Conditions of registration: NONE Date of last inspection 05/03/05 Brief Description of the Service: Braywood Gardens is owned by Nottinghamshire County Council Social Services and is a purpose-built care home for up to 60 older people. The home opened in 2002 and is a two-storey building divided into four residential units. There is a shaft lift to assist independent access between floors. All of the homes bedrooms are for single occupancy with ensuite facilities. The home is located within a housing estate, close to local amenities, which include shops, pubs and a leisure centre. There are well maintained gardens which are accessible to residents. Car parking is provided at the front of the building. One of the units provides intermediate care and is jointly funded and staffed by National Health Service and Social Services personnel. There is also a day centre for people living in the community, the day centre has separate facilities and operates seven days each week. This is an overview of what the inspector found during the inspection. This unannounced inspection was completed over two days and lasted for approximately 11 hours. It included the inspection of care and other records, a discussion with the manager, two care staff, as well as speaking with 10 residents and one visitor to the home. A partial tour of the building was also completed What the service does well:
The residents are very happy with the care they receive and made many positive comments. They describe the staff as helpful and considerate, two people said that the staff are ‘marvellous’. None of the residents spoken with could think of anything that can be changed, to improve the services provided by the home. The observed interaction between staff & residents was of a very good standard. The manager is well qualified and experienced and ensures that the home runs in the best interests of the residents. The residents stated that the manager is very approachable and that she seeks to include them in making decisions about the way in which the home operates. The staff have received a considerable amount of training since last inspection. All of the residents spoken with said that they enjoy the food provided at Braywood Gardens. They said that the homes routines are very flexible and that they can choose where and when they wish to eat their meals. The premises are purpose-built; they are well equipped and maintained to a very good standard. The bedrooms are spacious and all have ensuite toilet and shower facilities. People have been encouraged to personalise their bedrooms with pictures ornaments and small items of furniture. This was a very positive inspection with aspects of good practice highlighted in the main body of the report. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,3,5,6, Prospective residents needs are comprehensively assessed prior to admission. People are given the information they require, to enable them to make an informed choice about whether the home and meet their individual needs. The Intermediate Care Unit is helping people to maximise their independence. EVIDENCE: At the last inspection a requirement was made that additional information must be included in the homes Statement of Purpose. The manager has included all of this information in the literature that gives specific information about Braywood Gardens. She also informed the inspector that the standard Statement of Purpose documentation, produced by Nottinghamshire Social Services would be revised and reproduced in the near future. It was recommended that the summary of residence views of the home be produced in a user-friendly format. Of the three residents records checked during this inspection, two people had Extended Social Work Assessments, which had been obtained prior to their admission to the home. The other person was admitted to a residential home some considerable time ago and the manager believes that the preadmission

She stated that residents would never be admitted without a social work assessment. All of the service users spoken with during the inspection confirmed that they had been offered an opportunity to visit the home prior to their admission, so that they could decide if it would meet their individual needs. Intermediate care is provided in a separate unit within the main building of the home. This unit has specialist facilities, equipment and trained staff to provide intensive rehabilitation to enable service users to return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10. Not all Care Plans are implemented within the first seven days of residents admission to the home, nor are they all reviewed at least once a month, to ensure that staff are always aware of what assistance and support each resident requires. Resident’s health care needs are being fully met, they are treated with respect and their right to privacy is upheld. EVIDENCE: The care plan of one of the residents in the Intermediate Care Unit was viewed during the inspection. It was comprehensive and had been completed by a multidisciplinary team including a physiotherapist, trained nurses and support staff. Staff have obviously worked hard to produce care plans for all of the residents, which clearly set out how their personal and care needs should be met. However, one of the residents whose records were Case tracked during this inspection did not appear to have had a care plan produced until they had been in the home for approximately 4 weeks. The manager stated that people would normally have an initial care plan produced in the first week, following their admission to the home. This issue

He believed that an initial care plan had been produced but could not locate it. The residents and their representatives had not signed any of the care plans viewed during the inspection, nor had they been appropriately dated. There was also no evidence to show that these care plans are being updated/review at least once a month. The care records show that peoples health care needs are being appropriately met. The homes medication systems were checked in one of the four units. The medication is stored safely and the records of receipt and disposal of medication are well maintained. The staff who administer medication have all received appropriate training. (Staff training records were checked as part of the inspection). The units controlled medication was checked at random and had been well maintained. At the time of inspection none of the residents in this unit were administering their own medication, however an appropriate risk assessment format is available. All of the residents spoken with during this inspection said that staff are always friendly and respectful and that they ensure that their privacy and dignity is maintained at all times. The observed interaction between staff and residents was of a very good standard. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15. Residents are provided with an impressive range of activities and entertainment. People are encouraged to maintain contact with family and friends. The diet provided for residents is wholesome, well balanced and varied. To ensure the health and safety of the residents the records of food, refrigerator and freezer temperatures must be completed on a regular basis. EVIDENCE: There were posters advertising the various events and activities displayed throughout the home. (This is considered good practice). Eight of the residents spoken with said that they are very satisfied with the level of activities and entertainment provided by the home. One person said that they would like more activities but confirmed that they do not always participate in activities that are provided. One person said that they particularly liked the “pub” evenings held each Thursday, which their son attends on a regular basis. All of the people spoken with during this inspection, said that they can have visitors at any time and that their visitors are always made welcome. One visitor to the home said that she finds staff welcoming and friendly. Residents believed that they could invite their visitors to have a meal with them in the home, if they chose to.

There is a six weekly rotating menu, which provides a good variety of food. The cook stated that there are catering meetings approximately every two months and that she asks the residents individually if they would like anything special to be included on the menu. (This is considered good practice). All of the resident spoken with were full of praise for the food provided within the home, they stated that there is, always plenty of food available and that an alternative will be provided if they do not want the food suggested on the menu. People said that they can choose where they wish to eat their meals. One person said that they have all their meals in their bedroom. Each of the units at Braywood Gardens has a small kitchen area where people can make themselves drinks or snacks if they are assessed as safe to do so. (This is considered good practice). It was noted during the inspection that the food temperature records were missing for the end of March all of April and some of May 2005. The refrigerator and freezer temperature records had also not been completed every day. To ensure the health and safety of the residents, it is required that these records are kept up-to-date. All of the residents believed that they are encouraged to make choices about all aspects of their daily lives. People are encouraged to bring personal possessions into the home. Many of the people spoken with said that having their personal belongings in their bedrooms had helped them settle into the home. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,17,18. Braywood Gardens has a simple, clear and accessible complaints procedure. The residents spoken with believe that any concerns/complaints they may have, would be thoroughly investigated and appropriate action taken. Residents had been given the opportunity to vote in the recent general election. Appropriate procedures are in place to protect residents from abuse. EVIDENCE: A copy of the homes complaints procedure is displayed in each of the four units at Braywood Gardens. The homes complaints records were assessed as part of this inspection. Four complaints had been received, in the last year and all of these had been dealt with appropriately. Three of the complaints were not substantiated and one resulted in a resident choosing to move to another bedroom. Eight residents said that they were confident that if they had any complaints the senior staff would deal with them appropriately. All of the residents spoken with said that they had been offered the opportunity to vote in the recent General Election. The home has an appropriate Whistle Blowing Procedure and a copy of the local Protection of Vulnerable Adults Procedure. The member of staff interviewed during the inspection was fully aware of these two procedures. (This is considered good practice). The inspector was informed that there have been no allegations of abuse in the last year. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,24,26. The purpose-built accommodation is maintained to a very good standard. At the time of inspection the home was clean and there were no offensive odours. EVIDENCE: A partial tour of the premises was completed as part of this inspection. The purpose-built accommodation is spacious well decorated, comfortably furnished and maintained to a very good standard. The gardens are attractive and accessible to all service users. The residents spoken with all said that they are very satisfied with their bedrooms, they confirmed that they were encouraged to bring photographs ornaments and small items of furniture to personalise their individual rooms. People said that they could use their rooms at any time; one person said that they choose to spend most of their time in their bedroom. At the last inspection it was recommended that the carpet in one of the bedrooms be changed to a more appropriate floor covering. This room was checked and the carpet had been replaced.

The laundry floor finishes are impermeable and the wall finishes are readily cleanable. The relevant sections of the homes policies and procedures for infection control were included in the Handbook provided to the staff working in the laundry. It was noted that the COSH data sheets for one of the chemicals used in the laundry was not available. The manager agreed to provide the appropriate data sheet immediately. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,30. The home had sufficient experienced and trained staff to meet the assessed needs of the residents. EVIDENCE: The staff duty rotas for the week of this inspection were checked and staffing levels were sufficient to meet the assessed needs of the current residents. The inspector was advised that all of the staff providing personal care to residents are over 18 years of age and that all of the staff left in charge of the home are over 21 years of age. The homes records show that staff have completed a comprehensive programme of training and development. 18 members of ‘care’ staff have completed a NVQ level 2 qualification, 11 people are currently completing the level 2 training and 2 people are waiting to commence the training. (This is considered good practice). The manager stated that all new staff would complete an “in-house” inductiontraining programme within six weeks of commencing employment. They would commence are TOPSS induction training programme as soon as possible. (No longer than two months after commencing employment). Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,35,38. The home is well managed and run in the best interests of the resident’s. Appropriate accounting and financial procedures are in place to safeguard residents. EVIDENCE: The homes manager has been employed in management capacity with social services for over 20 years; she has been a registered manager since 1990. She has a professional social work qualification and informed the inspector that she has successfully completed the NVQ level 4 Managers Award. She was able to demonstrate that she has completed a considerable amount of training to update her knowledge, skills and competence. The staff and residents spoken with during the inspection said that the manager is very approachable and seeks to involve them in making decisions about the way in which the home operates. There are regular resident and

The home operates Nottinghamshire County Councils Quality Assurance System. Performance and development plans are produced from the information collated as part of the quality assurance system. At a previous inspection it was recommended that the homes satisfaction questionnaires be extended to service users families and professionals concerned with the home. The manager stated that satisfaction questionnaires are available in the reception area and that people are encouraged to complete them. The inspector stated that the information on the current satisfaction questionnaires would not be appropriate for professionals visiting the home. It was therefore recommended that a separate questionnaire be produced for professional people who provide a service to the residents at Braywood Gardens. The records of resident’s finances were checked at random and found to be well maintained. All of the residents, who were asked, said that they were satisfied with the way in which their finances are managed. Two people said that their families were managing their financial affairs. Records and receipts are kept of possessions handed over for safekeeping. Issues in relation to safe working practices were assessed at random and were maintained to a good standard. The inspector was informed that there are no issues outstanding from the Fire officer’s last visit. The homes Fire records have been maintained to a good standard. Environmental risk assessments have been completed and regularly and appropriately updated. Legionella risk assessments have been completed and water tests are carried out on a regular basis. It is required that care plans be reviewed at least once each month in consultation with the individual resident, and where appropriate their representative. (The home should produce some method of evidencing that this process has been completed). It is required that the records of food, refrigerator and freezer temperature is are updated each day.
