to attend. Two residents were asked about relatives visiting, and both said that this was not a problem. Visitors were made to feel welcome, and could visit at any time. One resident said that it was possible to see visitors in private, although usually “we just find a quiet corner in the lounge.” Berry Hill does have a small comfortable visitors lounge where residents can see visitors, which is equipped with tea and coffee making facilities. Three residents were asked about choice, and each one said they thought there was a lot of choice offered at Berry Hill. One resident talked about the Relatives Committee, and said that they would raise any issues on resident’s behalf. Meals were discussed, and all three residents said that there was always a choice of what to eat, and even if you didn’t like the choice, ‘they’ would find something for you, you could eat. All three residents said they felt in control of what time they went to bed, and got up, and that while Berry Hill did have some rules, they were not a problem. The kitchens were seen during the Inspection and the food being prepared appeared appetising and nutritious. The records relating to food were seen and all found to be correct and complete. Records of choices residents made around food were recorded, along with records of refrigerator and freezer temperatures and food temperature probing. Four residents were asked about the food and everyone said it was very good. “Smashing”, “Lovely”, “Very nice indeed”, and “Oh yes the food is very good” were some of the comments residents made. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18 The quality outcome in this area is: Good Residents and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon at Berry Hill. Residents at Berry Hill are protected from abuse. EVIDENCE: The complaints records for Berry Hill were seen, and these showed that since the last Inspection there had been three complaints – made on the 15th February, 18th February & 25th March. All three complaints were dealt with in accordance with the Berry Hill complaints procedure, and completed within the identified time frame. Two residents were asked about complaints, both said that they had not had to complain, but knew who to complain to if need be. Both identified the Registered manager as the person to whom they would complain. The relatives committee as already identified have an active role, and are involved in some monitoring issues at Berry Hill. The committee does act as another avenue for raising concerns and issues. Four residents were asked if they felt safe at Berry Hill, and each person said that they did. Staff training records show that 95 of the staff have been trained in safeguarding adults issues and the Protection of Vulnerable Adults, with the last training being delivered in December 2005. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 20 & 26 The quality outcome in this area is: Good Residents at Berry Hill live in a safe, well-maintained environment. Residents have access to safe and comfortable indoor and outdoor communal facilities. Residents live in safe, comfortable surroundings. Berry Hill is clean, pleasant and hygienic. EVIDENCE: A partial inspection of the premises showed a well-maintained building, with a comfortable and homely appearance. Five residents were asked about the building, and everyone made positive comments about the comfort and facilities on offer. There is a handyman employed at the home, and two residents when asked said that if things were broken they usually got fixed quite quickly. Berry Hill has four lounges (five if the visitor’s room is included) all of which were found to be well appointed, homely, and comfortable. Berry Hill Care Home DS0000040343.V299594.R01.S.doc Version 5.2 Page 15 Outside there is a spacious garden laid mainly to lawn overlooking public playing fields at the rear, with plenty of room for residents to sit out. Throughout the Inspection the building was found to be clean, tidy and fresh. Cleaning staff were seen to be hovering the carpets and cleaning toilets, while an inspection of the cleaning cupboards found them to be well, and appropriately stocked. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 28 29 & 30 The quality outcome in this area is: Excellent The residents’ needs are met by the numbers and skill mix of staff at Berry Hill. Residents at Berry Hill are in safe hands at all times. Residents are supported and protected by Berry Hill’s recruitment policy and practices. Staff at Berry Hill are trained and competent to do their job. EVIDENCE: The staffing rota was seen, and this showed that there were sufficient numbers of staff on duty to meet the resident’s needs at all times. In addition the rota showed that there were different grades of staff on duty, which evidenced that there is a good skill mix within the team. The staff training records were seen and these showed that the staff team are undergoing an ongoing programme of training and development. One member of staff commented that: “If we are not at Berry Hill working, we are at Kings Mill learning.” (Kings Mill is the local hospital.) Discussions with three members of staff about training at Berry Hill, identified that there was a depth and scope to the training courses being offered, that was relevant to staff working with older people. There is an ongoing programme for staff to be trained in the National Vocational Qualification (NVQ) at level II & III. Berry Hill Care Home DS0000040343.V299594.R01.S.doc Version 5.2 Page 17 Two staff files were seen and these contained all of the information required by Care Homes Regulations (Schedule 2) and this evidenced that Berry Hill have robust recruitment policies. Two members of staff were asked about their recruitment, and both said that they had completed an application form and had a formal interview, before starting, and that they could not, and did not start work until their references and Criminal Records Bureau checks had been completed. As already identified there is a good staff training regime in place at Berry Hill. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 33 36 & 38 The quality outcome in this area is: Good Residents at Berry Hill live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge her responsibilities fully. Berry Hill is run in the best interests of its residents. Staff at Berry Hill are appropriately supervised. The health, safety and welfare of residents and staff at Berry Hill are promoted and protected. EVIDENCE: The Registered manager at Berry Hill is suitably experienced and qualified to hold the post of Registered manager. A review of the manager’s personal file, showed that she has undergone periodic training to update herself, and that she meets all of the requirements of Care Homes Regulations to be the Registered manager. Berry Hill Care Home DS0000040343.V299594.R01.S.doc Version 5.2 Page 19 Three members of staff were spoken with, and all three said they felt well supported, informed and were happy in their roles. The staff saw the training that they were undertaking as being part of the ‘bigger picture’ in that it gave them the skills they needed to meet the resident’s needs. All three staff members said that there was good communication within the team, and that there was a feeling of ‘working together’. Three residents were asked about residents meetings, and said that they were run by the relatives and residents together. The meeting minutes were seen, and these showed that meetings were well attended. There is an ongoing quality assurance system in place, and the management of Berry Hill appears to be open and approachable. Records of formal supervision in staff files show that the staff are being supervised appropriately. Formal supervision is cascaded down through the team, so that every member of staff is getting formal support every six to eight weeks. Two members of staff were asked about formal supervision, and both said they felt very well supported, and that they were having regular formal supervision with a senior member of staff. A range of health & safety records were seen including Fire Safety records, the Gas safety certificate and records relating to the Control of Substances Hazardous to Health (COSHH). All of these records were found to be complete and in order.
