The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The manager is very experienced and has been in post for several years and has a staff team that is well trained and able to meet the needs of the residents. This home is one of a group of three and a training co-ordinator is employed for the group to ensure that training needs are addressed. Staff seen during the visit seem to have established good relationships with the residents and their families. The residents receive good meals, various activities are available and visitors are encouraged. The home was has a warm, homely and welcoming atmosphere.

What has improved since the last inspection?

The fabric of the home is being well maintained, so that all areas are clean, odour-free and pleasant to use. The garden has been completed and is enjoyed by the staff and residents. The home won two prizes for their gardens in the `Gosport in Bloom` competition.

What the care home could do better:

The registered manager and her staff team should continue their hard work and maintain the standards of care that they provide for the residents.

A maximum of 4 service users can be admitted between the ages of 55 and 65 years. That only 1 service user within the category of learning disability with the DOB 15/03/41 can be accommodated. That there is at least one full time registered mental nurse employed as part of the staff team. 2nd June 2005 Date of last inspection Brief Description of the Service: Bury Lodge is situated in Alverstoke, opposite the Gosport Memorial Hospital, close to shops and other local amenities. It is a care home providing nursing care for service users over the age of 55 years who have dementia. The home may also take one service user with a learning disability. The accomodation is over two floors and consists of fourteen single and three shared bedrooms, with two communal areas on the ground floor. There is a shaft lift for access to the first floor bedrooms. The home has a secure and attractive garden that is easily accessed by residents. The home is decorated to a high standard and is well maintained. This inspection was the second of two annual inspections for the inspection year 2005/06. The visit was unannounced, took place over one day and was undertaken by one inspector. The inspector was told that the service users prefer to be referred to as residents. The inspector was made welcome by all staff and residents. The registered manager, Mrs Richardson, was available throughout the inspection. The inspector was able to look around the home, view records, policies and procedures and talk to the staff and residents. The residents and visitors that spoke to the inspector were very positive about the care provided and the facilities available within the home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 The homes policies and procedures for medication protect the residents. EVIDENCE: The home has medication policies and procedures in place, which cover procedures such as receipt, storage and administration of medication. None of the residents have chosen to administer their own medication. The home has a contract with a local chemist for the disposal of medications, which complies with the recent changes in legislation. All medications are administered by the registered nurses, who have recently undertaken training in medication administration, which was provided by a pharmacist from a national chain of chemists. Staff that spoke with the inspector said that the training had been informative and had up-dated their professional skills and knowledge. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13 and 14 Residents are encouraged to exercise choice and control in their lives, and their visitors are encouraged. EVIDENCE: Residents are encouraged by the staff to maintain contact with friends and family and the home has an open visiting policy. They can entertain their visitors in the communal rooms or in the privacy of their bedrooms. During the tour of the home the inspector noted that the bedrooms had been made homely and personalised with the residents own possessions, such as pictures, ornaments and small pieces of furniture. Residents are encouraged by the staff to choose what clothes they will wear, what time they get up and what time they will go to bed, this information is contained in the personal care plans. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 and 30 Residents are protected and their needs met by an appropriately recruited and trained staff group. EVIDENCE: The staff skill mix includes qualified nurses, carers and ancillary staff for cleaning, catering and laundry services. The number of staff is sufficient and appropriate to meet the need of the residents. The home is part of a group of homes, which also employs gardening and maintenance staff. The home has robust recruitment policies and procedures in place. All care and nursing staff applicants are interviewed by the matron or director of care. Four staff files were examined by the inspector and found to contain suitable application forms, which had been completed. Proof of the applicant’s identity and documentary evidence of relevant qualifications was seen. Written references and CRB (Criminal Records Bureau) disclosures had been obtained before staff started work in the home. The home is one of a group of three, which has its own training department. The training co-ordinators organise the training for all staff. New care staff undertake the ‘Skills for care’ training as well as statutory training. All staff have undertaken statutory training, which includes manual handling, fire safety and adult protection. Seventy-five percent of the care staff have completed an NVQ (National Vocational Qualification) at level 2 or 3 and more staff have recently started the course. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 35 and 38 An experienced manager with good staff management, which is backed by sound procedures, runs the home in the best interests of the residents and safeguards their financial interests. EVIDENCE: The registered manager is a registered nurse, who has been in post for several years. She has recently completed a mentors course, which is required for her work with the overseas adaptation nurses that are working in the home. She is also doing her Registered Managers Award, which is an NVQ in management. The home has a comprehensive set of policies and procedures, which include Heath and Safety, Moving and Handling, Adult Protection and Record Keeping. The manager told the inspector that the policies and procedures are currently being reviewed and updated. Others keep small amounts of pocket money in the homes safe. This money is kept in separate, labelled, plastic wallets in the safe in the office. The balance was checked on three accounts and found to balance. When a resident is admitted an inventory is kept of all their personal belongings and possessions. Improvements in practices and processes such as all open packets of food being stored in appropriately labelled, sealable containers and individual accident reports are stored securely and separately from the Accident Report book, as detailed in the Data Protection Act mean that residents health, safety and welfare is being better safeguarded and promoted.
