This inspection was carried out on 26th September 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Service users and their relatives are involved and consulted regarding their care plans. Relationships with the staff and the relatives are good and contacts are well organised and pleasant. The manager and staff are approachable to service users and their relatives. The food at the home is good.

What has improved since the last inspection?

The service users care plans have improved since the last inspection, they now contain more detail and any specific actions, which staff need to take to ensure service users needs can be met.

What the care home could do better:

All reviews on care plans need to be documented. Reviews on service user risk assessments need to be evidenced.

There is ample on road parking and some spaces available to park in the home’s own car park. The gardens are private, well kept and have a pleasant water-fall feature, with many areas to chose from to sit out when the weather permits. The home is split over the ground and first floor, with the choice of a passenger lift and period style staircase. The inspector spoke to staff and service users. Some records were also inspected. These included service users care plans. Case tracking was used which involved the inspection of three service user’s care plans and ‘tracking’ to see if their needs were being met at the home. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8 The service users have care plans in which their needs are clearly set out. The service users health needs are met. EVIDENCE: Each of the 3 service user’s case tracked had files which contained detailed care plans. The plans for meeting the short term and long term needs are documented. The manager stated that monthly reviews of the care plans take place. These reviews however were not documented. There was evidence that both the service user and the relatives had been involved in the drawing up of the plans and they were appropriately signed. The health needs of the service users are documented in their plans and running records are made of any progress/issues. Appropriate staff have access to this documentation at all times. There is evidence that other health professionals are involved as required and notes are kept of all visits and appointments. The manager or staff accompanies Service users to their health appointments. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15 The service users are consulted about their lifestyle and their individual needs are met. Contact with relatives is good. The food is enjoyed by service users. EVIDENCE: Service users are consulted regarding any plans and one recently admitted services user said that they had been able to continue going to a ‘club’, which they enjoyed. One service user and their relative said that arrangements for contacts was good and one service user reported that they were pleased to live near to their family. Some contact information was present in the care plans but could be clearer. Relatives confirmed that the manager and staff were approachable. Menus are prepared a week in advance and with the input of the service users. They are able to order food as required and there is always an alternative should they require this. One service user reported the food to be good. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,38 The manager has a reputation from service users and relatives for being approachable. Self-monitoring takes place and quality assurance is being developed to ensure the home is run in the best interests of the service users. Risk assessments, although in situ should be reviewed regularly to ensure the safety and welfare of the service users. EVIDENCE: Relatives reported that the manager is approachable and the staff and relatives reported that there are good relationships between both parties. The manager is registered with the CSCI. She still has to complete her NVQ 4 in care and management. She plans to complete the award in the near future. The manager and relatives spoken with reported that there is a good relationship between relatives and management and the manager plans to monitor the relatives committee and make any required changes as appropriate. The service users have risk assessments, however one risk assessment was found to be out of date and some important information had not been reviewed since 2003. The risk assessments contained details about moving and handling and the risk of falls.
