Conditions of registration: Date of last inspection 21st July 2006 Brief Description of the Service: Falstone Court Nursing Home provides accommodation and nursing care for 41 older people with a dementia type illness. Falstone Court opened as a purpose built home in 1995 and is next door to a sister home Falstone Manor. A General Manager oversees both homes and is based at Falstone Manor, as are personnel records and financial information. The Home has two floors with communal rooms available on both floors. All areas within the Home are accessible to people with mobility difficulties. Ensuite toilet facilities are available in the bedrooms and toilets and bathrooms are available throughout the Home. The home is on the sea front and can easily be seen when driving along the sea front main road towards the city centre. A car park is provided and the home is accessible to wheelchair users. The scale of charges for the home starts at £379.00 per week. Other charges relating to continuing healthcare and self-funding are available in the home. This is an overview of what the inspector found during the inspection. The site visit for this inspection took place over one day on 24 August 2007. One inspector carried out the inspection. The General Manager of the service was present during the site visit as well as the unit manager. The views of people living at the home were gathered during the day of inspection, by talking to them, their visitors and by reading documents. Positive feedback was received from all of those whom the inspector spoke to. The manager was interviewed and documents were examined, including care plans. A tour of the home took place. A mealtime was observed but the inspector did not sample the food at this inspection What the service does well:
The home provides good clinical care and there is good liaison with clinical specialist and other health professionals. The unit manager is very well organised, staff morale is good and the staff respond well to his style of management. Service users, and their relatives, feel that they are well looked after by the staff and speak highly of the care given in the home. Staff members engage service users in conversation when carrying out care and staff members put elderly service users at their ease. The manager and her staff are flexible in relation to service user’s individual needs and wishes and try to ensure the service responds to the individual’s needs and wishes. Lifestyles are tailored to service users preferences as well as possible taking into account the service users capacity and abilities. The activities available to service users are tailored to individuals requirements and staff provide a wide ranging social and activities programme that includes outings in the home’s own transport. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. This judgment has been made using available evidence including a visit to this service. All service users entering the home have their needs accurately assessed by social care and health service staff. The home makes it’s own assessment in addition to this. Most service users and their family have the opportunity to visit the home before they come to stay, unless there are exceptional circumstances. This allows them to make an informed choice about staying in the home. EVIDENCE: The provider’s self-assessment was not available in advance of the site visit. Previous reports have indicated the information, provided by the home, is good and that service users find it helpful. Assessments are in the Southern Cross style and are very comprehensive, covering social and recreational needs as well as physical requirements. Care

Service users relatives were happy with the information they received. Qualified staff were observed giving clear and accurate information to service users relatives. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. This judgment has been made using available evidence including a visit to this service. Service users health care needs met. Service users have access to all healthcare services that they need. The home has appropriate policies and procedures to ensure service users receive their medication in a safe way EVIDENCE: The staff, who were spoken to have a good awareness of service users social recreational and clinical needs. The atmosphere on the unit is very calm. Medication is stored safely. A monitored dosage system is in use and recording is good. Stock balances are correctly kept and checked daily. Care plans are comprehensive and well written. The format of these has change in line with Southern Cross policy. The format is well laid out and the examples seen were completed to a good standard. Pressure damage risk was approriately assessed and all risk assessments being reviewed monthly. Service user’s care plans contain much useful clinical information. Some of the life histories were well recorded and detailed in most cases. Reviews are recorded and relatives views on the care are contained in these. All of these comments were positive about their relatives care experience.

Discussions with staff revealled that they have a high commitment to the service user’s welfare. The Liaison nurse from the Hawthornes was visiting during the site visit and she confirmed that the home had a productive and professional relationship with herself and the hospital. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is excellent. This judgment has been made using available evidence including a visit to this service. The home endeavours to provide a lifestyle that matches service users needs and preferences. Meals are of good quality and the timing and choice can be varied to meet service users needs EVIDENCE: The unit caring for dementia has been refurbished to provide corridors that look like old streets, complete with shops, a pub and a café. Cream teas are provided in the café on two days during the week. The was seen during the site visit and was much enjoyd by staff, visitors and service users. Corridors have wall hangings that are sensory (they can be touched) based on differing themes. The manager feels this environment gives some stability and strikes a chord with the service users. She feels this contributes to the calm atmosphere and helps the service users feel more “at home”. The care workers now have responsibilities for some social programmes and activities and there is an enthusiastic activities co-ordinator. Staff deal with relatives, in person, and in telephone enquiries, appropriately. The cook plans the meals using the Nutmeg programme to ensure good, balanced nutrition. A number of service users and their relatives commented

It was observed that service users were offered alternative choices if they did not want what was offered. The quality and presentation of food was good. Visiting is unrestricted and visitors were seen coming and going throughout the day. The home does have a dedicated bus. Service users can use the garden if the weather was suitable and were observed moving in and out of the sheltered patio areas. This area has raised beds but the quality of the plants are poor. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. This judgment has been made using available evidence including a visit to this service. The homes policy on adult protection reflects the multi-agency approach to adult protection, and local procedures and staff know how to use it. This should ensure the protection of service users. Service users and relatives are confident that the process and the staff will act in the service user’s best interests. EVIDENCE: The homes management has dealt with the issues of adult protection issues appropriately. Complaints are recorded well, ooutcomes are recorded and audited. The General Manager stated she had an open door policy and a weekly “surgery” so that relatives can contact her at any time. She was seen greeting visitors as they entered the home. Visitors and staff displayed a relaxed and friendly attitude towards each other. All of the relatives that were spoken to confirmed that they found the staff helpful and felt that the Unit Manager, and General Manager were very approachable. Service users and relative expressed confidence that staff would deal with any problems they had. One said, “ I can talk to the unit manager about any problems and they are taken up”, another said, “I can talk to any of the staff if there is a problem”

Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. This judgment has been made using available evidence including a visit to this service.The environment is well maintained and safe. The home has most of the equipment it needs to meet service users needs. Rooms are comfortable, furnished to service users tastes, and the home is kept clean. EVIDENCE: All fire precautions were being observed during the time of the inspection. Regular maintenance takes place and this is certificated. The home was pleasantly heated and lighting was at appropriate levels. Moving and handling equipment was available and there were fixed handrails in toilets, bathrooms and showers. The home still does not have an induction loop system for hearing aid users. There was no appreciable odour in either unit other than at the top of the stairs on the first floor. This is aspecific problem that is being addressed. The home is clean and tidy in all areas used by service users. The unit has a series of murals and pictures depicting varoious themes.

There is a wide variety of communal space including wide corridors and rest areas. One lounge has been transformed into a period public house and this is popular with service user’s. There is open access to a sheltered courtyard that has raised beds thatcontain a variety of plants. Rooms were well furnished and clean. Many had personal items and usefull prompts such as wall mounted clocks. A variety of wall decorations are displayed and these have differing textured surfaces to encourage tactile exploration by service users. The kitchen serving areas are clean but the units are worn and have damaged surfaces. These should be replaced. Falstone Court Nursing Home DS0000018192.V348167.R01.S.doc Version 5.2 Page 17 Staffing

The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. This judgment has been made using available evidence including a visit to this service. The manager ensures that the home operates safely and sufficient numbers of staff, who are appropriately trained, are available to meet service users needs. EVIDENCE: The manager confirmed that good level of induction take place and staff records confirm this. Staff themselves confirm they receive regular updates and a good induction and NVQ training. Duty rotas indicated an appropriate level of staff. The service users receive attention promptly and are well supported throughout the day. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. This judgment has been made using available evidence including a visit to this service. The Managers are professional in thier approach and has a positive vision of how the home should be run. Service users and staff benefit from the way they carry out thier duties. EVIDENCE: The home continues to introduce positive changes, such as the newsletter, and a varied social programme. The activities of all grades of staff, to provide entertainment to service users, also fosters teamwork and this is a commendable effort. The Unit Manager is an experienced nurse and the staff respond well to his leadership style. Falstone Court Nursing Home DS0000018192.V348167.R01.S.doc Version 5.2 Page 19 The Manager confirmed that they operate the Southern Cross Quality assurance system and that customer satisfaction surveys are carried out. This was also confirmed by relatives who said they had taken part in “A survey”. The results are fed back to each home. The Manager is aware of her statutory responsibilities, including health and safety, and record keeping is good. She stated that supervision takes place. Records confirm they have regular supervision sessions. The home keeps service user’s personal allowance and has a good and secure system to account for any expenditure. Records were seen on the site visit and they include an audited record and receipts for any purchases. Discussions with staff indicate the moral in the home is good and staff feel motivated and enthusiastic. They confirm the both Managers are approachable and they have confidence in them. Service users and relatives say also that The Manager is approachable. The General Manager confirmed she felt well supported in her role by the senior management team.
