This inspection was carried out on 29th September 2008.

CSCI found this care home to be providing an Poor service.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

People are now having their oral hygiene needs met by staff and records are being kept. Care plans are being reviewed frequently and records kept of this. The staff in the home are now using single use lancing devices when monitoring peoples blood glucose levels. The manager is aware of what events need to be notified to us. Staff are now disposing of soiled continence pads appropriately and not leaving them around the home.

What the care home could do better:

The home needs to complete the transfer of all care records to their new format and make sure that care plans are personalised to each person. When staff have recorded that a wound or injury is to be reviewed then this must take place and records must be maintained of this. The environmental issues identified must be addressed. This includes odours found in several rooms and commodes that had splits in the lid cover and rusty areas to the frame. The sink top in room 18 still needs to be replaced. The stained armchairs in the lounges by the main entrance still need to be cleaned or replaced. The carpets in the main entrance are still heavily stained and need to be cleaned or replaced. The home needs to continue to audit and monitor any unexplained bruising and injuries.

It is registered to accommodate older people with dementia. The home is situated off the A38 Gloucester to Bristol road and is approximately 20 miles from Gloucester. It is a detached house situated in attractive grounds. The communal areas are situated on the ground floor and include 3 lounges and a dining room. There are 25 single bedrooms (7 with en-suite) and 9 double bedrooms. The home makes information about the service, including CSCI reports available to people through a Service User Guide and Statement of Purpose available in the home. Care Homes for Older People Page 2 of 7 What we found: The purpose of this random inspection was to follow up on some of the requirements issued at previous inspections. The home has started to review and re-write care plans for some people who use the service. People are now having their oral hygiene needs met and staff are recording this. Some of the environmental issues identified at the last inspection have been addressed. An audit has taken place of the 18 un-witnessed accidents and incidents that were found at the last inspection. The manager said that all staff have received training in abuse and the local reporting procedures. The manager confirmed that she is planning to apply to us to be considered for registration. Yes R No £ Outstanding statutory requirements These requirements were set at the last inspection. They may not have been looked at during this inspection, as a random inspection is short and focussed. The registered person must take the necessary action to comply with these requirements within the timescales set. No. Standard Regulation Requirement Timescale for action 1 7 15(1) Care plans must be devised 16/09/2008 for people who use the service based on their individual assessed needs. This is to ensure their health and well-being are being met. This requirement remains outstanding since the last two inspections and has been partly met at this inspection. 2 12 16(n) To make sure people who use the service are stimulated a programme of activities based on peoples choices, needs and abilities must be put into place. This requirement was not followed up at this inspection. 31/08/2008 3 19 23 The issues identified in the 16/09/2008 Environment standards must be addressed as a matter of urgency to make sure people who use the service are not put at unnecessary risk. This requirement has only been partly met since the last inspection. 4 26 16(k) Odours in the home need to be eliminated to ensure 29/07/2008 Care Homes for Older People Page 4 of 7 people who use the service live in a pleasing and pleasant environment. This requirement is outstanding from the last two inspections. 5 30 18 1(ci) All staff must receive training 16/09/2008 pertinent to their roles to make sure people who use the service are not put at unnecessary risk by their actions. This requirement was not followed up at this inspection. 6 31 8 A competent, skilled and 06/11/2008 suitably experienced person must be appointed to manage this home and apply to us to be considered for registration. To make sure that the health, safety and welfare of people who use the service is protected. A system must be devised to 31/08/2008 make sure all staff are appropriately supervised and that they can meet the needs of people who use the service.
