Learning Disability (6) Mental Disorder (2) - Only associated with learning disability Date of last inspection 20th April 2005 Brief Description of the Service: 55 Cardinals Gate is a bungalow in a cul de sac in a residential area of Werrington, a village on the outskirts of Peterborough. It is no different in appearance from the neighbouring proerties. There is a small parking area and garden to the front of the bungalow and a spacious enclosed garden to the rear. There are six single bedrooms all with ensuite facilities. There is a spacious L shaped lounge dining room and an additional conservatory with patio doors to the garden. The home accomodates 6 young adults with a learning disability and is registered for two of the service users having associated mental health problems. The home has its own transport and the staff and service users also use local public transport. This is accessible from close by. This is an overview of what the inspector found during the inspection. The inspection commenced at 14.30 and the inspector was admitted to the home by a member of the care staff. There were 5 service users at home. The manager was not present and the staff member leading the shift took the lead in the inspection process. The inspector was shown a bedroom by the occupier but there was no in-depth inspection of the premises as this is a new home and the accommodation standards were all inspected at the first inspection. Service users were present for the entire inspection and were actively engaged in the inspection process offering opinions with regard to the standards. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2,4&5 Service users are able to express their needs and aspirations freely and had the opportunity to visit the home before moving in. EVIDENCE: Service users thoughts and aspirations are considered in all aspects of their lives at Cardinals Gate. These are explored as part of the assessment process and included in the care planning. All service users had the opportunity to visit the home and stay overnight before moving in. This is part of the homes’ admission policy. There is a copy of the statement of terms and conditions of the home on each individual file signed by the individual. The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,8,9&10 All service users have the opportunity to make choices in their lives and are supported by the staff when necessary. EVIDENCE: Each service user has a care plan that they have helped to compile and have agreed. They are able to make choices in all aspects of their lives and were able to tell the inspector this during the visit. There are monthly house meetings where everyone is free to put their opinion. Risk assessments are in place for current activities. Service users are aware of the need for confidentiality and also of respecting each other privacy. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11,12,13,14,15,16,17 The service needs to further develop appropriate leisure activities within the community. Personal and educational development is met. EVIDENCE: All service users have planned daytime activities including college courses. They participate in a variety of activities in term time but these were not available in the summer break. The home needs to consider planning more evening activities in the local community as well as those accessed within the learning disability services. Family relationships are supported although three service users present did not think they would be allowed to invite friends home. This was discussed with the inspector and care staff to reassure them that friends would be welcome and it was suggested they discuss this at the next house meeting. Service users help with the cooking, cleaning ,washing and ironing. They told the inspector of the holiday they are about to go on. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20,21 All personal and health care needs are appropriately met. EVIDENCE: Two service users need help with personal care and this is delivered appropriately. They are all registered with a local GP and have regular checks with dentists and opticians as appropriate. Outside professionals are consulted as needed. EVIDENCE: The service users present were able to talk about what they would do if they wanted to complain and what response they would expect. They all knew they could talk to the manager or speak out at a house meeting. JUDGEMENT – we looked at outcomes for the following standard(s): 34,35,36 The home is sufficiently staffed to meet the needs of the service users. EVIDENCE: There were noted to be four members of staff in the home during the inspection. Although no staff files were inspected two members of staff stated that as part of the recruitment process they had to provide proof of identity two references and undergo a Criminal Records Bureau (CRB) enhanced check, They have an initial induction followed by further training. All statutory training is up to date. Five staff are currently undertaking NVQ level2. Any staff shortfalls are made up by the use of agency staff and those used come regularly and service users know them. Staff have regular formal supervision and records are kept. JUDGEMENT – we looked at outcomes for the following standard(s): 37,38,43 The home is well managed. EVIDENCE: The manager is qualified to NVQ level4 and has the registered managers award. She is open and accessible to staff, service users and families. The manager does cover some care shifts and accompanies service users on social activities.
