Date of last inspection 1st September 2005 Brief Description of the Service: Chapel Cottage Care Home is registered to provide care for nine younger adults who have learning disability or mental health disorder. The premise is located three miles from the city of Leicester and is close to the town centre of Oadby where residents have access to a variety of shops and other amenities. The home is easily accesible by public and private transport. The premise consists of two floors for residents use which is accessible by use of the stairs. The home has five single bedrooms one with en-suite facility and two double bedrooms one with en-suite facility. This is an overview of what the inspector found during the inspection. This inspection took two hours and thirty minutes to complete. The outcome of the inspection was very positive in that three residents spoken with made positive verbal statements about the care they receive on a daily basis. The area manager, acting manager and one staff member were also spoken with as part of the inspection process. In addition, one resident’s care record and one staff member’s record was inspected. Observations made indicated that the home is maintained to very good standards. No Requirements or Recommendations were made during this inspection. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 4 and 5 The admission and assessment processes are very good for the care and protection of residents. EVIDENCE: The Statement of Purpose was inspected. The document clearly outlines the service provision in the home. One resident’s care records seen showed that this document is given to prospective residents or their relatives. One resident’s assessment document was inspected. The document seen indicated that the resident received a needs-led assessment and introductory visit to the home prior to moving in on a permanent basis. The document clearly outlines the care needs of the resident. Contained in the resident’s record are the home’s Terms and Conditions of residency including a contract, which is signed by this resident. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6 and 7 Residents care plans are robust so that they can receive good quality care and have a choice on how the care is delivered. EVIDENCE: One resident’s care plan was inspected. The document seen is written in great detail giving staff members instructions on how to meet the resident’s care needs. Observations made indicated that staff members’ interact professionally with residents to ensure that residents communicate their choices about the care they need. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11, 14 and 17 Residents have regular contact with family and friends in order to maintain contact with the community. EVIDENCE: Conversation held with two residents and the visitors’ records seen indicated that residents maintain contact with their relatives and friends. Evidence of this was seen also in one resident’s care records. Observations made at the time of the inspection showed that the residents were going to the theatre that evening to see a production. Conversation held with three residents indicated that they were happy about going to see a performance. Observations made of the evening meal process indicated that meal times are not rushed. The meal served appeared nutritious and wholesome. Conversation held with three residents indicated that they enjoyed their meal. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20 The medication process is robust in ensuring that residents receive their medication at times prescribed by their doctors. EVIDENCE: The medication process was inspected. One medication record seen indicated that staff members appear to be administering medication in accordance with doctors’ prescriptions. JUDGEMENT – we looked at outcomes for the following standard(s): 23 There is an adult protection procedure in place to respond to suspicion or allegation of abuse for the care and protection of residents. EVIDENCE: Observations made and conversation held with one staff member indicated that the provider appear to be adhering to safe practices that promote adult protection. For example providing residents’ care in accordance with their written care plans. Two residents spoken with indicated that they feel safe residing in the home. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 25, 26, 27 and 29 Residents’ surroundings are comfortable to meet their accommodation needs. EVIDENCE: One resident’s bedroom seen indicated that it is maintained in good condition. Conversation held with the resident indicated that she has the choice as part of her continued development to clean and maintain her bedroom to suit her lifestyle. Observations made during the inspection indicated that there are sufficient numbers of toilet and washing facilities based on the number of residents registered to reside in the home. Observations made indicated that there are sufficient fittings, fixtures and equipment in the home to meet residents’ care needs for example lighting fixtures in bedrooms and communal areas. JUDGEMENT – we looked at outcomes for the following standard(s): 33 and 36 Good staffing hours is provided for the care and protection of residents residing in the home. EVIDENCE: Observations made indicated that at the time of the inspection there were sufficient staff members on duty. Conversation held with one staff member and one resident indicated that a staff member is always available when needed. One staffing record seen indicated that suitable supervision is given to this staff member on a regular basis. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 38, 39, 40, 41 and 43 The management approach to managing this home creates an open and inclusive atmosphere for residents’ comfort and care. EVIDENCE: The acting manager’s record seen indicated that she is suitably experienced and qualified to manage this care home. Discussions held with the acting manager and observations made indicated that she is knowledgeable and competent in her role. All policies seen pertaining to the Standards inspected on this occasion, contains detailed information on health and safety issues and gives staff members guidance concerning good practice whilst working in the home. All documents and care records inspected were up to date and kept secured in lockable spaces. The maintenance and fire safety records seen indicated that all equipment including fire safety equipment are tested and have scheduled maintenance to keep them in good working order.
