The home is also known as Willowtree House and is one of four registered homes within easy walking distance of each other, supported by a Head Office and Day Centre in separate premises. Further support services are provided within the organisation for those moving on into a more independent lifestyle. The collective facilities are known as the Delos Community where residents are known as members. The home provides personal care and support for up to nine members whose primary care need is due to having a learning disability, but who may also have mental health problems. The environment is that of a family house. The range of fees at the home is currently £637-£735 per week. Details about the home in the form of the Statement of Purpose and Service Users Guide, some of which are in user-friendly format, can be obtained from the Registered Manager. This is an overview of what the inspector found during the inspection. The focus of inspections undertaken by the Commission for Social Care Inspection (CSCI) is upon outcomes for Service Users and their views of the service provided. This process considers the home’s capacity to meet regulatory requirements, minimum standards of practice; and focuses on aspects of service provision that need further development. The primary method of inspection used was ‘case tracking’ which involved selecting 3 service users and tracking the care they receive through meeting with the service users, a review of their records, discussions with the care staff and observation of care practices. The Inspector visited during the early morning period to observe practices by staff and to meet with members. Service users are referred to as members. Members have Learning disabilities, and may have additional mental health needs, but most current members were able to communicate effectively with the Inspector. The Inspector also undertook observations of care practice, and members’ relationships with staff to establish their levels of contentment with their lifestyles and routines. The Inspector spent over six and a half hours at the home. Some records are stored at the organization’s central offices, but it was not convenient to visit these offices on this occasion as staff training was taking place. The Inspector has visited central offices on previous occasions during the year and found that the organization functions professionally, and with due regard to the wellbeing of Members and the upkeep and audit of appropriate systems and records. Since the last inspection five comment cards were received from members, three from relatives and two from health care professionals. Comment cards consistently reflected generally positive views of the home. What the service does well:
The atmosphere of the home is calm and nurturing, and conducive to members’ happiness and wellbeing. Members are complimentary about the care received. The communal rooms are decorated and furnished in a homely manner and give service users a variety of places in which to spend their time. A pleasant garden area is provided with a patio area where members have barbeques and meals outdoors in summer. Two part-time support workers are employed to encourage social activities and involve individual service users in the wider community. The staff at the home are well supported by the organization (The Registered Owners, Delos Community), which shows a commitment to professionalism, development and good outcomes for members. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Members’ admission to the home is supported through thorough and flexible processes that ensure that members will benefit from the placement and that the home can meet their needs. EVIDENCE: There have been no recent admissions to the home but procedures were discussed with the Registered Manager. These evidenced that a professional and sensitive process are followed regarding preadmission assessment and admission, which leads to good outcomes for members. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,9 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Members are enabled to enjoy a full and stimulating lifestyle with a variety of choices available to them. They are enabled to take reasonable risk in pursuit of their independence. EVIDENCE: Members assessed and changing needs are known and understood by staff whose response is timely and appropriate. Members spoke positively about the care that they received and there was evidence through discussion and records that they are appropriately involved in their care plans. Members’ decisions about their lifestyles are discussed and risk assessed appropriately. The Registered Manager was advised of some shortfalls in the recording of the care plan and risk assessment reviews. Dates of reviews and any necessary update should be recorded. There was no evidence that outcomes for members had been compromised by this shortfall on this occasion.

Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15,16,17 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Members are very happy with their various lifestyles. Activities and daily routines are planned around their wishes and interests. EVIDENCE: Two part-time support workers are employed to encourage social activities and involve individual service users in the wider community. The inspector discussed the duties of one of the support workers with the staff member concerned and learnt that she enables service users to access the town or local parks in summer, on a regular basis as this is what service users choose to do. Records of service users’ preferred hobbies and interests were seen in the files sampled. Several of the residents attend the day centre run by the company. Service users spoke of their general satisfaction with their lifestyles. The home has vehicles available as transport for some activities, and members are enabled to go on holidays of their choice. Staff felt that service users activities in the home and in the local community met with service users wishes. As the homes population ages some prefer to rest quietly at home and this choice is fully respected. Service users are involved in the planning of the menu and stated that they enjoy the food at the home. Healthy eating is promoted, and properly balanced against members choices and wishes to eat out or have take away meals on occasions. Staff promote main mealtimes as a social occasion and eat with the members. Members were observed to be helping themselves to breakfasts of their choice at the time of the inspection. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20,21 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Personal and healthcare support is good, and members’ physical and emotional needs are well met. EVIDENCE: The Inspector observed that members’ personal and healthcare needs were being met at the time of the inspection with proper regard by staff for members’ privacy, comfort and dignity. Staff were observed to be sensitive to members choices, spoken and unspoken, and to their feelings. There are occasional shortfalls in the overview of consistent hygiene standards, for example teeth cleaning. Members are sometimes reluctant to respond to staff prompts where the care needed, as detailed in the care plan, is for staff to prompt or remind members regarding self-care. This was discussed with staff and with the Registered Manager who have agreed to review how this area can be overviewed and improved. Records demonstrated a good response to members needs. Community healthcare teams are appropriately accessed with or on behalf of members.
Delos Community Ltd, 7 Poplar Street DS0000039660.V323989.R01.S.doc Version 5.2 Page 14 Advice was given to the Registered Manager on the need for a District Nurse to assess the suitability of the bedsides in use for one member, and the need to consider an electronic hoist for the comfort of one member. Medication is well managed and members reported that they always got their medications on time. Records of this area are properly monitored by senior staff at the home. The home is able to deal with the areas of aging, illness and death and management and staff have has shown an ability to respond promptly, sensitively and effectively to these issues. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22,23 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Members are protected by the quality of professionalism and the observation and listening skills of staff. EVIDENCE: Members said that they could talk to the Manager about any problems and showed confidence that they would be listened to and taken seriously. There have been no recent complaints made to or about the home. It is the inspector’s judgement that the ethos of empowerment and the good listening skills of staff pre-empt any serious complaints. However the Registered Manager was advised of the need for staff to familiarize themselves with the formal complaints procedure. Staff have had training and have a good understanding of safeguarding vulnerable adults and “whistle blowing”. There is a prompt response to issues arising. JUDGEMENT – we looked at outcomes for the following standard(s): 24,30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The members’ house is homely, clean, comfortable and well maintained, suiting their needs and lifestyles. EVIDENCE: The communal rooms are decorated and furnished in a homely manner and give service users a variety of places in which to spend their time. A pleasant garden area is provided with a patio area where members have barbeques and meals outdoors in summer. The home was seen to be clean comfortable and well maintained throughout. JUDGEMENT – we looked at outcomes for the following standard(s): 33,34,35 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Members are well supported by suitably trained and experienced staff. EVIDENCE: The Inspector did not review staff records at this inspection due to these being held at the central office which was being used for staff training on the day of the inspection. The evidence below is therefore based on observations and discussions. The Inspector met privately with both day and night staff on duty. Staff were seen to be competent and appropriately trained, and said that they felt well supported by the Registered Manager and by the organization in their duties. Staff showed sensitivity and understanding of members’ needs and an attitude of professionalism in their duties. Observations of their working practices showed that they had warmth and proper respect for members. A visiting relative who had been involved at the home for several years confirmed and supported these views of staff competence. The Organization has an ongoing programme of training for staff that enables them to follow up their own training interests as well as undertake coretraining courses. The Registered Manager was advised of the shortfall in training in respect of Standard 42.2, there not being a current first aider on every shift. The difficulties of ensuring this when he makes up the rota were also discussed as staff training records are held centrally. This judgement has been made using available evidence including a visit to this service. Management of the home is effective and responsive and members thereby benefit from a well-run service. EVIDENCE: It was evident from discussion with the staff and members that the management of the home is being effectively undertaken. A member expressed confidence that members’ views were included in the running of the home. Staff expressed full confidence in the Registered Manager and felt that they were able to approach him with any issues or problems and they would receive a very positive response. Policies were not reveiwed at this inspection, but staff showed that they knew where they were and could access them freely. Health and Safety is generally properly promoted. The Inspector advised on some matters for example, bedsides, a slightly loose stair carpet, and the safety of the a free standing radiator, and is confident that the Registered Manager will follow these issues up effectively as discussed.
