This inspection was carried out on 22nd August 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The manager and staff have achieved high standards within the home. The administration of the home is good and information is up to date and easily accessed. A wide range of health services, leisure activities and social events are provided. The environment is well furnished in a homely and comfortable style. The communal areas are light warm and clean, there is wheelchair access to the garden area, which is well maintained, and full of colour seating areas are provided. Staff work well as a team with the manager and a high number of staff have worked at the home since it was first opened eight years ago.

What has improved since the last inspection?

There is little room for improvement as indicated throughout the report. However the manager continues to develop and improve as an ongoing process.

The Home is set in its own extensive landscaped gardens that have been designed to enable wheelchair users and other residents easy access.The Home is situated on the edge of a rural area and the village offers only a few shopping facilities. However, the Home is only a short drive from the town of Ilkeston and the city of Derby both of which offer good shopping facilities and amenities.The services offered are 24 hour staffed care, 3 meals per day, personal laundry, heating and lighting, transport a range of leisure activities and social events. This was an unannounced visit that took place at the home over a four hour period additional time was spent in preparation for the visit, looking at previous reports and other documents. During the inspection process a number of documents were examined, including residents’ care files, staff files and records, time was spent looking around the building and speaking to a number of residents, and to the manager and staff. The inspector spent a specific amount of the inspection concentrating on the care arrangements for the two residents for the purpose of case tracking. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1.2.3.4.5. The manager and staff continue to provide a good administrative structure and information is detailed and easily accessed. EVIDENCE: The homes Statement of Purpose was assessed and found to meet requirements. A full and informative information guide is given to each resident on admission. Two random residents’ files were assessed they contained detailed needs assessments. Residents’ signature on the documents indicated the participation of residents in the process Residents are given all of the information that they need therefore they are able to make a positive choice about moving into the home. The home also provides respite care and this is often the way residents experience the home in the first instance, and later make a decision to have a permanent placement. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7.8.10.11 A wide range of health services are available to residents. Residents are treated with respect. EVIDENCE: All residents’ have a general practitioner dental care is provided on a regular basis, ear and eye tests are given as and when needed. Chiropody is provided by both National Health Service and private according to the residents’ preference. Health visits are recorded, as are accidents. Staff were observed talking to residents in a sensitive manner, using the residents preferred address. Residents spoken to spoke of staff being caring and respectful. Staff were observed knocking at residents’ doors before entering. A relative spoken to on the day of the inspection made a point of an occasion when his wife had been in hospital and a member of staff had visited and taken chocolates the relative had been very touched by this gesture, and spoke highly of staff commitment to residents. It was noted that residents’ funeral arrangements had been discussed and their wishes recorded in their individual care plans. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12.13.15. The home provides a wide range of leisure activities and social events. Residents are able to do what they want when they want there are no strict regimes in place. The residents enjoy a high level of visitors to the home. EVIDENCE: An activities programme is in place fun days are held outings are arranged; the home has its own activities room so a range of crafts can be made available to residents. The visitors book indicated a high level of visitors to the home, and during the inspection period a number of visitors were seen coming and going many of the visitors were acquainted with other residents and stopped to talk to them in passing. Relatives and visitors spoken to stated that they were always made to feel very welcome, and they could talk to staff or the manager at any time. Residents spoken to confirmed that there were lots of things to do as one resident said “your never bored here”. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16.18 Minor complaints are dealt with daily as they occur more formal complaints are dealt with appropriately and promptly. EVIDENCE: There had been one minor complaint since the last inspection, which had been dealt with to the satisfaction of all concerned and according to the homes own complaint policy, which is displayed on the notice board. Residents and relatives spoken to stated that they had no complaints but if they did they would talk to the manager in the first instance and they were confident that it would be put right. All staff have received appropriate adult protection training and one member of staff is going to do the training offered by Derbyshire County Council to enable them to train other staff members. Staff had supplied two references and had the required medical and police checks carried out prior to commencing their employment. All new staff are supervised for a period of six weeks before working alone. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19.20.22.23.24.25.26 The environment is warm, light and clean the furnishings are of good quality homely and comfortable in style. EVIDENCE: A number of residents’ rooms were assessed all contained the required furniture as identified in Standard 24. All the residents’ rooms assessed were personalised and reflected the interests of the residents who occupy them. Items of their own furniture and personal possessions were in evidence. There are a suitable amount of toilets and bathrooms provided, toilet and bath aids were provided to enable residents to bathe and toilet safely whilst still retaining their privacy and dignity. Communal areas are spacious warm and clean. Wheelchair access is provided to enable residents to enjoy the well-maintained and colourful gardens provided. Residents spoke to made positive comments about how lovely the gardens were and how some residents enjoyed sitting in them weather permitting. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27.28.29.30 Staff levels are maintained at a level sufficient to meet the identified needs of the residents. Staff are well trained and skill mix is taken into consideration when duty rotas are prepared. EVIDENCE: The staff rotas were examined and found to meet the staffing requirements. Staff have very good training opportunities provided and this is reflected in the very high number of staff who have achieved National Vocational Qualification at level two and three. All staff have received their mandatory training and there is a very comprehensive induction process which is carried out over a six week period. Staff spoken to confirmed that their training opportunities were very good and that they enjoyed the challenge of the training. Two staff files were assessed and found to contain the documents as required in Schedule two of the National Minimum Standards. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31.32 The managers style is open providing good communication channels to residents, their relatives and to staff members. EVIDENCE: The manager is appropriately trained and has a number of years experience in the caring profession, she presents as competent and confident. Staff spoken to during the inspection stated that they had confidence in the manager and would not hesitate to take problems/concerns/complaints to the manager and they were confident that appropriate action would be taken if necessary. There are regular staff and residents’ meetings taking place, and it was evident that both residents and relatives were able to talk to staff.
