This inspection was carried out on 13th December 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The manager and staff provide a high quality service to the people who use the service. The home provides a relaxed atmosphere for service users. Staff and service users had an open and supportive relationship, which was observed throughout the day. Care plans are detailed giving staff clear guidance on the level of assistance required for a range of care needs. The service ensures that personal and healthcare needs are met. The staff team are well trained and have achieved the target of 50% having a relevant care qualification. Other training includes adult protection, epilepsy and autism awareness. The manager is experienced, supportive and approachable.

What has improved since the last inspection?

The manager continues to ensure that service users are consulted and the service is reviewed and monitored.

A suitably qualified and experienced manager who is registered with the National Care Standards Commission must be employed in the home at all times. Staffing must be provided in accordance with the residential forum quidance by April 2004. 13th July 2005 Date of last inspection Brief Description of the Service: This home is registered to accommodate up to six adults who have a learning disability of both sexes. It does not offer permanent accommodation, but offers a respite service for over a hundred people in the Wyre area. The home is situated in Thornton and is close to local amenities such as shops and public transport. It used to be two semi-detached houses that have been converted into one building that has two lounges, a conservatory, kitchen and three bathrooms. All accommodation is single and four bedrooms are on the first floor with a further two situated on the ground floor. There are no en-suite facilities. Two bathrooms are upstairs and both provide a bath with shower. The ground floor has an assisted bath. There is a garden at the rear of the property that over looks a primary school playing field. The inspection was unannounced and there was one service user in the home. The inspection included case tracking a service user, talking to staff, observing staff interaction with the service user, looking at documentation and looking around the building. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6 and 7 The individual plans reflect the changing needs and personal goals of service users so staff have clear guidance when assisting them. EVIDENCE: Records were seen for the service user who was staying in the home. There was evidence to show that the home had consulted with the family prior to the visit to make sure that any changes since the last visit were documented. The care plan for this service user was up to date and reflected the person’s needs. The service user was offered choices about how to spend the day, and made decisions about having snacks. Staff on duty had good knowledge of the service user’s needs and good relationships were observed. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 and 19 The care plans give clear guidance for the best way to support people so staff assist in a manner that meets individual needs. EVIDENCE: Care documentation give staff detailed guidance on how best to assist people with personal care. The records for the service user who was accommodated provided information regarding seizures, night time support needs, bathing requirements and skin map to record any bruising or tissue breakdown. Details of input from other healthcare professionals while a service user is staying in the home are kept. The outcome of appointments are recorded in daily notes and this information is given to relatives at the end of the visit. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24,25,26,27,28,29 and 30 The respite service continues to provide a homely environment for service users during their stay. EVIDENCE: There have been no changes to the environment since the last inspection. The home has six single bedrooms, two of which are on the ground floor while the others are on the first floor. All bedrooms have suitable storage, and new televisions and CD players have recently been purchased for all bedrooms. There are no en-suite facilities provided in the home, however there are three bathrooms available for service users. The maintenance plan has identified areas for improvement such as the need to make the garden more accessible and replacement of some doors and windows. This work is being attended to in the New year. There are two lounges and a conservatory. The home was clean and bright and there are suitable infection control policies in place. Lancashire County Council provides suitable personal protective equipment such as gloves and aprons. The home has arranged for clinical waste to be disposed of appropriately. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 33 and 35 Staffing levels are sufficient to meet the needs of the service users. Service users are assisted by people who are aware of good practice as training continues to be provided to staff. EVIDENCE: This inspection was unannounced and there was one service user at home. There were two members of staff on duty as well as the registered manager. The staff team have achieved the target of 50 having National Vocational Qualification (NVQ) level II in care. Other training that is provided includes adult protection, epilepsy, autism awareness, risk management and health and safety. Recruitment and selection practices were not assessed on this occasion. EVIDENCE: The registered manager has managed the home for approximately four years. She has over ten years experience of working with adults who have a learning disability in a residential setting. She has almost completed the Registered Managers Award.
