Conditions of registration: \none Date of last inspection 2 June 2005 Brief Description of the Service: Fairfield Residential Home is situated on the outshirts of Oxford and can be easily reached by bus, taxi, or for the more energetic on foot. It is also within walking distance of Summertown, a small shopping area. Fairfield is currently registered for 30 elderly residents. The house is a large town house with exceptionally large well maintained grounds and gardens. The house has been extended over time to provide a majority of single bedrooms most with en suite facilities. The internal decor is of a high standard and the home provides spacious accommodation. There is a small passenger lift to the first floor, and the residents have the use of two small kitchens. This is an overview of what the inspector found during the inspection. This was an unannounced inspection that took place on Tuesday 30th August 2005. The inspector had a tour of the building and gardens: the home was bright clean and tidy and the gardens were well maintained with an abundance of vegetables, fruit trees and shrubs. A majority of the time was spent talking with residents who made very positive comments about their home and its management and individual time was spent with the manager in discussion and viewing paperwork in relation to the health and safety of the residents. Water temperatures were taken in three bathrooms and records of complaints were seen. What the service does well: What has improved since the last inspection?
The plumber has pre set all valves in the en suites of resident’s bedrooms to ensure their safety from high water temperatures. Plans are in hand to re tarmac the driveway and this is due to start on 31st August 2005. The Fairfield guide has been revised to incorporate the new bedroom and other facilities within the home.

This will be in addition to the current gardener’s time. All medication is now securely locked in the medicine cupboard when not in use. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 10 All staff respect the privacy and dignity of their residents. EVIDENCE: Residents confirmed that staff were kind, gentle and courteous, respected their wishes and respected their privacy and dignity. One resident was clear that she was always consulted about her care, that she wished to remain as independent as possible and this was respected. Staff were seen being sensitive to residents’ needs, knocking on doors before entering and addressing residents by their preferred title. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 &18 Residents know how to complain if necessary, but the home needs to record all complaints whether verbal or written. The home makes sure that residents are protected from abuse. EVIDENCE: The home has had no written complaints since the last inspection. Residents confirmed they were more than able to voice any concerns, and they were confident that any concern would be listened to and acted upon. Residents also confirmed that the manager was always available to speak to residents or their families. During the time spent in the manager’s office, many residents called in to speak with the manager. It was very much an “open door” policy. The inspector has recommended that all areas of complaint whether verbal or written are recorded in some format, and the manager has agreed to incorporate an issues log to highlight anything that is brought to her attention. The manager confirmed that all staff receive training on the protection of vulnerable adults and this training is regularly updated. They also have a video as part of induction, which further highlights this area of practice. All staff receive a copy of the Oxfordshire Multi Agency Codes of Practice for the prevention of abuse. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 21,22,23&25 Residents have comfortable and pleasant bedrooms and the necessary equipment to maximize their independence. There are sufficient bathrooms and washing facilities for all residents. There is still an area raising concerns for the safety of some residents. EVIDENCE: Several bedrooms were seen and they reflected the owner’s personality with their own possessions and pieces of furniture. All residents spoken to praised the standard of cleanliness in their bedrooms. Most bedrooms have en-suite facilities and there are bathrooms and shower rooms in close proximity to bedrooms and communal areas. There is a small passenger lift, handrails, assisted baths and toilets to enable residents to move around the home safely.

The manager contacted the plumber whilst the inspector was on site, and this is to be rectified immediately. There needs to be a more vigilant system of checking water temperatures on a regular basis. The inspector would like to have sight of the recordings of the temperatures in the new annexe. JUDGEMENT – we looked at outcomes for standard(s) 29 The manager operates a thorough recruitment procedure thus ensuring the protection of the residents. EVIDENCE: Fairfield House have not had any new staff since the last inspection, therefore staff records have not been assessed. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for standard(s) 38 The health, welfare and safety of residents and staff are promoted and protected. EVIDENCE: The fire log, servicing records, water temperatures, gas and electrical tests were all found to be up to date, complete and stored safely. The environmental health inspection report on the kitchen was seen and most of the requirements from that visit have been completed. The manager is putting in a request to the committee for a full time chef who will take on the full responsibility of the food safety and hygiene in the kitchen.
