What the care home does well

During this inspection, some of the residents were out on a trip visiting a local garden center. People living at Brockshill Woodlands are given regular opportunities to participate in recreational activities outside of the home. Staff and residents spoken with spoke positively about interim management arrangements and felt that both senior managers were approachable and would listen to their concerns.

What the care home could do better:

Some people living at Brockshill Woodlands have advanced cognitive impairment and can display challenging behaviour such as verbal or physical aggression. Some staff had not received sufficient training on how to manage challenging behaviour or how to keep people safe. Care plans did not provide enough detail about the actions staff should take and did not consider possible reasons for this behaviour or how the well being of the individual could be increased in order to reduce this type of behaviour. This has resulted in frequent episodes of verbal and physical behaviour towards staff and sometimes towards other residents. One resident told us they feared other residents. Staff did not always follow safeguarding procedures and had not referred such behaviour to the appropriate agency for investigation. Two people we case tracked were at risk of falling and had a history of frequent falls, this was not sufficiently addressed within care plans and therefore risk was not minimized, however on the day of this inspection a referral was made to a community nurse who specializes in the management of falls risk. Not all staff had received training regarding the mental capacity act or deprivation of liberty safeguards. This training is required by staff who care for people who may lack the mental capacity to make some decisions and where staff may need to take action to deprive liberty in order to protect people from harm.

Since the last key inspection concerns have been raised regarding some care and some management practices at the home and these matters are being investigated by the Care Quality Commission, social services and the police. The providers have removed the registered manager from the home and have assigned two senior managers to manage the service while the concerns are being investigated. The purpose of this inspection was to check compliance with Care Home Regulations and that people living at Brockshill Woodlands were having their needs met and were protected from harm while interim management arrangements were in place. The main method of inspection used was case tracking which meant selecting seven residents and tracking the quality of their care by checking records, discussion with them and with staff and observation of care practices.
