Date of last inspection Brief Description of the Service: Cedar Court is a care home providing personal care and accommodation for fifty-two older people which includes older persons who have a physical disability. The home is owned by Southern Cross Care Group, operators of a number of care homes within Leicestershire. The home is located in Wigston Magna close to the town centre of Wigston where service users have access to shops, pubs, the post office and other amenities. The home is easily accessible by private or public transport. The home is a purpose built three-storey building with level entry access. Access to all floors can be gained by using the passenger lift or stairs. Residents have access to a number of amenities such as washing, bathing and toilet facilities situated on all floors. There is a choice of dining and lounge space. The premises have forty-eight single bedrooms with twenty-one having en-suite facilities. There are two double bedrooms without en-suite facilities. The home has a garden to the front and rear of the building which is well maintained and which is accessible to all residents residing on the premises. This inspection took four hours to complete. The outcome of the inspection was good based on positive comments made by four residents, indicating that they receive good quality care on a daily basis. In addition to speaking to four residents, five residents’ care records and other related documents were inspected. Two care staff members, the registered manager and the office manager were spoken with also as part of the inspection process. No Requirements or Recommendations were made. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2, 3, 5 and 6 The assessment and admission processes are good in promoting residents’ choice and identifying their care needs. EVIDENCE: Five residents’ assessments were inspected. The documents contained good information that identified their care needs. Two staff members gave good verbal responses about the care required by the residents. Five residents’ care records seen and conversation held with them indicated that they had the choice to visit the home prior to moving in. An inspection of the providers’ admission process indicated that prospective residents are given a contract which include the Terms and Conditions of their residency in the home. Discussion held with the registered manager indicated that the home does not provide intermediate (rehabilitation) care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10 The care plan and medication processes are effective in ensuring that residents receive good quality care. EVIDENCE: An inspection of five residents’ care plans showed that all aspects of their care needs are being attended to. Robust information is contained in the documents which gives staff members’ good instructions on how to meet these residents’ care needs. Four residents spoken with indicated that they are satisfied that their care needs are being met everyday. One resident stated ‘The carers are very good’. Observation of the medication process indicated that staff members appear to be adhering to safe medication practices. One resident’s medication record was inspected which was up to date. Five residents’ care records inspected and conversation held with four residents indicated that robust processes are in place so that residents can receive nursing care and when it is required, medical care from General Practitioners. Observation made indicated that care staff members attend to residents personal hygiene care in the privacy of their own bedrooms. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 Robust processes are in place in order to give residents the opportunity to exercise choice over their own lifestyle. EVIDENCE: Four residents spoken with indicated that they are able to go to and get out of bed at their own convenience. Discussion held with the registered manager indicated that mealtimes are flexible to suit residents’ choice regarding when to eat and drink. Verbal statements made by four residents confirmed the registered manager’s verbal statement. JUDGEMENT – we looked at outcomes for the following standard(s): 17 There is an adult protection process to protect residents’ legal rights. EVIDENCE: Five residents’ care records seen and conversation held with four residents indicated that systems are in place so that residents can participate in voting processes and consult with advocates when required. The registered manager indicated that in cases where residents are unable to go to polling stations to cast their votes, postal votes are utilised. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20, 21, 23 24, and 25 Residents’ surroundings are comfortable to meet their accommodation needs. EVIDENCE: Observations made during a tour of the premise indicated that there is sufficient shared space in the home based on the number of residents residing in the home for example the dining and lounge areas. Observations made indicated that there are sufficient numbers of toilets and washing facilities based on the number of residents residing in the home. Registration documents seen prior to this inspection indicated that bedroom and communal rooms meet the required measurements. Four residents spoken with indicated that they are satisfied with the size of their rooms. Observations made and conversation held with four residents indicated that there is sufficient water supply, heating, ventilation and lighting in the home. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28 and 30 Sufficient numbers of staff members are employed to work in the home for residents’ care and protection. The recruitment and training processes in place are good in ensuring that suitable staff members are recruited for employment in the home. EVIDENCE: The staffing rota seen and observations made indicated that there were suitable numbers and skill mix of staff members on duty at the time of the inspection. Four residents spoken with indicated that they are satisfied that a staff member is available when required. One staff member’s training record was inspected. The records seen indicated that the staff member has completed core training such as moving and handling and fire safety. Discussion held with the registered manager indicated that over fifty per cent of the staff team have achieved their National Vocational Qualifications (NVQ 2) in care duties. Training records seen confirmed the registered manager’s verbal statement. One staff member’s recruitment record seen indicated that all relevant documentation such as two suitable references and a current Criminal Records Bureau disclosure are contained in the file. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 37 and 38 The home is managed well and staff members sufficiently supervised for residents’ care and safety. EVIDENCE: An inspection of the registered manager’s records prior to the inspection indicated that the manager is suitably qualified and experienced to manage this home. Conversation held with four residents and one staff member indicated that the manager’s approach to managing the home create an open and inclusive atmosphere. All care records inspected are kept up to date and are stored in lockable spaces. The fire safety process was examined. All fire safety equipment for example fire extinguishers are serviced regularly and regular servicing of other equipment are carried out routinely.
