Dunkley. The home is registered to provide personal care for up to 46 Older People on a permanent basis. The premises consist of 6 self-contained units with lounge/dining/kitchenette, toilets and bathing facilities. 1 unit provides care for people with Dementia. The home has a central lounge area with bar and main meals are provided from a central kitchen. There are garden areas for each unit and a central courtyard garden. The Home is situated in a residential area, close a post office and newsagents, mini-supermarkets, public house, chemist, and health centre. This is an overview of what the inspector found during the inspection. The Inspection was unannounced and took place over 5 hours. Seven staff, 11 Residents and two visiting relatives were spoken with. Positive written comments were also received from 12 Residents and 3 visiting Professionals. A partial tour of the premises took place and care and other records were inspected. What the service does well: What has improved since the last inspection? What they could do better:
Written guidance for staff on how to deal with the behaviours of Residents suffering from Dementia must be improved to give detailed instruction on the support and strategies needed. The toilet areas must be improved to allow a hoist to be used for disabled Residents. Improvements to the systems for the safekeeping of Residents moneys and valuables must be made to ensure Residents have are able to draw sufficient money at all times and that valuables held are fully recorded. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 3,4,5, Prospective Residents are provided with information to enable them to make informed choice regarding their placement. The pre-admission assessment is thorough and effective in ensuring that the needs of people admitted to the home can be met. EVIDENCE: The admission process ensures that all prospective residents are visited and assessed by staff from the Home to ensure their needs can be met. Residents and their relatives have opportunities to visit the Home and are given information on the services and facilities. Residents spoken with felt that staff were well briefed on their needs and the care to be provided. Staff spoken with felt that they were provided with good information on their Residents needs, routines and wishes. Individual records are kept for each of the Residents and inspection of the records showed that the assessment process was thorough, specific assessment tools were used to identify needs and risk and assessments were carefully documented. Contracts are provided to all Residents, these are currently being revised to give fuller information.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10. Good progress has been made on documenting Residents care needs and providing detailed instructions for staff on how the care is to be provided. Further development of the care plans for Residents with Dementia is needed to provide guidance for staff on strategies for managing behaviours. EVIDENCE: Individual plans of care are available for all Residents. Health and personal care needs were clearly documented. The level of instruction provided to staff on how the care is to be carried through has been increased and was reflective of Residents wishes and preferred routines. Staff spoken with felt that the plans gave them good guidance and that they were kept informed of any changes in needs. The Home has a specific Dementia Care Unit with a dedicated staff team to provide continuity, consistency of care and supervision and monitoring. Progress has been made in the development of Dementia Care plans. Needs are identified and personal life histories are gathered to aid understanding of the individuals’ needs. Whilst broad instructions and guidance for staff were recorded it was not always clearly documented how the Carers are to respond to individual behaviours. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14, 15. The meals in the Home are good, offering choice and variety and catering for special dietary needs and individual likes and dislikes. Residents preferred routines are respected and their social and recreational interests and needs are met. EVIDENCE: A number of Residents were spoken to and everyone who commented on the food said it was good, they had choice and their special dietary needs and likes and dislikes were catered for and respected. Residents are asked to comment and offer suggestions on the menu and changes are made accordingly. The midday meal was efficiently served, nicely presented and of good quality. Staff helped Residents with their meals where necessary. Residents felt routines were relaxed and flexible and that their lifestyle preferences were respected. They felt that they were free to decide on how and where they wished to spend their time and that they were enabled to be as independent as possible. The Home has an activities programme for each of the Units. Currently meaningful activities are being further developed for Residents who have Dementia. Residents felt that staff made time to assist them in the pursuit of individual interests and to talk with them. The Home has an open visiting policy. Visiting Relatives commented that they were made welcome, extended hospitality and that staff made time to discuss

Residents confirmed that they were enabled to receive their visitors in private if they wished. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 17, 18 Systems are in place to protect Residents from abuse and to ensure that complaints are listened to and acted upon. Resident’s rights are protected. EVIDENCE: Residents confirmed that they had been given the Home’s complaints procedure which is also displayed in the Home. Residents and visiting Relatives spoken with felt confident and able to raise any issues or concerns with staff. A complaints record is maintained. No complaints have been received by the CSCI in the last year. The Home has a procedure for the Protection of Vulnerable Adults. Staff spoken with had received training on Elder Abuse and showed that they would react quickly and appropriately to any allegations. Senior staff have responsibility for the reporting procedures to the Authorities. Residents are supported to vote and postal votes are obtained. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19-26. The Home was safely maintained, well kept, warm and comfortable. The Toilet areas are too small for the use of a hoist and therefore the choice and dignity of disabled Residents is compromised. EVIDENCE: Since the last Inspection there has been considerable renewal of furniture and carpets and some redecoration work, the Home is comfortably furnished to a good standard and is safely maintained. The fitting of replacement cupboards in the main Kitchen remains outstanding from the last Inspection. All the toilet areas in the Home are very cramped and a hoist cannot be used. Residents needing hoisting have no choice but to use commodes in their rooms. Residents are enabled to personalise their rooms as they wish and have their furniture and belongings around them. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28, 30 Procedures for the recruitment of staff were robust and provided safeguards to offer protection to people living in the Home. The deployment of staff to individual units ensures continuity and consistency of care. EVIDENCE: Residents spoken with said that the staff were very kind, committed and caring. Relationships between staff and Residents were good. On daytime shifts 1 care staff member is deployed to each unit with responsibility for monitoring and supervision. Currently the Home has two vacant units so additional care staff act as “floaters” and help out in all Units as necessary. Staff responded quickly to call bells and Residents requests for assistance. During the afternoon staff devoted time to conversing with their residents, both on a group and individual basis. The sample of two staff members records inspected showed that the necessary checks and references had been obtained. Training on specialist areas is provided and staff are encouraged to undertake National Vocational Qualifications. Staff spoken with showed a commitment to the well being of their Residents and knowledge of the Home’s aims and objectives and policies and procedures. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33. 34 38. The Management of the Home is effective and accessible and responsive to the needs of both the Residents and staff. The systems for moneys and valuables held for safekeeping need further development to ensure Residents have access to their monies at all times and valuables are appropriately documented and receipted. EVIDENCE: Staff spoken with felt that the Manager was easily accessible to them and was willing to discuss any issues and guide them in practice. Supervisions systems were in place to ensure that staff receive guidance and support. Residents felt the Manager was readily available to them. They commented that regular Residents meetings were held and that the Manager also sought their individual views. Residents felt that their opinions were listened to,

The system for safekeeping of Residents moneys must be reviewed, as staff do not have access to the system in the Manager and Administrators absence. At these times only small amounts of money can be withdrawn from the petty cash system. Items held for safekeeping, such as bankbooks were not documented or receipted.
