Conditions of registration: N/A Date of last inspection 7/3/05 Brief Description of the Service: Fernleaf is a care home providing personal care and accommodation for up to 21 older people and has four beds allocated to care for people with dementia. The home provides short term, long term or respite care and accepts emergency admissions subject to bed availability. The home is owned by Mr. and Mrs. Sooriah and is a run as a family business. The home is located in Mansfield town centre and is close to shops, pubs, the post office and other amenities. The home was opened in January 1985 and consists of a former hotel building with extensions added19 of the home’s bedrooms are single, and 9 of the bedrooms have en-suite facilities. Bedrooms are located on 2 floors and there is a passenger lift. There are 3 bedrooms on the first floor that are accessed by 3 steps which h\as a stair lift fited.The home has an enclosed garden to the rear, which is easily accessible. There is car parking available for up to 8 cars to the front of the home in a small car park, which also houses the homes mini bus. The home has had considerable amount of refurbishment since the proprietors purchased the home in 2003. This is an overview of what the inspector found during the inspection. This inspection was the first of two unannounced inspections to be carried out between April 2005 and March 2006. The inspection lasted for 4 hours and the main method of inspection used was called case tracking which involved selecting 3 residents and tracking the care they receive through the checking of their records and discussing this with them. Other residents were spoken with and additional records were seen. A discussion was had with the manager, care staff on duty and care practices were observed. Other residents were spoken with but no visitors to the home were seen. The premises were not inspected in detail but various areas of the home were visited as part of the inspection What the service does well:
Care plans are completed well and informative. Residents said that their needs were being met and also said that they have had recent eye tests. Where any health concerns are identified staff monitor these using forms prepared for this purpose and use any equipment needed to assist the resident. There is a separate medical room which was well organised and the drug administration records were correctly completed. Residents confirmed that they are given their medication. Residents are helped to be as independent as possible and some go out to the local town centre and there is a monthly church service held in the home. Visitors are welcome at any time. Residents spoke positively about the food describing it as homely. The décor and fabric of the building is good and residents said that there have been many improvements since the new owner bought it. Laundry arrangements are good and residents spoke of their clothes being handled in the way they wished. Staff are trained in safe working practices and the routine safety checks and tests are carried out at the correct timescales. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 and 9 There are good arrangements in the home for meeting the health and personal needs of residents. EVIDENCE: Care plans seen were well-ordered and contained clear directions as to how residents needs are to be met. Plans are based upon the activities of daily living and are reviewed monthly. Staff were familiar with the care plans and residents said that staff knew how to meet their needs. There are systems in the care plans for monitoring the well being of residents and a record is made of all appointments attended and treatment given. Appropriate equipment is available to promote residents well being and staff are provided with training on common ailments associated with old age and were aware of their role in maintaining residents health. Residents referred to recent medical appointments and health checks and one resident said she had just ordered some new glasses following a visit to the optician. The storage of medicines is well organised in a separate medical room. The home uses a monitored dosage system and records of medicine administration (MAR) were clear and fully completed. Some residents self medicate all or part of their medication. Staff are required to complete a safe handling of medicines course and be familiar with the home’s policy on the safe storage,

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 and 15 Residents are able to maintain their independence and follow their individual interests, but there are not enough organised activities provided. There are good arrangements for the preparation and serving of food that are suitable for the residents. EVIDENCE: Residents are encouraged to follow as independent lifestyle as they are able and residents gave examples of assistance they receive for this. One resident pointed to a ramp that had been fitted to enable her to use the garden independently. Some residents had gone out to the local town centre and another resident said she was waiting for an electric wheelchair so she could go out. Residents described various pastimes, including looking after a rabbit and reading; although they said that they would like more activities to be provided. One resident said she had only played bingo three times in the last year and another said she would like to play scrabble. A monthly church service with communion is held in the home and one resident said she goes to a local church on Sundays. Residents said they had appreciated the effort staff had made to mark VE day, including coming into work in 1940’s dress. Visitors are welcome to visit the home at any time and can see residents in private if they wish. A record is made of when visitors have been. Residents are able to bring their own possessions into the home. One resident said that

There is a four week menu which provides a balanced diet and a choice at all meals. Residents said that the food was homely and they had plenty to eat. Drinks are provided at regular intervals and residents can ask for one at any other time. EVIDENCE: The home was in good decorative order, well maintained, clean and fresh. Residents said that the provider has made considerable improvements since he purchased the home. Residents enjoy using the garden, which is tended to by a gardener. The laundry is suitable equipped and there are procedures in place for infection control. A resident said she had asked for her trousers to be ironed in a particular way and this was now being done.
