This inspection was carried out on 22nd November 2005.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Each care file has a detailed assessment of need and relatives are given the opportunity to participate. Relatives are encouraged to visit the home prior to arranging a move. Medication is recorded appropriately and administered by registered nurses. A choice of menu is offered and is available on the notice board. Complaints are documented and responded to within the agreed timescale. The manager has a number of staff trained to either NVQ 2 or 3 and a number of staff waiting to enrol. All staff files had appropriate documentation to ensure the safety of the service users. Service users monies are stored securely and clear accurate documentation kept.

What has improved since the last inspection?

The repairs highlighted at the previous inspection have been addressed providing a pleasant environment. Following the appointment of a training officer, training of staff has become more frequent

What the care home could do better:

The staff should have regular supervision with their line manager, at least 6 times per year. The manager should continue to pursue the changing of bathroom 4 to a shower room to provide a suitable bathing facility for the service users.

Option of admitting up to 10 persons aged 50 years and over within the total number registered. Date of last inspection 7th July 2005 Brief Description of the Service: Cedar Court care home has two units. The larger unit provides nursing and personal care for up to 50 persons aged 65 years and over with dementia, including up to 10 places for persons aged 50 years and over. Cedar Court is situated in a rural location near to Bretby village. Your Health Limited owns the home. The home is a two-story building; adapted for use as a care home. This unit has 34 single and 8 double bedrooms. 30 rooms have ensuite facilities. Access to the first floor is by stairs and a passenger lift. The unit has 3 lounges and dining areas on the ground and first floor, and has a large enclosed garden, which is accessible to service users. Although the ground floor is mixed sexes, there is a specific area on the first floor for male service users. The inspection was unannounced, in addition to addressing key standards not assessed at the previous inspection, the inspector was investigating an anonymous complaint made about the home. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,4,5 Assessments are completed prior to admission providing staff with information required to meet the needs of the service users. EVIDENCE: The inspector examined 4 care files, each file contained a detailed assessment of need and care plans developed from the assessments. The assessments of need covered all aspects of care. The relative spoken with stated that he and his family had visited the home prior to his wife moving in. He had been involved with the care plans and provided valuable information regarding her care. He was confident that following the visit and information gathering time, that the home were able to meet her needs. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9 Detailed information is documented and a variety of other professionals are involved as necessary, all ensuring that the health and social needs of service users are met. EVIDENCE: As stated previously the assessments of need in the files examined covered all aspects of care. Detailed care plans developed from the needs highlighted were easy to follow and reviewed on a regular basis. There was evidence of input from the tissue viability nurse, dermatologist and GP. There were continence assessments and nutritional assessments completed. Consent forms for the Flu vaccine had been signed by relatives. Registered nurses administer all medication. The home keeps a record of medication received, administered and returned. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 15 The meals in this home are reported to be good, offering both choice and variety for service users. EVIDENCE: The menu is documented on the notice board. Choices are offered. The relative spoken with stated that the quality of food was good and the staff did ensure dignity was preserved during meal times. The inspector did not examine the kitchen on this occasion. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 Complaints are documented and investigated appropriately ensuring that service users and relatives are listened to and concerns acted upon. EVIDENCE: The CSCI have received an anonymous complaint about the home, this was investigated during the inspection and was not upheld. The manager keeps a clear complaints record, documenting the complaint, the investigation and the resolution along with dates, ensuring that all complaints are resolved within the 28 day timescale. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) EVIDENCE: None of the standards within this section were assessed on this occasion. All key standards were assessed at the previous inspection in July 2005. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29 Staff training and stringent recruitment procedures ensure that the service users safety. EVIDENCE: The manager has 9 staff on duty throughout the day; 1 being a registered nurse and there is always a first aider on duty. There are 2 staff completing NVQ 4. There are a number of staff with NVQ 3 and a number with NVQ 2, this is an ongoing programme and staff who are not completing an NVQ are due to enrol. The inspector examined a cross section of staff files, all contained an application form, 2 written references and a Criminal Records Bureau check. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 35,38 Regular audits of the service ensure that service users are safe and care practices improved as necessary. EVIDENCE: Service users finances are kept securely and a clear record of monies deposited and withdrawn is kept along with receipts. Monthly safety audits are completed by the qualified Health & Safety person. Monthly maintenance checks are carried out. All staff have completed Fire training and Moving & Handling training. The fire alarms are checked weekly as is the emergency lighting.
