This inspection was carried out on 18th February 2006.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Residents are treated as individuals by staff who were seen to have a caring warm approach. Good assessments, risk assessments and care plans, which are regularly, are undertaken leading to good appropriate care being delivered. The home operates good recruitment procedures. Regular Health and Safety audits identify problems and action needed to sort them out.

What has improved since the last inspection?

The new manager who has been in post for 6 months is providing stability and continued leadership. The home is working towards providing appropriate foods for residents from all cultural backgrounds Electric storage radiators in bedrooms have been replaced and are effective at keeping rooms warm. The management of medicines now follows national guidelines.

What the care home could do better:

Complete the action needed to provide culturally sensitive food. Ensure that all the old radiators are replaced. Take all the actions needed following the safety audit to ensure that the home is safe for residents.

All service users shall be above pensionable age and require care by reason of mental disorder (excluding learning disability) or dementia. Minimum nursing staffing levels indicated in the Notice served in accordance with Section 13 (5) of the Care Standards Act 2000 issued on 29 October 2003 must be maintained. 15th September 2005 Date of last inspection Brief Description of the Service: Beyer Lodge Nursing Home is registered to provide accommodation with nursing care for a maximum of 16 older people aged 65 years and above assessed as requiring nursing care for mental health needs. Anchor Trust operates the home. When there is a vacancy at the home Manchester Mental Health and Social Care NHS Trust have rights to beds at the home and only if the vacancy is not filled within 8 weeks is it offered elsewhere. Residents’ ongoing mental health care is the responsibility of a NHS Consultant in older age psychiatry. The home is situated on Taylor Street in the Gorton area of Manchester. Shops, public houses and other social areas and amenities are within a short walk of the home, which is close to public transport services. The home is purpose built and set in its own small grounds. The home offers a secure, well-maintained and accessible garden area for use by all service users. It has its own car park. There are 16 single, en-suite bedrooms. Along with 3 lounges and dining rooms including one designated for the use of people who smoke. This inspection, which was unannounced was carried out on a Saturday starting at 07:30 am and lasted for 4.5 hours. During the inspection, 8 residents and 9 staff were spoken with. Requirements from the last inspection carried out in September 2005 had either been fully or in one case partially completed. The premises were kept well maintained, clean and tidy and provided suitable accommodation for the residents. During this inspection only a selection of key National Minimum Standards were assessed therefore to gain the full picture of how the home meets the needs of residents, this report should be read with the previous report. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 and 9. Regular reviews of care plans ensure that there relevant to residents needs is maintained. Appropriate medication practices mean that residents are less likely to be put at risk. EVIDENCE: Standards 7 and 9 were fully assessed at the last inspection. Requirements made at that inspection were reviewed at this inspection. The home was required at the last inspection to ensure that all residents had care plans and associated risk assessments that were regularly reviewed. This was now happening and regularly reviewed care plans and risk assessments were in place that meets resident’s needs. New plans were written as changes to needs were identified. At the last inspection it was found that some of the homes medication procedures needed amending to comply with Royal Pharmaceutical Society of Great Britain Guidance. Since that inspection the home has responded to the requirement by amending the way in which medication is obtained and this now ensures the guidelines are followed. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 15. Action that has been started on serving food culturally sensitive to all residents need consolidating. EVIDENCE: At the last inspection the food served was found to be of a good standard but the meals on offer were not culturally sensitive to the needs of all the residents. Since that inspection the home have had discussion with relatives and others including catering staff at other Anchor Homes and types of food and how to cook it. The cook on duty at this inspection talked about how they were trying to meet these needs. The home should continue to do this and record action taken by adjustments to available menus. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 25 and 26. The home is well maintained, has a homely feel, and is kept very clean. The replacement of the remaining storage radiators is required to keep the home at an appropriate temperature. EVIDENCE: Bayer Lodge is a clean home with no offensive odours. It was found to be well maintained and the quality of this pointed out minor matters such as the odd scratch to doors caused by wheelchairs, which need to attended to in the general course of the evident ongoing refurbishment. Corridors, along with some bedrooms, had been redecorated since the last inspection. The homes smoking area, which is a substantial conservatory, did have a brick floor. This has now been covered with a wooden floor that is altogether more homely. The area has also been redecorated. All the communal areas of the home have a very domestic feel enhanced by such small touches as a calendar of by gone film stars, which details information about important dates such as birthdays. Beyer Lodge Nursing Home DS0000021633.V279096.R01.S.doc Version 5.1 Page 13 It was noted during the inspection that there were a number of portable oil filled radiators in the home. The home had been experiencing heating problems and was in the process of replacing the electric storage radiators which are the means of heating and must be completed as soon as possible. The use of the portable radiators was required to keep the home warm. Bedroom storage radiators have already been replaced and the adequacy of the new radiators was obvious. The use and placement of these had been subject to a risk assessment to ensure that they were placed in areas were they presented a low risk to residents accidentally burning themselves. The home had appropriate policy and procedures in place for infection control and staff were seen to adhere to these. JUDGEMENT – we looked at outcomes for the following standard(s): 29 The homes recruitment procedures should ensure that residents are protected from unsuitable staff. EVIDENCE: At the last inspection the home had appropriate recruitment procedures in place apart from having a record in those files seen of a record of the outcome of a Criminal Records Bureau check. Records were now found to contain these. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 and 38 The new manager is having a positive effect in the home. Residents’ monies were handled appropriately and internal mechanisms available to review and report on health and safety matters in the home. EVIDENCE: The manager, who has been in post since September 2005, was found to be having a positive effect on the home. Staff commented that she was very resident orientated and hands on. She is a registered nurse with many years experience who is about to commence the Registered Managers Award. The home used an Anchor monitoring tool called SAM to gain residents and relatives views on their experience of the home. The manager was honest that this had been lower on her list of priorities since coming to the home but stated that she was to commence this shortly. Income and expenditure was clearly shown and records were reconciled by two people on a weekly basis that provided protection for residents monies handled by the home. The manager had recently undertaken health and safety training and notices relating to health and safety practice were seen about the home. An internal Anchor safety audit recently had shown a number of areas that required attention. These must be actioned to ensure resident safety.
