Date of last inspection Brief Description of the Service: Coalville Nursing and Residential Home, first registered in 1995 and part of Rushcliffe Care Limited, is a purpose built home registered for 40 residents over 65 years of age and may also provide for a limited number of older residents with physical disability, sensory impairment and/or those who require terminal care. The home is managed and staffed by Registered Nurses, care staff and ancillary staff. The home may now also admit up to and including twenty residents within the category of dementia DE(E). The home is bright and clean with a high standard of décor throughout, with two spacious lounge/dining rooms, and adequate assisted bathing and toilet facilities. All rooms have en-suite facilities. It is situated on two floors with a 13-person passenger lift servicing the first floor. There is a safe, well-planted and attractive garden and all areas of the home are accessible to residents who use wheelchairs. Food is prepared on site, with a choice of menu, and special dietary requirements are catered for. Situated in North West Leicestershire it is within easy reach of Coalville town centre and the towns of Ashby de la Zouch and Loughborough. The local bus route is within a few minutes walk of the home and there is ample car parking for visitors. This is an overview of what the inspector found during the inspection. This inspection took place over one day. An opportunity was taken to look around the home, view records, policies and care plans and to talk to staff, residents and their relatives and visitors. The primary method of inspection used was ‘case tracking’ which involved selecting three residents and tracking the care they receive through a review of their records, discussion with their relatives, the care staff and visitors to the home, and observation of care practices. Many of the residents were seen during the inspection. Three residents, three of the residents’ relatives and staff spoken with, gave the Inspector their impressions of the home. What the service does well:
Coalville Nursing & Residential Home provides safe and supportive care for residents within the nominated categories. The home has many adaptations to assist with care and to help residents achieve their optimum independence, and exercise personal choice within their capacity to do so. The home is well managed by the Registered Manager. Care planning and risk assessment are detailed to ensure that the residents’ health and welfare needs are met. Staff relationships with residents and their relatives are very good demonstrating care, patience and understanding of each resident’s individual care needs. The standard of care is high, and the home is clean and well maintained throughout. Three visitors said that their relatives were very well cared for; that the standard of care and cleanliness in the home was high; that the food was good and they were welcomed and supported by the staff. One relative described the care and staff as ‘fantastic’. The home provides a high standard of care for the residents for which the Registered Manager and staff are to be commended. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 & 6. The admission process is well managed and residents receive a satisfactory assessment, thereby ensuring that their health and welfare needs are being met. The Registered Manager said that these had been archived as the residents had been admitted sometime ago, in 2003. A third, ‘new’ resident had a pre-assessment form, completed by the Registered Manager, which was reflective of the health, welfare and social needs of prospective resident. The home has one funded ‘assessment bed’. Following hospitalisation a resident can be assessed to establish if they require continuing nursing/residential care or may, at some future time, return to their own home. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 & 10. Quality of this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Residents are well looked after in respect of their health and personal care. EVIDENCE: Individual care plans were detailed and comprehensive to ensure that the residents’ needs were fully met. Care plans had not yet been signed (as recommended at the previous inspection in January 2006) by the residents or their relatives/representatives to demonstrate that residents or their representatives were involved in the care planning process. Nevertheless, visitors said that they were consulted, involved and kept fully informed of their relative’s healthcare needs. Two relatives commented specifically about the good communication between the staff and themselves. Only trained nurses administer the medication: mandatory training is provided and internal audits are completed to ensure the safe and timely delivery of residents’ prescribed medicines. The home also provides ‘homely remedies’, such as paracetamol and cough linctus, within specific guidelines and in agreement with the resident and their GP. Observation during the inspection showed that staff have a good awareness of how to protect residents privacy and dignity; staff were seen to be kind and patient and treated the residents in a respectful, friendly and supportive manner. Residents and their relatives said that the staff demonstrated respect and maintained the residents dignity at all times. Staff were seen to be very discreet when speaking to residents about their personal care needs. Three residents and three relatives spoken with said that staff were very caring. One relative said that the staff were ‘fantastic’ and that she was made to feel ‘part of the Coalville family’. She said that Linda (the Manager) was ‘caring and supportive’ and a resident who had lived in the home for two years said that she was ‘very happy there’, was ‘well cared for day and night’ and ‘could not praise the staff enough’. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 & 15. Quality of this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The staff work hard to ensure that residents experience a safe, homely life style. EVIDENCE: The home has a pleasant, friendly atmosphere. Many visitors came and went throughout the day, including children, some staying for long periods. Relatives said that they could visit at any time and staff always made them feel welcome. The home has a routine, which includes specific meal times and activity timetables. Residents were able to choose how and where to spend their day. Coalville Nursing & Residential Home DS0000001895.V286180.R01.S.doc Version 5.1 Page 13 There is an Activities Organiser who arranges a weekly programme of activities, suited to the residents’ particular needs and abilities. During the site visit residents enjoyed ‘song of praise’ organised by people who regularly visit to provide this service. Most of the residents participated and appeared to be enjoying themselves. There are celebrations throughout the year at Christmas, Valentines Day etc; this enables residents and their relatives to enjoy seasonal and community activities. Birthdays and special days are celebrated. A programme of forthcoming activities was posted on the home’s notice board and there was an Easter raffle and a coffee afternoon and Hot Cross Bun Day organised for 14th April to ensure that residents have access to variety of activities. There were no residents from ethnic minority groups requiring specific cultural or religious support but the religious and cultural needs of the residents are supported as required and the home organise monthly church services. Residents and their relatives praised the quality of the meals provided and confirmed that a choice is always available, as are cooked breakfasts, snacks and drinks. Staff were observed assisting residents with their meals in an appropriate and sensitive manner. Special requirements are catered for such as diabetic, soft diets and prescribed nutritional supplements supplied. Residents are weighed regularly and professional nutritional advice sought where required ensuring that residents remain well nourished. Quality of this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Arrangements for receiving and responding to complaints are sound resulting in satisfactory protection of residents’ rights. EVIDENCE: The home’s complaints process reflects the government’s adult protection guidelines, set out in the local Multi Agency Policy & Procedure For The Protection of Vulnerable Adults from Abuse, No Secrets’ publication. Staff are aware of these procedures and receive relevant training. Complaints and concerns made to the home are dealt with appropriately. This judgement has been made using available evidence including a visit to this service. A comfortable, clean and safe standard of accommodation is provided for the residents. Coalville Nursing & Residential Home DS0000001895.V286180.R01.S.doc Version 5.1 Page 16 EVIDENCE: The home is safe and well maintained with many adaptations to suit residents’ specific needs. It is decorated and furnished to a high standard, which creates a comfortable and homely environment, and there is a system of maintenance and refurbishment. Residents are able to bring items of their own furniture and possessions with them to personalise their rooms. Resident’ rooms were clean and well decorated. Many rooms were personalised with photographs, posters and ornaments etc. All rooms have en-suite facilities and there are additional toilet, bathing and assisted bathing facilities. On the day of the site visit all areas of the home were clean and maintained to a high standard, which was commented upon by all of the relatives spoken with. Two residents and three relatives commented that the home was always clean, smelled fresh and that residents’ rooms were cleaned daily, and commented positively regarding the recent refurbishments and new carpets in the sitting rooms. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 & 30. Quality of this outcome area is good. This judgement has been made using available evidence including a visit to this service. At the time of inspection staff at the home were sufficient in numbers to meet the residents’ needs. EVIDENCE: Staffing levels, at the point of inspection were satisfactory and in accordance with that indicated by the Department of Health Residential Forum Guidelines, and were sufficient to meet the residents’ needs. The home employs a mixed staff group consisting of Registered Nurses and care staff, supported by domestic, catering and maintenance personnel. There are always qualified nurses on duty, day and night, to ensure that the residents’ nursing needs are met. The Registered Manager is supernumerary. Staff have a positive, proactive and committed attitude towards the care of the residents. Relatives said that the staff were capable and caring and worked very hard, and residents, relatives and staff said that they felt that there are sufficient staff on duty to meet the needs of the residents at the present time. All new staff have an induction programme to complete. Care staff attend an additional three day training course at Epinal Way Care Centre and annual

Rushcliffe Care Limited, Epinal Way Care Centre undertakes the recruitment process centrally and this is inspected when inspecting the care homes at the Epinal Way Care Centre. This judgement has been made using available evidence including a visit to this service. The Registered Manager is a trained nurse, and is knowledgeable, experienced and professional, and effectively manages the home to protect the rights and needs of the residents. The Registered Manager is a Registered Nurse and has many years of experience and is well supported by a professional senior management staff team. She has completed a student nurse mentorship scheme, is an NVQ assessor and Internal Verifier and is currently undertaking the Registered Managers’ Award. There is an ethos of openness in the home; staff are friendly, helpful and very caring towards the residents and their relatives. Two relatives said that the Registered Manager was approachable, very supportive and kind. Residents’ personal allowances are appropriately managed and records were in order. Staff receive formal recorded supervision by the Registered Manager to ensure that staff delivery safe and appropriate care for the residents although the Registered Manager stated that she a little behind with the process. Health & Safety Policy and Procedures, such as regular recorded fire drills and fire alarm tests are completed. Both the Registered Manager and a Senior Manager complete various internal audits to ensure that standards are maintained at all levels and to assure the health and safety of the residents and staff. Although on the day of inspection it was noted that hot water temperatures were close to 43 degrees Celsius, they had not been checked since January 2006 (records not available) and the last record available was September 2005. Company policy is that the hot water temperature is checked monthly to safeguard residents from the risk of scalds.
