Conditions of registration: Date of last inspection 19th August 2005 Brief Description of the Service: Fern Lodge is a Victorian, three storey semi-detached house close to the Chester city centre. The home is situated close to a main bus route and ten minutes walk from the nearest shops. The house is also within walking distance of the Countess of Chester Health Park. Bedroom accommodation is on three floors. The home does not have a passenger lift and access to all floors is via a staircase. The bedroom accommodation consists of four twin bedrooms and thirteen single rooms and there are en suite facilities in one of the single rooms. There is an external fire escape. Service users primary care needs are due to mental disorder and in accordance with statutory requirements there are Registered Mental Nurses on duty at all times. This is an overview of what the inspector found during the inspection. This short, unannounced inspection looked at standards which were not examined during the detailed inspection on 19th August 2005. A tour of the home was completed, files and other documents examined. Staff and residents were both observed and spoken with. Discussions took place with the Registered Manager. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 4 Potential residents are given an opportunity to ‘test drive’ Fern Lodge before deciding whether to move in. EVIDENCE: Potential residents are usually referred to Fern Lodge through psychiatric services, often via hospital. Residents are offered a ‘progressive admission’ procedure. This means that they can come to visit the home, stay for a meal, then stay overnight and progressively increase the time spent at Fern Lodge until they are sure their needs will be met at the Home. Records are kept of these visits so that should an individual choose to move into Fern Lodge staff already have information about individual need. The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8 and 10 Efforts are made to encourage residents to participate in the running of Fern Lodge. Records and documents are stored appropriately. Both the policies and practices of the Home reflect good confidentiality procedures. EVIDENCE: Resident’s meetings are held at Fern Lodge each month. All residents are able to attend these meetings and are encouraged to contribute to the agenda. A copy of the agenda was on display on the resident’s notice board and they are able to add new items to this. The Registered Manager stated that he was planning to share the minutes of these meetings with residents. It was discussed with the Registered Manager and staff members other ways of getting residents to participate in the running of the home and including a discussion and review of policies and procedures at resident’s meetings was recommended. Residents were observed to take part in the daily routines of Fern Lodge. Residents have responsibility to launder their own clothes and keep their room tidy, although staff will assist if necessary. Some residents participate in

Residents do not currently get involved with staff selection. It is recommended that the manager and staff discuss ways of doing this with residents. Fern Lodge has a clear policy regarding confidentiality and staff were aware of it’s content. Staff sign a confidentiality agreement. Fern Lodge has asked other visiting professionals to sign up to it’s confidentiality policy. Documents and records are stored securely and appropriately. See Recommendations 1 and 2. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11 and 15 Residents are given opportunities to improve life skills and access specialist services. Residents are supported and encouraged to maintain and develop personal relationships. EVIDENCE: Fern Lodge does not provide specific rehabilitation facilities, for example residents do not have access to the kitchen. Residents are encouraged to participate in specialist services outside the Home, for example supported employment. Some residents have an outreach worker, provided by the local psychiatric service, who will do specific rehabilitation work on an individual basis. One resident has recently moved to independent living following rehabilitation work with an outreach worker. Staff at Fern Lodge do work with residents around life skills, for example budgeting, social activities in the local community and household chores. Some residents were noted to have a fridge and microwave in their own room, as well as a kettle, and this promoted their independence.

Some residents attend local church services and a weekly visit and service from a Catholic Lay preacher is popular. Fern Lodge has an open visiting policy. The Registered Manager stated that relatives are encouraged to visit and are able to see residents in private, but often choose to sit in the lounge and chat to everyone. Some residents are in intimate relationships and this is supported by the staff team. One resident has been assisted by staff to obtain appropriate contraception. The Registered Manager stated that discussions had taken place with residents regarding appropriate relationships. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 21 Fern Lodge will support residents who are ageing, ill or dying in line with a comprehensive policy. EVIDENCE: The Registered Manager stated that, where possible, residents remain at Fern Lodge until the end of their life. There had been instances, in the past, when the decision to move a resident to hospice care or to elderly care services had been deemed appropriate in order to meet individual need. However, in general, residents who are ageing, ill or dying are able to remain at Fern Lodge with support from staff and appropriate care services, for example Macmillan Nurses. Fern Lodge has a clear and comprehensive policy with regard to the ageing, illness and death of residents. This enables staff to give residents appropriate information about the long term care they can expect to receive. The Registered Manager described how funerals had gone from Fern Lodge and how he had participated in services for residents in the past. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 23 There is clear information and procedures regarding the protection of vulnerable adults, and staff are fully aware of their role with regard to this issue. EVIDENCE: Fern Lodge has thorough policies and procedures regarding the protection of vulnerable adults. The policy details what is meant by abuse and describes ways of spotting whether abuse is occurring. This means that staff have a clear idea of the issues after reading the policy. The policy goes on to describe a clear and appropriate procedure for reporting and dealing with alleged abuse. There are notices in the Home, both for staff and residents, which advise about what to do if abuse is happening or suspected. The Registered Manager had a clear understanding of the procedures regarding the Protection of Vulnerable Adults (POVA) list. JUDGEMENT – we looked at outcomes for the following standard(s): All standards were looked at during the previous inspection and found to be fully met. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 34 Fern Lodge has clear recruitment procedures and staff are employed following adequate identification and personal checks. EVIDENCE: Staff are recruited at Fern Lodge following appropriate procedures. One new staff member described the thorough checks which had been carried out prior to her starting work, including identification, references, Criminal Record Bureau check and the checking of her Nurses PIN (Personal Identification Number). Residents are not involved in staff selection, although the issue has been raised in resident meetings. The Registered Manager stated that he was planning to look at ways of getting residents involved in this. Once staff are employed they are given an induction pack, which they then work through. This contains a range of information and workbooks which provides the staff member with a clear understanding of their role and expectations at Fern Lodge. The induction pack is based on the Training for Care standards. The use of this pack benefits the residents as all staff are working with the same information and have the same baseline knowledge regarding life at Fern Lodge. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 42 Fern Lodge has good health and safety policies and procedures. Safety checks and risk assessments are in place and these have been combined with individual resident’s risk assessments to ensure a safe environment for all. EVIDENCE: Fern Lodge has clear policies and procedures regarding health and safety issues. Fire safety is prioritised with training, risk assessments, precaution assessments and drills being thoroughly documented. Each room in Fern Lodge has a clear map showing means of escape should a fire occur. Risk assessments have been completed, and are regularly reviewed, for the environment, for example the kitchen. All risk assessments have been done together with individual resident’s risk assessments, for example smoking risk and fire risk, and this has led to a comprehensive analysis of risk. Appropriate documents are in place for relevant health and safety legislation, including Control of Substances Hazardous to Health Regulations (1999), Reporting of Injuries, Diseases and Dangerous Occurrence Regulations 1995, Gas and Electricity Safety regulations.
