The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The residents looked very well cared for, one relative visiting the home said, "mum is always nice and clean, she is very well cared for, the staff here are tremendous". All of the residents spoken to at the inspection said that staff were kind and caring. There was a range of activities taking place at the home that catered for most people. There was a positive atmosphere and it was evident that there was good relationships formed between the staff and service users. The manager made herself available to all visitors, and it was obvious that the residents were familiar with her being around and working as part of the care team. There were written plans in place for each resident. This helps staff make sure that each resident gets the support and assistance that is needed for them to live safely and comfortably. Residents said they enjoy the food and there was a choice of meals available.

What has improved since the last inspection?

The statement of purpose has been updated and a new homes brochure has been produced. The requirements and recommendations that where made at the last inspection have been actioned, resulting in staff now being trained properly in such things as moving and handling service users, the protection of vulnerable adults and infection control. The manager is now making sure that people are being properly checked before they start work at the home. A quality assurance system has been introduced to the home to make sure that people are happy with the service they are receiving. Since the last inspection work has been undertaken on the fire alarm system, this was extensive work that involved all areas of the home.

What the care home could do better:

The management team agreed that the environment of the home is looking tired and in some areas in need of total refurbishment, this must be given priority.

One named individual who is under the age category is allowed to reside in the home. 4th July 2005 Date of last inspection Brief Description of the Service: Dinsdale Lodge is a large Victorian building, which has been extended and redesigned to provide a two storey residential care home, with nursing, for up to 28 elderly frail users. The home has twenty two single and three double bedrooms. All the rooms are very individual in shape, size and decor. The rooms have wash-hand basins but no en-suite facilities. There is a ground floor dining room, a smoking and non-smoking lounge, and a small quiet room that can be used by visitors. There is a small passenger lift, which can take a single wheelchair, between the ground and first floor. There is flat access into the home and parking on site. The home is in a pleasant residential area close to local bus routes. This inspection was carried out over a period of 4 hours on the 19th January 2006. The plan for the inspection was to check whether the home had implemented the requirements and recommendations made at the previous inspection; to talk with the residents about living in the home; to meet with care staff and the home’s management team; and to look at records. In addition the visit was also to follow up on the outcome of a strategy group meeting regarding a service user at the home. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1&3 Potential service users are given information about the home in order to make an informed choice. Assessments of all new residents takes place prior to them living at Dinsdale Lodge. This is to ensure that individual care needs can be met within the service. EVIDENCE: The assessment documentation and recording of a new resident was good and in sufficient detail so as to make it possible for the care needs to be identified and from which a comprehensive care plan has been developed. The manager confirmed that she visits all new residents prior to admission, and takes into consideration the equipment they require and the dependency of the other residents living at the home and the bedroom available with regard to location and size of room. A new home’s brochure has been produced and is used as the statement of purpose. This is given to all prospective service users and contains all relevant information in order for prospective service users make a choice about living at the home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 Good systems are in place to ensure that health care needs of the service users are met. EVIDENCE: All residents have a plan of care. The care plans of four residents were inspected and found to be of good quality with careful and thoughtful strategies to address particular needs or problems. There was evidence of involvement of specialist healthcare people such as the community psychiatric nurse and the dietician. During the inspection the community rehabilitation team were visiting the home to assist a service user with their mobility. Staff confirmed that they are keen to make sure that residents maintain as much mobility as possible and support the service users carry out walking practice in conjunction with the rehabilitation team. A visiting doctor said “the staff are really motivated, and interested in what’s best for the patients living here. Whenever I visit the residents are always clean and tidy” Dinsdale Lodge Nursing Home DS0000000164.V255634.R01.S.doc Version 5.0 Page 11 A nurse was observed giving out the lunchtime medication. This was done sensitively and professionally and was in keeping with good practice on the administration of medication. The medication record charts were correctly filled in and a stock check of one resident’s medication was done and was correct. From observation staff were treating service users with dignity, care was given discreetly and sensitively in order to maintain service users privacy. One service user was very poorly and a member of the care staff was sat with the service user. She said “ matron does not like anyone to be on their own when they are so poorly we always look after all our poorly residents like this” Dinsdale Lodge Nursing Home DS0000000164.V255634.R01.S.doc Version 5.0 Page 12 Daily Life and Social Activities The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 &15 Residents living at the home are supported to have choice and control over how they choose to live. EVIDENCE: Residents confirmed that they could have as many visitors as they like and at any time they chose. The visitors are made to feel welcome during their visits and there were several visitors coming and going during the inspection. The home employs an activities co-ordinator to organise activities for the people living there. On the day of the inspection service users were playing bingo, they said they really enjoyed this and the “prizes are always useful” One resident said they had plenty of things to occupy their time but most of all liked watching television and reading in their own room. Residents said they really enjoyed the food, one service user said, “you can have anything you fancy.” One care assistant was observed carrying four bowls of food. The bowls contained food that had been pulped together into brown sludge. When asked what was in the bowls the care assistant said it was mince pie and potato and vegetables it looked most unappetizing. She said this was the lunch for the residents who needed help with feeding. If service users require their food to be pureed it should be served on a plate in separate components. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18 Residents concerns are dealt with. Service users are protected and their interests are safeguarded. EVIDENCE: There are adequate written policies and procedures in place to deal with complaints and the care staff spoken to confirmed they were aware of these. Staffs knowledge of this ensured that they were able to address any issues or anxieties of the residents, relatives and visitors to the home. Residents said they knew who to complain to if they were unhappy about anything. A relative said that she would complain directly to the manger if she was unhappy about any anything at the home she said the manager is “always around and very approachable, we have no complaints about mums care” Staff training has taken place in the protection of vulnerable adults in abuse. Staff gave a clear account of what action to take should they believe a resident was being abused, and were clear on different types of abuse that may occur in a care home. Staff recruitment procedures were adequate and staff were employed and deployed following appropriate CRB and POVA checks. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,25 &26 EVIDENCE: Since the last inspection extensive work has been carried out on the fire alarm system at the home. This has now been completed and a redecoration program for some communal areas has been identified as a matter of priority. The manager and administrator of the home agreed that some areas of the home are tired and in need of redecoration and refurbishment, some resident’s bedrooms have been redecorated and were nice and bright. Identified at the inspection was a carpet in a service users bedroom that either required repair or replacement because it was worn and if left would become a tripping hazard. The home was exceptionally clean and the residents said it was always spotless and clean and that there bedrooms were cleaned and dusted regularly. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 & 30 People are cared for by competent staff in sufficient numbers to meet their needs. EVIDENCE: Staff were deployed in sufficient numbers and there was a skill mix of nursing staff to reflect the needs of service users in the home. Residents said they were well looked after by the staff at the home. The home had staff files in place which provided evidence that the appointment of a new staff member is made through proper recruitment processes. This includes the vetting of staff through the use of references, POVA first checks and Criminal Record Bureau (CRB) checks. All registered nurses have their personal identification number (PIN) validated with the Nursing and Midwifery Council. All new staff receive induction and this was documented in the staff files examined. Staff training has taken place in the protection of vulnerable adults, safe moving and handling of residents, and fire awareness. A training matrix has been developed for all staff. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33 The views of people living in the home are actively sought and used as part of the home’s quality assurance system. The home is well managed and as far as possible people health, welfare and safety is safe guarded. EVIDENCE: The manager of the home is a first level registered nurse, who is experienced in managing a care home. Staff, residents and visitors to the home said the management team were always approachable and helpful. Quality assurance systems are in place. This helps the management make sure that they are providing a good service and identify areas that could be improved. The system includes service users and relative’s surveys.
