Conditions of registration: Date of last inspection 14th March 2005 Brief Description of the Service: Conifers is a bungalow situated on the edge of Bocking village. This home accommodates 6 service users who are between the ages of 18 and 65 years of age who have learning and physical disabilities. The home has large well maintained grounds and ample visitor parking. There is a local shop and local transport, the centre of Braintree being only a short distance away. This is an overview of what the inspector found during the inspection. This was a routine unannounced inspection, which took place on 28th of September over 4 hours. The inspection included a tour of the house and grounds, reviewing of care files, looking at policies and procedures and health and safety information. The inspector met 5 service users, the manager/proprietor and 2 staff members. Overall the standard of care offered in this home is of a high standard. There are currently 5 service users resident who all have a regular programme of activities inside and outside the home. One service user said that they “liked the home especially the garden”. Another service user said that they “liked their room”. Two of the service users have profound communication difficulties. The care staff showed great attention to the needs of the service users and demonstrated a good rapport. Care staff prepared all the meals within the home. A varied menu is offered which is devised with input from the residents. Friends and family are encouraged to keep in contact with the service users. Individuals are encouraged to have their rooms decorated to their choice of colours and to have personal objects for example furniture and fittings, TV, video that they choose and can use for themselves. Care planning is now comprehensive and reviews include all relevant parties. Policies and procedures include a policy on intimate care. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,9 There is a consistent care planning system in place and the health and social needs of individuals are set out in their plans. Service users are encouraged to make decisions about their lives. Risks are seen to be part of decision making and planning for individuals. EVIDENCE: Each person has a care plan that is regularly reviewed. Health and social needs are record as well as a plan for meeting the needs identified. Risks are identified for individuals and are reviewed regularly. Service users are included in all aspects of care planning as are families. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15,16,17 Staff have a good understanding of the needs of the group of residents. Personal support is offered to promote dignity and independence. A range of opportunities for social, leisure and educational pursuits are offered. A balanced diet is offered. EVIDENCE: On the day of inspection, individuals were engaged in individual programmes of activity which included, horse riding, panting, gardening, listening to the TV and going out to a local college. The risk assessments for activities were up to date. In discussion the manager was aware of the balance between allowing participation and choice and keeping someone safe. There is a varied and balanced diet offered which is based on a 4-week menu that is planned with the service users. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20 There are clear policies and procedures with regard to medication. Medication is distributed in line with policies and in a way that safeguards service users. EVIDENCE: Staff distributes all medication. All staff have been trained to do this. The medication is secure locked away in the office. There are signed medical consent forms for all service users. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24,25,26,27,28,30 The standard of the environment within the home is good providing the residents with a homely place to live. There is a good standard of maintenance. EVIDENCE: Fixtures and fitting are fit for purpose and the furnishings and fittings are of a homely design. Residents have their own rooms that are individually decorated and people are encouraged to have their own possessions in their rooms. Two service users showed the inspector their rooms and discussed how they had been involved in the redecoration and chosen colours and furniture. Bathrooms and toilets afford privacy. During the inspection a plumber was fitting a new sink and bath in the bathroom. All of the premises are accessible to all the residents and there are suitable safety arrangements in place. JUDGEMENT – we looked at outcomes for the following standard(s): 34,35,36 Recruitment practices are consistent and appropriate checks are carried out.Staff are competent and individual needs are understood and met as are those of the group. EVIDENCE: Staff files reviewed showed that criminal records checks are carried out and that copies of all references, identifying documents and photographs have been collected. Induction and core training is undertaken and individual staff members sign to demonstrate this. Supervision is given every 2 months and this is recorded and available for inspection. JUDGEMENT – we looked at outcomes for the following standard(s): 37,39,42 The manager/proprietor has a clear understanding of the service which is offered by this home and provides clear leadership spending time working alongside staff and service users. Service users views are sought on the service offered but there is no clear development plan. Health and safety is promoted. EVIDENCE: The manager/proprietor is involved in the day-to-day work within the home. There is a clear quality questionnaire which seeks the views of the individual service users and stakeholders. The manager does have a clear vision for the coming year and beyond of the developments that will take place but there is no written development plan or a clear link between the resident and stakeholder feedback and his plan.
