Date of last inspection Brief Description of the Service: Linden Lea is a residential home for 4 adults with learning disabilities with three resident at the time of the inspection. Residents all have complex needs including autistic spectrum disorder and epilepsy. The home is situated in a quite residential part of Quorn village and is close to local amenities such as shops and public transport links. The home is well maintained and decorated and has a large garden to the rear. It was not possible to ascertain the scale of charges on this occasion. This is an overview of what the inspector found during the inspection. The primary method of inspection used was ‘case tracking’ which involved selecting three residents and tracking the care they receive through review of their records, discussions with them and with the care staff and observations of care practices. A plan was made prior to the visit in which available information from the previous inspection report and service history was summarised. The inspection was positive indicating good outcomes for residents. No requirements or recommendations were made. What the service does well: What has improved since the last inspection? Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents’ needs are assessed in order to inform the care planning process. EVIDENCE: All files tracked contained useful assessment documents. These covered activities of daily living and were thorough and comprehensive. One file also contained a community care assessment and care plan that had been completed by his social worker at the time of admission. A separate file contained records of reviews. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,9 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents have comprehensive care plans that focus on the individual and useful risk assessments. These enable them to make choices, take risks and for their needs to be met. EVIDENCE: Care plans were present in all files tracked and they were comprehensive and individual. The format detailed areas of need, desired outcomes and strategies for achieving outcomes. Communication was a focus of the plans giving staff strategies to enable residents to make choices. All files also contained risk assessments covering a broad range of topics such as behaviour, daily living skills and trips away from the home. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15,16,17 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents’ rights, activities within the local community, contact with families and the quality of the food all contribute to a positive lifestyle. EVIDENCE: The two residents who spoke with the inspector stated that they are able to engage in a range of activities within the home, the local community and further afield. One resident enjoys shopping, going on the bus and doing puzzles while another goes to college, swimming and to church with his brother every Sunday. A member of staff described how he takes the third residents out regularly and how he has built up a positive relationship with that resident, despite behaviour that can be challenging. One resident stated that he has friend at another home that he is able to keep in touch with. Both residents stated that the food is good; one likes to go shopping with staff and to help plan menus. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents’ personal and healthcare needs are met and they are protected by medication policies. EVIDENCE: Files contained comprehensive information regarding residents’ personal and healthcare needs. Appointments with healthcare services are recorded along with any outcomes or advice. One resident was supported to attend a GP appointment on the day of the inspection. Medication was inspected and was stored, administered and recorded appropriately. Medication records have been improved in line with recommendations at the last inspection and are clear and readable. JUDGEMENT – we looked at outcomes for the following standard(s): 22,23 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents are protected from abuse and their concerns and complaints are listened to and acted upon. EVIDENCE: The home has a robust and comprehensive complaints procedure. All concerns are recorded in a dedicated file. No such complaints had been recorded since the last inspection. Training in adult protection has been organised which will add to staff’s existing knowledge. The home has a comprehensive adult protection policy and has access to the Multi-Agency Vulnerable Adult Protection document No Secrets. The property is homely, comfortable and safe. EVIDENCE: Both residents who spoke with the inspector stated that they like the house and their rooms in particular. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32,34,35 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents’ needs are met and they are protected by the recruitment and retention of a well-trained, experienced staff team. EVIDENCE: Staff have attended training on managing aggression, first aid, epilepsy and medication. The registered manager confirmed that she has identified an adult protection course and NVQ programmes. Residents who spoke withy the inspector stated that they like the staff. Both said that staff help them. Staff files were examined and indicated a useful supervision and appraisal programme. Two of the files contained only one reference relating to their employment. It is essential that two references are sought. In both cases two referees had been identified on the application form. Staff were observed interacting positively with residents. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37,39,42 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home is well managed. EVIDENCE: The manager is now registered and demonstrated her fitness for the role throughout the process. Residents who spoke with the inspector both stated that they like the manager and appear to find her approachable. Observation of the registered manager on the day of the inspection indicated that the home is well managed. Monitoring visits are carried out regularly within the home. It was not possible to examine other quality assurance documents as the registered manager had to leave the home before the end of the inspection. Fire records were up to date and accurate; drills are carried out very frequently. Other records relating to health and safety were also completed appropriately.
