This inspection was carried out on 1st December 2009.

It is an annual review prepared by CQC after examining previous reports and information from the provider. At the time of this report, CQC judged the service to be Excellent.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

We give a quality rating following a full review of the service. We call this review a ‘key’ inspection We do an annual service review when there has been no key inspection of the service in the last 12 months. First Floor No one may be admitted to the first floor who falls within category PD (physical disability) unless they also fall within category DE (dementia). First Floor The first floor may accommodate no more than 30 service users who fall within the category/combined categories PD,DE. Ground Floor The ground floor may accommodate no more than 30 service users who fall within category OP (older persons) and PD(E) (older persons with a physical disability) of which 15 service users are accommodated in the Eaton Unit also located within the ground floor. Ground Floor The service provided to persons admitted to the home who fall within category PD must be in accordance with the Home Statement of Purpose as submitted to the CSCI on 12 August 2004. Ground Floor Within the Eaton Unit the home may accommodate up to 15 service users of which up to 8 service users in total who fall within category PD (physical disability) may also be accommodated. Ground Floor Within the Eaton Unit the home may accommodate up to 2 persons between the ages of 35 and 45 years who have a diagnosed degenerative neurological disorder and fall within category PD. No one in categories OP, PD(E) & DE(E) may be admitted to Castle Donnington Nursing & Residential Home where there are 60 persons in total of these categories/combined categories already accommodated in the home. If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service Castle Donington Nursing & Residential Care Home is purpose built on two floors, and can accommodate residents over 55 years with dementia, a physical disability or terminal illness, within a safe environment. There is a separate unit, within the home, for eight residents with a diagnosed neurological condition (35 years and above). Trained nurses and care assistants staff the home, with a trained nurse on duty at all times. All residents rooms have en-suite facilities and there are additional toilets and assisted bathing facilities. There are a number of pleasant, separate sitting rooms and dining areas throughout the building and a safe enclosed garden. All areas of the home and garden, accessible to the residents, are wheelchair friendly, and there is a passenger lift to the first floor. The home is situated in the pleasant village of Castle Donington, on the boarders of Leicestershire, Nottinghamshire and Derbyshire. Village amenities are close by with local buses to nearby towns of Coalville, Ashby-de-la- Zouch and Loughborough. There are car-parking facilities next to the home. We looked at all the information that we have received, or asked for, since the last key inspection or annual service review. This included: The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Surveys returned to us by people using the service and from other people with an interest in the service. Information we have about how the service has managed any complaints. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The previous key inspection and the results of any visits that we have made to the service in the last 12 months Relevant information from other organisations. What other people have told us about the service. What has this told us about the service? The home sent us their Annual Quality Assurance Assessment (AQAA) when we asked for it. It was clear and gave us all the information we asked for. We looked at the information in the AQAA and our judgment is that the home is still providing an excellent service and that they know what further improvements they may need to make. We sent surveys to people who use the service and to people who have an interest in the service. We received two from health professional who work closely with the home, four from staff and six from people who live there (some with the help of relatives). The health professional told us that, The staff at Castle Donnington are always polite, cheerful and respectful to clients, families and professionals. Their approach is client centered. They carefully assess their clients need and are pro-active in providing care. They meet the healthcare and social needs of a divers client group (in terms of age and level of need) effectively. A Registered Nurse told us that the home was well managed, that staff were professional, that care plans were always evaluated, and that liaison with all other services, professional and families was excellent. Annual Service Review Page 4 of 6 Staff told us that the home had a welcoming atmosphere and that they provided good quality care and meaningful activities for the people who live there. They said that management and team-work was good, and that staff were provided with good training. They said that their opinions were listened to and that there were regular meetings for staff, residents and relatives. People who live in the home told us that they were happy living in there. One person said that the, Care is good. My need are taken into account and concerns responded to, and that there was, Quick attention to medical needs and good communication with the local medical center. Another person said, Staff were always friendly and make you feel at home and keep you well informed. Another wrote, The home is wonderful and the management could not be better: there is a lovely homely feel to the place, very welcoming, and nothing is too much trouble. No-one who responded to the survey felt that the home could improve on the care they provided. CQC have received one complaint about the service which was investigated under safeguarding protocols, and was concluded with no further action required. The service continue to let us know about things that have happened since our last key inspection in 15th December 2008 when the service was rated a three star excellent service, and they have shown that they have managed issues well. They work well with us and have shown that their service continues to provide excellent outcomes for the people who use it. What are we going to do as a result of this annual service review? The next inspection of this service will be based on the Fees and Frequency Regulations 2007 and the assessment of risk of the service. Further clarity will become evident as the new registration and inspection system under the Health and Social Care Act 2008 is confirmed.
