The home was opened in 1988 and consists of a two -storey building with a newer purpose built extension. There are 16 single rooms and 8 double rooms; one single and one double room provide en-suite facilities. There are suitable adaptations and equipment including a passenger lift which ensures that the home is suitable for the client group accommodated. This is an overview of what the inspector found during the inspection. This inspection was conducted unannounced. The registered manager and registered provider were present during the inspection. A range of information was used to inform this report, The history of the home’s compliance with the National Minimum Standards, Pre-Inspection Questionnaires and the service history. The inspection reviewed the arrangements around the care that 3 people were receiving based on their individual care plan, this method is called case tracking. Those case tracked were spoken with and their rooms viewed. 5 members of staff were spoken with including one new member of staff. 2 visiting relatives were also spoken with. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3-6 Quality in this outcome area is excellent. This Judgement is made using available evidence including a visit to this service. The home has a sustained track record of delivering appropriate pre admission assessment and ensuring that the needs of the resident are assessed and planned for. Intermediate care is not provided. EVIDENCE: The home has a sustained track record of compliance with these standards. The most recent admission to the home was case tracked and records reflect continued compliance with the standards required. Significant time and effort is spent making each persons admission personal and well managed. Each prospective resident receives a comprehensive assessment from a suitably qualified person prior to admission. Any external professional assessment is also obtained. Prospective residents have information they need to inform them about the services the home provides. Wherever possible the home manager and senior staff are allocated to work on the day of the planned admission which enables a clear up to date assessment is undertaken at that time and that information can be effectively cascaded to staff. Residents from different ethnic groups are assessed for their cultural and religious needs along with methods of communication and advocacy. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7-8-9-10 Quality in this outcome area is excellent. This Judgement is made using available evidence including a visit to this service. The health and personal care which residents receive is based on their individual needs. The principles of respect, dignity and privacy are put into practice. The management of medicines safeguards the residents. EVIDENCE: The records of three residents were examined. Each resident has a robust care plan in place that reflects the persons individual needs based on a thorough assessment of their activities of daily living. The home ensures that the resident and their representative are involved in care planning and reviews of the care plan. Plans are updated monthly or as conditions change. The plans are a working tool and are used and understood by staff. They are written in clear language and could be used by people not familiar with there content.

Residents are assessed for their need for specialist equipment and staff keep a check on aids, making sure they are in good working order and improve the quality of life for the resident. There was good evidence that the home promotes the independence of residents, undertstanding their individual goals and expectations. Residents and relatives discussed how the staff promote their privacy and dignity, how they felt safe and well cared for. Relatives said they found the staff were skilled and in one case said they felt more confident about the delivery of good health care at the home than they had experienced within the NHS. Care plans are developed using the outcomes of any assessed risks to inform the care delivery for that person, which is good practice. Relatives said they had been able to spend as long as they wished at the home and were also able to have their meals there. Medicines are well managed within the appropriate legislative framework, which safeguards residents. The supplying pharmacist inspects the service in line with its contractual arrangements with the Primary Care Trust. The last audit of the homes medicine management was satisfactory with no recommendations made. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12-13-14-15 Quality in this outcome area is excellent. This Judgement is made using available evidence including a visit to this service. Residents are able to choose their lifestyle, social activity and keep in contact with their family and friends. Social, cultural and recreational activities meet the expectations of the residents. Residents receive a healthy and varied diet following continuous assessment of their dietary preferences and nutritional needs. EVIDENCE: Policies and procedures in place ensure that each resident has a social history completed within their care file. Each resident has a member of staff allocated to them as their social mentor, Staff spend individual time with them, this ensures that the resident receives a full and varied lifestyle that meets their individual tastes and preferences. Residents have recently been on a boat trip organised by the home. The staff encourage independence, residents are supported to visit local shops either escorted or if able, alone. There was some excellent work undertaken to establish a rehabilitation programme for one younger adult that is accommodated at the home.

One full time staff member is a social care co-ordinator. The home has also joined the National Association for Providers of activities for Older People, which ensures good practice guidance and access to training for staff. The home provides a healthy diet, written menus and choices of food. Residents said they loved the food provided. The manager monitors the satisfaction with the diet provided through quality assurance systems such as questionnaires, meetings and obtaining the likes and dislikes of the resident during care planning. Residents have access to a robust and effective complaints procedure and are protected from abuse. EVIDENCE: The home has a sustained track record in meeting these standards. The service has a complaints procedure in place which is available to all those that use the service. Concerns are recorded and show they are dealt with effectively and sensitively. Residents said they were very satisfied with service provided. The home ensures that through training, supervision, review and quality monitoring that care staff fully comply with the policies and procedures relating to safeguarding the rights of residents. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19-26 Quality in this outcome area is excellent. This Judgement is made using available evidence including a visit to this service. The physical design and layout of the home enables residents to live in a safe, well maintained environment which encourages independence. EVIDENCE: The home provides a safe and clean environment for residents. Residents case tracked had a range of equipment to support their independence. There is evidence that different cultural needs can be supported within the environment. An enclosed rear garden is accessible to people with mobility problems and provides a safe outside space. There is a selection of communal areas, which allows residents a choice of sitting quietly, meet with family and friends or engage with other residents. The manager is an infection control link nurse who attends the community link nurse meetings and ensures that the practice of infection control meets the National guidance. This Judgement is made using available evidence including a visit to this service. The staff at the home are trained, skilled and sufficient in numbers to meet the changing needs of the residents. EVIDENCE: The service has a well developed recruitment procedure that has the needs of the residents at its core. Management prioritise training and facilitate staff to continually update their knowledge and skills through external qualifications and internal updates on practice issues. The staff team have a period of induction at the start of their employment and ongoing training throughout their employment. Training reflects the needs of the residents. Staff have also undertaken training in Dementia care. Residents and relatives reported that staff were skilled in their role and able to meet the needs of the residents. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31-33-35-38 Quality in this outcome area is excellent. This Judgement is made using available evidence including a visit to this service. The management and administration of the home is based on openess and respect, has a quality assurance system in place and a qualified competent manager. EVIDENCE: The registered manager has the required qualification and experience, is highly competent to run the home and met its stated objectives. A registered nurse with many years of experience in the care of older people, she is a qualified NVQ assessor and continues to update her knowledge and skills. The home is owned and managed by members of the same family. There are excellent arrangements in place that ensure both the manager and the provider have clear roles and lines of accountability within the service.

There are good quality monitoring systems in place and significant time is spent ensuring the views of all those involved in the service are obtained and their comments acted upon to inform the service provided. Innovative ways of providing feedback to residents and relatives such as newsletters and cheese and wine evenings have been implemented. All records are clearly written and dated; any findings from risk assessments have been actioned. There is strong evidence that the ethos of the home is open and transparent with the views of the staff and residents listened to and acted upon. The registered persons are committed to ensuring the health and welfare of the residents and staff. Arrangements for the fire prevention in the home were reviewed and found to be fully compliant with the local Fire department following a recent inspection by them. During the inspection it was noted that the visiting hairdresser was using a hairdryer from an extension lead in the ground floor bathroom, the manager dealt with the risk this imposed at the time ensuring the practice did not continue.
