This inspection was carried out on 10th February 2006.

What the care home does well

The home continues to provide a good standard of accommodation for the residents. A particular strength of the home is enabling residents to meet their full potential by encouraging and supporting residents to access a wide range of Educational, Social and Leisure activities and promoting the development of residents` daily living skills.

What has improved since the last inspection?

The home has made significant improvements to the home care planning system with the development of Person Centred Planning. The home has been completely redecorated and carpet throughout the home replaced.

What the care home could do better:

Accommodation is offered in a family style house in keeping with other houses in the locality. There are two bedrooms, bathroom, toilet and office on the first floor. The ground floor consists of the third bedroom, small laundry, toilet, kitchen and lounge/diner. Two of the service users have lived at the home since it opened in 1999 and spend their weekdays independently at various day centres and have the opportunity to access facilities in the community. This was a routine announced inspection that lasted for five hours. The inspection process included discussions with the manager, deputy manager, one resident, a tour of the building and examination of a sample of policies procedures and records. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2, 4, 5 The home operates a thorough responsible admission process which ensures the home can meet individual needs. EVIDENCE: Care plans evidenced that residents are carefully assessed and given ample opportunity to visit the home before making a decision. Two residents have been in the home since it opened in 1999 and the third resident admitted in September 2004. Each resident has a comprehensive terms and conditions of residence. However the home has still not received a contract from the sponsoring authority even though several representations have been made by the home. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6-9 The home demonstrated that residents are involved in all aspects of their personal care and daily living activities. EVIDENCE: The deputy manager has completed Person Contract Planning (PCP) for two of the residents and is working with the remaining resident in completing their PCP. The deputy manager felt that the introduction of PCP’s have proved a positive experience for the residents. One resident’s PCP was examined in detail. It contained comprehensive details of how the resident wished to be cared for and their aspirations for living a full and active life. Each resident has an informative diary in which the care staff documented how residents had spent their day. Comprehensive risk assessments were in place which demonstrated each residents perception of the risk and how the risk could be reduced by the residents themselves and detailed any staff support required. The home is commended on its efforts to improve the home’s care planning system. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11-17 Residents are encouraged and supported to live a full and active life and to develop to their full potential. EVIDENCE: One of the strengths of the home is the encouragement and support given to residents to live a full and active life. Residents access a wide range of educational, social and leisure activities. Two residents attend work placements. Residents are encouraged and supported to improve their daily living skills. Residents’ personal relationships are encouraged and visitors are welcomed at any time. Two residents have been provided with key to the home’s front door and to their bedrooms. Residents are involved in choosing the home’s menus and are involved in the shopping and preparation of food. The home encourages a healthy eating diet. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18-20 Residents’ personal and health care needs continue to be consistently met by the home. EVIDENCE: Residents are supported and encouraged to manage all aspects of their personal and health care needs. Only one resident is on medication which is administered by staff who have received training in the administration of medication. All residents are registered with a GP and are supported to access all the relevant community health facilities. JUDGEMENT – we looked at outcomes for the following standard(s): 22-23 The home has appropriate policies, procedures and staff training for responding to residents concerns and protecting them from abuse. EVIDENCE: The home has appropriate policies, procedures and practices in place to ensure residents concerns are appropriately responded to. All staff have received training in the protection of vulnerable adults. Service users live in a homely, comfortable and safe environment. Service users’ bedrooms suit their needs and lifestyles. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24-30 The home provides a safe, well maintained homely environment which meets residents’ needs. EVIDENCE: Fairmead Lodge is an end of terraced house, which is in keeping with other houses in the locality. A small well maintained garden is located to the rear of the house. Residents have their own bedroom, which have been personalised to each resident’s taste. The resident who was in the home on the day of the inspection showed the inspector their room and expressed their satisfaction with their accommodation. He explained he was going out to buy a new television set at the weekend. There are toilets on the ground and toilets and a bathroom on the first floor. There is a small accessible laundry on the ground floor. Since the last inspection the home has been decorated throughout and new carpet fitted. Communal space consists of a large lounge/diner and a kitchen. The home is well furnished and maintained. It was clean, tidy and odour free. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31-35 Staff levels and skills are appropriate to meet the needs of the service users. EVIDENCE: Staff rotas indicated that agreed staffing levels continue to be maintained. Staff records indicated a wide range of training undertaken by staff including, the deputy manager training at NVQ level 4, one staff undertaking a degree in Social work, one member of staff has achieved NVQ level 3. Staff induction programmes were seen to meet the Learning Disabilities Award Framework. The resident who was in the home during the inspection commented that the “staff are really good and helpful”. Examination of the latest member of staff recruited demonstrated that robust recruitment practices are in place. Service users are confident their views underpin all self-monitoring, review and development by the home. Service users’ rights and best interests are safeguarded by the home’s policies and procedures. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37-43 The home continues to be well managed by the proprietor/manager and the deputy manager. EVIDENCE: The proprietor/manager has managed the home since its opening in 1997. She holds City & Guilds 325/3 Advanced Management in Care and has extensive experience with the client groups. Residents are constantly involved in the day to day management of the home. The home continues to develop quality monitoring systems with questionnaires sent out to all residents and staff. Management is aware of the further developments required. A random sample of policies, procedures and records were found to be well maintained. Records are securely held in the homes’ office. Accurate records of residents expenditure are maintained. Current safety certificates for services and equipment were in place and records maintained for testing the homes fire precaution systems. There was no evidence to support that the home is not financially viable. YES STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1. Standard YA39 Regulation 24 Requirement The Registered Person must implement and maintain an effective system for monitoring the quality of the service delivered at the home. A copy of these findings are to be sent to the CSCI regularly. This is a repeat requirement. Timescale for action 31/03/06 RECOMMENDATIONS These recommendations relate to National Minimum Standards and are seen as good practice for the Registered Provider/s to consider carrying out.
