Conditions of registration: N/A Date of last inspection 24/01/05 Brief Description of the Service: Canwick House is a large three storey, listed building with a modern extension. The home is situated in a small village, which is approximately one mile from the centre of the city of Lincoln. The home has been adapted and extended to provide accommodation for up to twenty two residents to receive personal care. The home is accessed via a shared driveway which opens on to a small tarmac car park. There is a local church nearby with all other ammenities located within the city centre. The home is not located on a bus route and residents rely on relatives or the proprieters for transport when this is needed. To the rear of the property are secluded grounds and gardens, which are well maintained. Accommodation for residents is on the ground and first floor of the building. The second storey is reserved as the private accomodation of the proprieter. The home is run as a family business and the proprieter is fully involved in the management of the home as well as the delivery of personal care. This is an overview of what the inspector found during the inspection. This unannounced inspection was undertaken over a five hour period during which the Inspector toured the building and talked with residents and care staff using a method of inspection called “Case Tracking”. This involved selecting three residents in order to track the care they receive through the checking of their records, discussion with them, the care team and observation of care practices within the home. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 and 4. Residents are only admitted to the home if the manager and senior staff are sure that their needs can be met. EVIDENCE: Three residents told the inspector that their needs were assessed before moving into the home. There was evidence of pre – admission assessment information on Residents files and that service users are given an information pack about the home prior to admission wherever possible. One resident showed the inspector information about the home which she had been given before admission which explained how support would be provided by the home. The manager/senior carer undertakes assessment visits before admission and explains that all new residents who move into the home stay for an initial trial period so that needs can be further assessed together with the resident. As part of this process new residents and their relatives are encouraged to visit and to look around before deciding to move in to the home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,9,and10. The Manager and Care team know all residents well and respect their privacy and dignity in a variety of ways. Comprehensive care plans are used to ensure residents health, personal and social care needs are met. Systems are in place to ensure the safe storage, recording and administration of medication. EVIDENCE: All residents have an up to date care plan which shows how each persons daily need is being met. These plans are kept in the managers office with residents having access whenever needed. Care plans were found to be accurate and included health, social and personal preferences of each individual. Each resident is allocated a key worker and wherever possible the care team encourages residents to self medicate. Only senior staff who have been trained to give medicines do so. Care plans include a risk assessment so that where support is needed with medication it is discussed together with the resident and relatives. The home uses a “nomad” cassette system for storage and dispensing of residents medicines which was observed in use on the day of inspection along with Medication records which were fully up to date. Staff were seen to be talking to residents in a gentle and caring way, ensuring that privacy was respected. Locks are provided on doors to residents rooms with a key whenever requested. One resident told the inspector “I have brought all

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13and15 The importance of individual and group activity for residents is taken seriously. There are good relationships between the staff, Manager and residents which supports individuals in retaining choice and control over their lives. The food in the home is of very good quality, which ensures choice, variety and a nutritious diet. EVIDENCE: Friends and relatives were observed visiting the home and told the inspector that they were always made to feel welcome. Care plans include details on how residents like to spend their time and what they prefer to do. The manager and senior carers discuss needs together with residents through group meetings and individual discussion. The manager takes responsibility for organising activities both in and outside the home. A group of residents told the inspector they liked dominoes and that this was a regular feature within their weekly activities. Another resident said “I listen to my own talking books which my daughter brings in for me, I listen to them whenever I want to”. Other religious cultural needs are detailed in care plans and are arranged to suit residents needs. There was evidence of a wide range of outside activity trips which are organised by the manager through discussion with resident. The Care team regard nutrition as an important part of the care provided and there was evidence of a varied, nutritious menu which is planned on a monthly basis through full discussion with residents. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,22 and 26 The home provides a clean, welcoming, comfortable and homely environment for all residents. EVIDENCE: The home is very clean, well maintained and decorated to a high standard. The area outside the home is also well maintained and landscaped by a gardener who attends to the grounds on a weekly basis. Each room is individually furnished and the home provides a range of aids to assist residents safely as needed within rooms. There is a well maintained stair lift which was observed being used by residents which enables access for individuals to both floors within the home safely and independently. Communal areas were seen to be very clean and well maintained and there are call bells located for easy use by residents in their rooms. Testing and observation of the “call” system during inspection provided evidence of a rapid response to need by the care team. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 and 30 Competent and caring staff are organised and supported to meet the needs of residents. The manager ensures that staff training is provided to enable staff undertake their work appropriately. EVIDENCE: The inspector observed a good skill mix within the staff team on duty and was told by residents that their needs were met. One resident told the inspector that “I wouldn’t want to be anywhere else, the staff cannot do enough for me”. A relative visiting the home told the inspector, “ I am very pleased with the care they give, they have turned Mum around”. Staff were seen providing care in an unhurried manner and demonstrated personal knowledge of each residents needs. There was a sufficient staff to resident ratio observed during inspection. However, The manager and other senior staff undertake cooking duties at present as the position is currently vacant. The inspector observed lunches being cooked and served without loss of staff time. Staff rotas confirm that an extra member of staff is brought in when the manager or another senior is working in the kitchen and the inspector observed lunches being cooked and served without additional pressure being placed on the care team. The manager informed the inspector that the cook had recently left and that she is actively attempting to recruit to this post. staff files looked at confirm that the support for the training needs of staff, and discussion with two members of staff provided very positive comments about the support given by the manager. The manager is planning to provide update training on adult protection for all staff and handling of medication for senior staff.

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33 and 38 The home is well managed and run in the best interests of residents living in the home. The systems in place for fire safety are not satisfactory which is placing residents at risk. EVIDENCE: The home has a registered manager who takes a full working role within the home. This “hands on” approach gives the manager a good understanding of resident and staff needs. Staff are supervised by the manager and their views are sought about improvements to the services provided. The home has a range of policies and procedures for staff to follow which are kept with staff files in the managers office. Testing of fire equipment is up to date, however, a fire risk assessment recently undertaken by a fire safety inspector highlighted a need for smoke detectors to be fitted to two rooms and the basement within the home. This was discussed with the manager at the time of inspection who confirmed that the work is currently being arranged.

STATUTORY REQUIREMENTS This section sets out the actions which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. Standard OP19.5 Regulation 23(4)(i) Requirement Smoke detectors must be installed in the basement, bedroom 15 and bedroom 11 on the first floor. The detectors must be linked to the existing fire alarm system.
