Page 2 of 8 Date of last inspection Brief description of the care home Brockhurst provides care and accommodation for older people over the age of 65. It was purpose built in the 1970s and is owned and managed by Surrey County Council. It is located in Ottershaw, near Addlestone, in a quiet residential area, close to local amenities. Accommodation is arranged in seven units on two floors, all rooms being single. Each unit has its own bathroom and toilet facilities, a kitchenette, and a communal lounge/dining area. The reception area, main kitchen and laundry are on the ground floor and there are offices on both floors. There is a lift and stairs to the first floor. This inspection was a specialist pharmacist inspection to look at the handling of medicines in the home following concerns that were brought to the attention of The Commission earlier this year. We looked at the storage of medicines, records kept in the home, staff training, policies and procedures and talked to staff. People who use this service are given their medicines by care staff who have been assessed as competent to handle medicines. Detailed written procedures are available to staff to provide additional guidance and to enable medicines to be handled consistently by all staff. No one was looking after their own medicines when we visited. This can be arranged if people wish to do this. Clear records were kept to show when people had their medicines. These showed that mostly people get their medicines correctly. However we did see that of the 14 people whose medicines we looked at one had not had one of their medicines for 4 days as the supply had run out; another person had not had a cream applied for a week when the supply had run out and they had not had a second medicine for 3 days when staff failed to find the supply that was in the home. This could have put these peoples health at risk. Clear records were being kept when Warfarin is given to people. When people are given this medicine a second carer checks and make additional records of what is given so as to ensure that it is given correctly. Warfarin dose changes are notified to the home in writing and a copy of this information is kept with the medicine records so staff can refer to it when they give the dose. When it was necessary to make additional entries on the record charts supplied by the pharmacy these were done accurately. It is however good practice to have these entries checked by a second carer to ensure that they are accurate and people get their medicines correctly. The home has a system for monitoring the medication administration record charts to check that people are getting their medicines. This had picked up a small number of issues. There was no evidence of the action that had been taken to deal with these and to prevent them from reoccurring. Medicines on the units were stored securely so as to protect the people who use the service. A Controlled Drugs cupboard was available to provide extra security to these medicines. This cupboard would comply with the Misuse of Drugs (Safe Custody) Regulations 1973 if it were correctly secured to a solid wall. Controlled drugs awaiting return to the pharmacy for disposal were not being kept in the controlled Drugs cupboard, as the law requires. Additional records were kept of the usage of Controlled Drugs so as to readily detect any loss. These were well maintained. What the care home does well: What they could do better:
