One named person may be over the age of 65 Sevice users shall be within category LD Date of last inspection 29th June 2005 Brief Description of the Service: Cherry and Rowan Villas is a care home providing personal care and accommodation for 13 younger adults who have and learning difficulties. The home provides 11 long term beds and 2 respite beds and has dedicated beds for this purpose. The home is owned by Oaklands (formally known as Warsop and District Mentally Handicapped Association), which is a registered charity. The home is located in a residential area of Warsop and is close to shops, pubs, the post office and other amenities. The home was opened in 1986 and consists of a 2 purpose built bungalows with a large community centre on the same grounds.11 of the homes bedrooms are single, and 3 of the bedrooms have en-suite facilities. The home has extensive gardens that are well maintained and easily accessible. This is an overview of what the inspector found during the inspection. This inspection was the second of two unannounced inspections carried out between April 2005 and March 2006. The inspection lasted for 1 ½ hours and the inspection focussed on key standards not inspected at the last visit.. The main method of inspection used was called case tracking which involved selecting 3 residents and tracking the care they receive through the checking of their records and discussing this with them. Only one resident was present at the inspection and care staff were either going off duty or were accompanying residents out of the home. . A discussion was had with the manager and deputy who were the only staff present. . No visitors came to the home during the period of inspection. The premises were not inspected in detail but various areas of the home were visited as part of the inspection. What the service does well:
Residents are able to move freely between the bungalows, community centre and grounds. Care practices are designed to respect residents rights. Residents are encouraged to help with domestic chores to the best of their ability. Regular holidays are organised and this year this has included trips abroad. A resident said he had enjoyed the holiday he had been on. Residents are able to have a meal not on the menu if they fancy an alternative and staff are able to assist in feeding. A resident said that his favourite meal was bacon, egg and tomatoes. Staff practices are designed to promote residents’ privacy and dignity and each resident has all their own toiletries. Residents go out to the shops with staff to buy their own things, including their clothes. Weekly training sessions are held and these are designed to meet the identified training needs of staff. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 17 Residents rights are respected and staff provide support in achieving these. Meals and mealtime arrangements are suitable to the needs of the residents EVIDENCE: Residents can move freely around the grounds and between the two bungalows. Each resident is risk assessed for leaving the grounds, road safety and using transport. Staff knock on residents’ doors before entering and where a resident is unable to answer wait for a minute before entering. All residents are given a key to their room, but only one chooses to use it. Staff will read out any mail that comes to the recipient resident. Residents will help with domestic chores according to their ability and understanding. Staff said that most enjoy helping wash pots, make beds and empty bins and some are able to use the vacuum cleaner. Any resident who chooses to smoke is able to do so outside s both bungalows are no smoking environments. There have been a number of successful holidays arranged recently including trips to Spain, Disneyland Paris and Center Parcs, which has provided some

A resident aid that he had enjoyed his holiday at Center Parcs and he had a beer. Residents prepare a menu for the following week on Fridays, but this is only used as a guide as residents frequently change their mind on the day. A resident said that he enjoyed bacon, egg and tomatoes which he said he often had. A record is made of each mal provided which showed that a varied, healthy and balanced diet is provided. Soft diets are available when required and staff will provide residents assistance with feeding where needed. The dietician has previously attended the home to provide advice on nutritional intake. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 Residents are not always able to express the support they require but staff work on providing support in line with what they see as best practice. EVIDENCE: The manager said that all staff are inducted on how to promote and maintain residents privacy and dignity when providing personal care as many can not communicate how they would like to be supported. Each resident has their own toiletries and one resident aid that he had shaved himself with an electric razor. A physiotherapist comes to the home on a monthly basis to provide support and guidance and is in the process of acquiring a new sling for the hoist. One resident with mobility problems has been provided with a walking frame but has been unable to get used to it. Residents who have daytime commitments are got up in the mornings so that they can meet these, but are able to have a lie in bed over the weekend. Residents go shopping for their own clothes and one resident said he was going out shopping for new clothes shortly. JUDGEMENT – we looked at outcomes for the following standard(s): 22 Systems are in place to hear the views of residents and to protect them from abuse. EVIDENCE: A poster in pictorial form is displayed in each bungalow depicting the complaints procedure. The home has a complaints book but no complaints have been recorded and the manager said that none have been made. There was a copy of the Adult Protection Procedures in the office and staff have been provided with training on these. The home has a whistleblowing policy. The manager said that there have not been any incidents of abuse and a resident said that all the staff are kind to him. JUDGEMENT – we looked at outcomes for the following standard(s): 32 and 34 Staff are provided with the raining to ensure they are qualified and competent in their role. The home’s recruitment practices do not include all the necessary safeguards to protect residents. EVIDENCE: The home provides a weekly training session and where training needs are identified arrangements are made for these to be met. Recent training has included dementia awareness and safe handling of medicines. A number of staff have completed their NVQ level 2 or 3 and more staff are working towards these. New staff are appointed through a recruitment procedure that requires them to attend an interview, provide 2 references and undergo a CRB check. Prospective staff are not required to complete an application form or provide details of their employment history as they should. EVIDENCE: The home has adopted the Quality Tree framework to provide feedback from residents on the services they receive. Weekly residents meetings are held. Relatives are actively involved in the overall running of the home as a number are members of the Board of Trustees. All mandatory training is up to date. The required electrical, gas and water safety checks and tests are undertaken and a new boiler was fitted in October 2005.
