Conditions of registration: 1. 3 (three) people under the age of 65 years may be admitted to the home on the condition that they fully meet the assessment criteria or who have early onset dementia. 26th October 2005 Date of last inspection Brief Description of the Service: Bushbury Court is a care home providing nursing care for up to 21 people with mental health problems usually over the age of 65 years, on a continuing care basis. It is owned by the Primary Care Trust (PCT) and Managed by Heantun Care Housing Association on their behalf. The home is situated in the Bushbury area north Wolverhampton and is within easy walking distance of local shops, which includes a Library, hairdressers, pub, church and doctors surgery. The home consists of a single storey purpose built building. Twelve of the bedrooms are single occupancy and four twin rooms. The communal areas are spacious and homely. This is an overview of what the inspector found during the inspection. The announced inspection took place over 3 hours on Monday 13th March 2006. This inspection was announced at short notice to the home, the pre inspection questionnaire was returned as agreed at the inspection. Staff, service user and relative questionnaires were distributed but none were returned at the point of writing this report. The inspection included observing activity within the home, a tour of the premises, looking at records, and staff and service user files along with care plans were inspected, together with supporting documents. The manager, three care staff and a registered nurse were on the premises at the time of the inspection; they were welcoming and offered their fullest cooperation throughout the inspection. 11 people are currently living at the home; this is below capacity and the service is currently under review by the PCT with a view to broadening the range of service users it may accommodate. On this occasion a total of 12 out of 38 National Minimum Standards were assessed. The other standards considered to be key were assessed at the inspection on 26th October 2005. Most of the National Minimum Standards were met; the home provides a good service for people; consequently, on this occasion only those standards identified as “key” by CSCI have been inspected. What the service does well: What has improved since the last inspection?
Staff supervision has greatly improved since the last inspection and this must be maintained.

Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8&9 The staff are sensitive to the individual health needs of each service user and meet these in a professional manner. Health care needs are met and the medication was well managed promoting good health. EVIDENCE: The health care needs of people who live at Bushbury Court are fully met. The Manager stated that GPs and consultants visit the home on a daily/weekly basis; this was supported from notes in the records. The surgery for most service users is located next door to the home. District nurses Doctors, chiropodists and opticians visit the home and this process is recorded. All other specialist health care services are accessed when the need arises; health care needs are reviewed monthly. The home operates a 28-day cycle of medication administration in the bottle and box system. The medication administration records are completed each time.

A list of specimen signatures to enable staff identification when administering medication was held on the file. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 & 14 There was a range of social, religious and recreational opportunities available to meet the expectations of the service users who live at Bushbury Court home. Visitors were welcomed in an open and friendly manner and service users were supported and encouraged to maintain contact with family and friends and to make their own choices about their day-to-day lives. EVIDENCE: Formally social activities are arranged throughout the year, this has included outings using community transport and events undertaken on a one to one basis. Generally activities are arranged on a daily basis to suit the needs of service users on that particular day. During the inspection, staff were observed welcoming relatives and friends of the service users and Manager confirmed that there was open visiting for family and friends. The registered manager explained that wherever possible relatives and representatives handle the financial affairs of service users. Staff assist service users to make choices of their every day life.

Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18 Concerns and complaints are dealt with promptly and professionally and procedures and practices are in place to ensure that individuals are protected from abuse. Service users are protected from abuse by the home’s Adult Protection procedures regular training in Adult Protection and Abuse Awareness takes place. EVIDENCE: There had been no complaints made to the home since the last inspection and none had been made to CSCI. There was a complaints procedure on site and this was issued to the residents in the service user guide, complainants are aware they can lodge a complaint with CSCI at any time. No incidents of neglect or abuse of any kind has been reported. The policy documentation was seen, the manager confirmed that residents are protected from all forms of abuse. At the time of this inspection all staff had received training in adult protection issues and this included how to report any issues. There were appropriate policies and procedures on site and also a copy of the updated multi agency guidelines for adult protection. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 26 The home was very clean throughout and contributed to the overall control of infection. EVIDENCE: At the time of the inspection the home was clean, pleasant and hygienic. Laundry facilities were found to be well organised, clean and hygienic. Foul laundry was washed at the appropriate temperatures to ensure it was thoroughly clean and to control the risk of infection. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28 & 30 Staffing numbers and the skill mix was appropriate to meet the assessed needs of the service users. A thorough recruitment and selection procedure was in place and ensured the continued protection of the service users. Some improvement is required in relation to the keeping of CRB records at the home. EVIDENCE: At the time of the inspection, there were 11 service users accommodated at the home, all with continuing care needs. The recruitment and selection procedures within the home were good with exception of some CRB information. Minor improvement to CRB records in relation to recording the protection of Vulnerable adults status of individual staff members is required. Three staff files were examined and each contained an application form, a record of CRB clearances, but not in sufficient detail in relation to POVA clearances. Two references and details of qualifications and training proofs of identity were also held on each file including a recent photograph of the individual member of staff, all other information was well-documented and showed a thorough approach to ensuring the protection of those living within the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,35 & 38 There was a good atmosphere within the home and the home is run in the best interests of the people who live there. Records were stored safely, kept up to date and accurate to safeguard the rights of the residents. EVIDENCE: The care manager is a first level nurse and has the skills, experience and qualifications to manage the home on a day-to-day basis. She is currently waiting for the outcome of her registered managers award. The manager confirmed that further training has been accessed to update knowledge, skills and competence. Relatives and representatives are encouraged to handle financial matters on behalf of service users. The home operates a petty cash system for sundry expenditure with the head office issuing invoices for receipt of payments.
Bushbury Court Nursing Home DS0000017182.V275810.R01.S.doc Version 5.1 Page 16 Systems are in place for promoting and protecting the health, safety and welfare of service users and staff The manager was a competent and experienced person who, with her staff, had developed an open and positive approach creating a relaxed and inclusive atmosphere within the home. This running of the home was supported by robust procedures and policies, developed and communicated to staff. Staff meetings took place to enable staff to contribute to the way in which the service was delivered and feedback was actively sought from residents via an annual questionnaire, which was sent out each year.
