Conditions of registration: None Date of last inspection 23rd November 2004 Brief Description of the Service: Dean Lodge is a purpose built home that provides a short break service for 7 people with learning difficulties. The home is a bungalow situated in a residential area close to the town of Ferryhill. At any one time the home can take up to 3 people who have multiple disabilities; the home has aids and equipment for this purpose. All service users are provided with a single bedroom (all of which exceed the National Minimum Standard) and have free access to lounge, conservatory, and dining room. There is a domestic style kitchen that service users can use with supervision. All accommodation is on ground floor level with good access. The home has easily accessible rear gardens that are used by service users in clement weather. There is parking for visitors to the front of the home. This is an overview of what the inspector found during the inspection. This unannounced in spection took place on 26/7/05 over a period od three hours. During that time discussion took place with the manager and staff in the home. Service users in the home on the day of the inspection where unable to communicate verbally but where observed to be happy, relaxed and interacting with staff. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 3, 4 and 5 The homes Statement of Purpose and Service User Guide are excellent providing service users and their relatives with details of the services the home provides enabling them to make an informed decision about admission to be made. The admission process to the home for prospective service users is clear, involving and informative. EVIDENCE: The home has in place an up to date Statement of Purpose and Service User Guide. These documents are produced in an appropriate format. The information included in these documents enables service users and their relatives to make an informed decision as to whether they wish to stay in the home. An attached service satisfaction questionnaire is included in the Service User Guide. Service users and their relatives are encouraged to complete this questionnaire when they leave the home. Care plans examined contained a pre – admission assessment. The manager said service users and their relatives are involved in the assessment process. The manager said the home only admits service users for whom they have the capacity to meet their needs for short breaks. All service users admitted to the home are given a copy of the Terms and Conditions of residency. Evidence was available to show that relatives had signed on behalf of service users as to their agreement with the terms of residency. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7, 8, 9 and 10 There is a clear and consistant care planning process in place to provide staff with the information they need to satisfactorily meet service users needs. EVIDENCE: Care plans examined during the inspection where generated from a pre – admission assessment. Care plans were up to date and included risk assessments to enable service users to lead an independent lifestyle supported by staff in the home and out in the community. On discharge service users relatives are provided with a discharge summary to inform them if the service users needs had changed in any way from admission. Service users who visit the home for short breaks on a regular basis are asked to complete a pre – admission questionnaire. This form provides the home with information and make the manager aware of any change in the individual service user’s needs where the service may have to be adapted to meet their needs. Where verbal communication was difficult service users are encouraged and supported by staff to make their needs known and to lead an independent life style, assisting with small domestic tasks in the home if they so wish and out in the community. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11, 12, 13, 14, 15, 16 and 17 The range of activities and links with the community are good and support and enrich service users’ social opportunities. Staff support service users in such a ways to promote a fulfilling lifestyle. Meals in this home offer both choice and variety as well as catering for special dietary needs. EVIDENCE: Staff spoken to said they encourage and support service users to get involved in the daily routine in the home but it is their choice if they wish to do so. While staying in the home for a short break, service users are offered the opportunity to participate in a varied range of activties in and outside of the home. The home has transport available giving service users the opportunity to visit places of interest and social venues in the wider community. The manager said while staying in the home service users are encouraged to maintain contact with their relatives by telephone. Relatives are welcome to visit during the time the service user is staying in the home.

Service users where observed to be moving freely around the home and interacting with staff. Service users in the home during the inspection where unable to communicate verbally, but where involved in activities with staff and were smiling and relaxed. The manager said service users are encouraged to participate in meal preparation if they so wish. Some service users like to set and clear tables at meal time. Meal times are flexible and service users can always get a snack if they so wish. Good pre – admission information ensures that service users dietary needs are met including special diets. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 19, 20 and 21 The staff have a good understanding of the service users support needs and offer personal care in such a way to promote privacy and dignity. It is evident that positive relationships have been formed between staff and service users. The medication in this home is well managed promoting good health. EVIDENCE: Staff spoken to during the inspection where able to demonstrate that they repected privacy and dignity and encouraged and enabled service users to lead an independent lifestyle of their choice. Some service users who come into the home for a short break are not from the local area. Arrangements are in place with a local GP and health authority walk in service to meet service users health needs if required. Service users who have been receiving continuing treatment from other health care professionals continue to do so while they are staying in the home. Due to service users lacking capacity the manager said all service users admitted to the home are administered medication by staff. A policy and procedure for the safe handling of medication was in place. A policy and procedure is in place in the event of serious illness or death.
