Conditions of registration: Date of last inspection First Inspection Brief Description of the Service: The home is a large detached house. It is surrounded by large well-maintained gardens. All areas of the home and garden are accessible to the service users. There are a number of communal areas including a large. All bedrooms have en-suite bath or shower room. The service users have shared access to a large log cabin within the grounds of the home next door, it has a sensory room and an activity room. The home provides long term care for adults with a learning disability. Transport to access the community is also provided. This is an overview of what the inspector found during the inspection. This visit was unannounced and was the first visit since the registration of the home. The manager was present throughout and 2 senior managers visited during the visit. Information for this report was gathered before and during the visit. Information was gathered by reading records, speaking with service users and speaking with staff. What the service does well:
Detailed assessments of need provide valuable information assisting staff to meet the needs of the residents. Risk assessments in place ensure that service users are able to lead a full and valued lifestyle taking necessary daily risks. Positive relationships between staff and service users and detailed care plans ensure that social, emotional and physical needs are met. The manager promotes positive working relationships with both the staff team and the service user group. He has gathered valuable information within the assessments of need and developed a service user plan from this. Details of likes and dislikes are documented. Files relating to the service users are kept up to date. Family and friendship contact is supported and encouraged, each file examined had a list of friends contact numbers and addresses. Service users rights to privacy and choice are respected and the inspector observed this on the day of the inspection. Complaints are taken seriously and documented. There is a complaints procedure in place. Fire alarms, lighting and extinguishers are checked weekly. The operations manager completes unannounced visits and seeks the opinions where possible, of the service users during these visits. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 Quality in this outcome area is good. Detailed assessments of need provided valuable information assisting staff to meet the needs of the residents. This outcome has been made from evidence gathered before and during the visit to the service. EVIDENCE: Each of the 2 service users’ files examined had a completed assessment of need, both of the files had detailed information from the previous placement and a community care assessment and review. There was evidence of regular professional reviews taking place. The service user spoken with was aware of his file and his right to access it and it was clear that the manager and staff had worked hard to involve the service users as much as possible with their care file. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,9 Quality in this outcome area is good. Risk assessments in place ensure that service users are able to lead a full and valued lifestyle taking necessary daily risks. This outcome has been made from evidence gathered before and during the visit to the service. EVIDENCE: The service user spoken with stated that the staff support him to make decisions and to take risks, this was evident when reading his care file. Service users are supported to continue contact with friends and family. Each of the files examined had risk assessments in place to meet the needs highlighted in the assessments. There was a missing persons procedure in place and service users who are at risk of wandering have an individual risk assessment on place. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15,16,17 Quality in this outcome area is good. Participating in a variety of activities within the community ensures that service users become valued members of society. This outcome has been made from evidence gathered before and during the visit to the service. EVIDENCE: The inspector spoke with 1 service user who confirmed that he participated in a number of activities. Information within the files evidenced a range of activities and likes and dislikes of the service users. The member of staff spoken with stated that they try to involve service users within the local community as much as possible, with visits to the local pub, library, supermarket and again this was confirmed by the service user spoken with. Service users are offered the opportunity to walk to the local shop daily.

The service user spoken with stated that they decide menus between them and then do the shopping between them, as well as being involved with the preparation of the food where possible. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20 Quality in this outcome area is good. Positive relationships between staff and service users and detailed care plans ensure that social, emotional and physical needs are met. This outcome has been made from evidence gathered before and during the visit to the service. EVIDENCE: It was evident on the day of inspection that service users are treated with respect. Staff knock on doors prior to entering. The manager stated that staff make appointments for service users when necessary and take them to the appointments. It was evident in service users files that staff meet the needs highlighted in the assessments. The home keeps a record of all medication received and administered. Medication was seen to be stored appropriately. The manager provides staff with an induction into safe administration of medication, 3 staff have received formal medication training.

The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22,23 Quality in this outcome area is adequate. Policies were in place in respect of Adult Protection, however there is a potential for service users to be at risk of abuse due to the lack of staff training in this area. This outcome has been made from evidence gathered before and during the visit to the service. EVIDENCE: The service user spoken with was confident that staff listened to him and addressed any issues he may have. The complaints record was not examined on this occasion. Training records examined evidenced that three members of staff had received training in NVCI (Non violent physical crisis intervention) to assist them with the management of any aggressive outbursts, however a number of staff still need to receive this training. Two staff had received training in Adult Protection and it was planned for a further 8 over the next four months, training for a further five is still to be planned. Service users are supported by competent and qualified staff. Service users are supported by an effective staff team. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32,34,35 Quality in this outcome area is adequate. Lack of staff training potentially puts service users at risk. This outcome has been made from evidence gathered before and during the visit to the service. EVIDENCE: Staff were aware of their roles and responsibilities. All staff observed on the day of inspection worked very well and positively with the service users. Three staff files were examined, each contained 2 written references, a completed job description and a Criminal Records Bureau check (CRB) obtained prior to commencing at the home. The staff training records evidenced that a number of staff still need to complete mandatory training; Adult Protection, manual handling, First Aid. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37,39,42 Quality in this outcome area is good. A suitably qualified manager with support from senior management ensures that the service changes with the needs of the service users. This outcome has been made from evidence gathered before and during the visit to the service. EVIDENCE: The manager has relevant experience and the necessary skills to manage this service. He works with an open door policy and service users spoke with on the day of the visit were confident about approaching him with any problems they may have. There are detailed policies and procedures in place to safe guard service users. Staff are aware of these and able to access whenever necessary. The Operations Manager completes monthly, unannounced monitoring visits, in addition to other visits throughout the month. During these visits she seeks opinions of the service users and documents them.

Fire drills are completed 2 monthly. Accident records are clear and up to date. Fridge and freezer temperatures are completed daily and within safe limits.
